On Wednesday, Shamique Martin was one of 9 nurses disciplined by the board. It placed her license on probation for four months and ordered her to take courses in ethics and being a mandated reporter of child abuse.

In February, 2017, Rocky Hill  police arrested her in connection with her daughter’s burns. In September 2017, Martin pleaded guilty to one count of risk of injury to a minor and one count of making a false statement. Martin left her child in her boyfriend’s care even though she knew he was not allowed to be unsupervised with the child, state records show. When alerted about the burns, Martin failed to seek timely medical care and lied to police about how the child was injured, records show.

Rocky Hill police said that Martin’s two-year-old daughter was burned in 118-degree water, and her boyfriend, Michael Shamel Davis, staged a fire scene in a bedroom to make it seem that she had been burned there, the Hartford Courant reported. Martin had agreed in a state Department of Children and Families plan not to leave her children unsupervised with Davis, the Courant said.

In December 2017, Martin was sentenced to nine months in prison and three years of probation. In January 2018, Davis pleaded guilty to risk of injury to a minor and was sentenced to two years in prison and five years of probation.

Martin told the board in January that her daughter’s burns were healed in about a month.

The board also revoked the registered nurse license of Melissa Tackett of Cromwell after finding that in 2018, she forged two prescriptions, using the name of a resident at Westside Care Center, the nursing home where she worked in Manchester, to obtain Oxycodone for herself, a memorandum of decision said. She also used the painkiller to excess, the memo said. Criminal charges are pending against her, records show.

The board also revoked the LPN license of Nicole Miller of East Hartford, who tested positive for alcohol in 2018, just two months after being placed on a four-year probation for alcohol use, a memorandum of decision states.

State records show the board also:

• Suspended the LPN license of Christine DeFusco of Rocky Hill, saying that she violated a four-year probation by failing to submit drug and alcohol test results and failing to take part in therapy. In 2016, she pleaded guilty her third driving while under the influence charge.

• Reprimanded the advanced practice registered nurse license and the RN license of Ellen Sollazzo of Farmington, saying that in 2017, she failed to meet the standard of care by failing to perform appropriate patient psychiatric assessments. She also violated professional boundaries with the patient. Both licenses were also placed on probation for three years.

• Placed the RN and APRN licenses of Ashley Dizney of Southington on probation for four years and required her to pass drug and alcohol tests. In 2017, she paid a $1,000 fine and her license was placed on probation for four years for stealing Fentanyl from patients. In 2018, she tested positive for opiates.

• Approved a two-year license probation for RN Skye Muli of Middletown. Muli must undergo drug and alcohol tests. In 2017, she used alcohol, marijuana, opioids and cocaine to excess.

• Reprimanded and placed the LPN license of Beata Ivana Ventura of Shelton on probation for a year and ordered her to complete a course in ethics. Ventura was accused of stealing nine tablets of Zofran, a drug used to prevent nausea, while working at Greenwich Woods Health Care Center in 2017.

• Placed the LPN license of Joseph Iannicelli of Waterbury on probation for two years and ordered him to under drug and alcohol tests. In 2017, he tested positive for opiates and cocaine.

The board revoked the license of Christine Tracy, a licensed practical nurse, for abusing heroin in 2014. The board’s memorandum of decision said that Tracy of Ansonia was arrested in 2014 after driving on the wrong side of the road and striking two other cars. After being hospitalized, she was found in possession of seven bags of heroin. She was arrested and placed on probation for three years in 2016 after pleading guilty to possession of narcotics and failure to appear in court, the memo said.

The board also placed the RN license of Juliet Clifford of Mount Vernon, New York on probation for four years, its memorandum of decision said. The memo gives the following account: Clifford, who ran a nursing home in Queens, New York, was arrested in 2013 and charged with falsifying records after a 74-year-old dementia patient was missing. She failed to report that he had left and destroyed pages in his medical record to conceal that he had left the home. In 2014, she pleaded guilty to a misdemeanor violation of New York health laws, lost her New York nursing license and was placed on three years of probation.

Connecticut law allows the nursing board to discipline nurses with Connecticut licenses if they have been disciplined in other states.

State records show the board also:

• Imposed a four-year probation on the RN license of Connie-Jo Gibeault of Brooklyn, Connecticut for abusing alcohol in 2016, in violation of an earlier probation.

• Placed the RN license of Sarah Sanders of Middletown on probation for a year in connection with alcohol abuse and mental illnesses that the board found was affecting her ability to practice as a nurse.

• Ordered a one-year probation on the LPN license of Beverly Wilson of Marlborough on probation for one year. She took a narcotic painkiller from Marlborough Healthcare in 2013 for her own use.

• Placed the RN license of Concetta Mancini of the Oakville section of Watertown on probation for four years. In 2006, she was disciplined for abusing Percocet and an anti-anxiety drug from Waterbury Hospital, and she appeared impaired while working between 2008 and 2010.

• Imposed a two-year probation on the RN license of Christina DiSanto of Stratford on probation after she was found to have abused morphine, Xanax and other drugs to excess.

• Placed the RN license of Brian Gross of West Hartford on probation for four years in connection with his abuse of alcohol from 2013 to 2017.

Correction: Dianne Layer-Kelly, a registered nurse from Lindenhurst, New York, was not disciplined in September by the state Board of Examiners for Nursing. This story originally reported that the board had reprimanded her.

The board last Wednesday revoked the registered nurse license of Lisa Fabrizio, who is formerly from Monroe, after it found that she took jewelry from patients and computers from her work at Lighthouse Home Healthcare in Old Saybrook and was trading the goods for heroin, state records show.

In June, she was charged by Stratford police with third-degree larceny after a detective determined she was pawning stolen jewelry, tools and electronics in local shops, state records show.

She is also facing multiple criminal charges in connection with a hit-and-run accident in August, when she told police she had recently used heroin, records show. The board found that her abuse of heroin was affecting her practice as a nurse and that her thefts constituted a failure to conform to the standards of the nursing profession, records show.

The board also revoked the RN license of Adrian Kozikowski of New Britain because, state records show, he left a shift in July at Autumn Lake Healthcare in New Britain and fell asleep or lost consciousness due to drug use.

Between April and July, he abused alcohol, marijuana and Ecstacy, records show.

The board also revoked the license of Dorsey Saunders, a licensed practical nurse from Bridgeport, for failing to comply with the terms of a four-year probation imposed in March 2016.

The probation was ordered after she was found stealing drugs, including oxycodone and fentanyl, while on duty at Fairview of Fairfield, a nursing home in the Southport section of Fairfield, records show.

The board also:

• Placed the RN license of Amanda Alarcon of Shelton on probation for four years after finding that she abused heroin from 2011 to 2014. Records show she is receiving substance abuse treatment.

• Imposed a two-year probation on the RN license of Jessica Carlson of Middletown. Records show that while working at MidState Medical Center in Meriden, she took the opioid Dilaudid for her own use in 2011 to 2012. Records show she also abused Dilaudid from 2011 to 2014, and while working at the Water’s Edge Center for Health & Rehabilitation in Middletown, she took controlled substances and failed to document medical records.

• Reprimanded the LPN license of Akiko Mallett of West Hartford, who, records show, inappropriately physically examined a patient at Arden Courts Manor in Farmington after becoming aware of possible inappropriate physical conduct between an employee and a resident. The facility filed a plan of correction with the state Department of Public Health, and Mallett completed courses on critical thinking skills and the scope of practice for an LPN.

• Imposed a one-year probation and a reprimand on the RN license of Melissa Tackett of Cromwell. Records show that in September, she took 46 capsules of the antibiotic Cephalexin from the Middlesex Health Care Center while working there as a nurse.

• Placed the RN license of Heather Wettemann of Killingworth on probation for four years and ordered random drug tests. Records show that Wettemann has admitted she has a history of abusing controlled substances.

Also, the board dismissed all charges against Dawn Noyce, an RN from Canterbury, because she voluntarily surrendered her nursing license. Records show she has struggled with drug and alcohol abuse and pleaded guilty in April 2015 to driving under the influence of drugs and alcohol.
