In this episode, Dr. Bland reflects on the early questions that sparked his lifelong pursuit to better understand why people become chronically ill. At a time when little research existed around the biology of chronic disease, he became determined to uncover answers that could transform medicine into a more proactive, health-focused model of care.
Dr. Bland also shares how these ideas ultimately led to the formation of the Institute for Functional Medicine in the early 1990s, built on the belief that chronic illness could be addressed through lifestyle, nutrition, and environmental interventions - not just pharmaceuticals. His work helped pioneer a teachable framework that continues to influence the evolution of integrative and longevity medicine today.
Don't miss this inspiring conversation as Dr. Bland explores the origins of functional medicine, the future of personalized healthcare, and why shifting from disease-focused medicine to health-focused care may be one of the most important transformations in modern medicine.

IIn this episode, Dr. Ginsberg challenges the traditional reliance on standard blood testing, explaining how hormone levels - particularly estrogen - can fluctuate significantly and still appear "normal" on conventional lab results. She highlights how these fluctuations can profoundly impact the brain and overall well-being, often leaving women misdiagnosed or overlooked within rigid clinical labels.
Looking ahead, Dr. Ginsberg shares her work in developing innovative, AI-driven solutions that integrate genetics and symptom tracking to better assess individual estrogen sensitivity. This forward-thinking approach aims to move beyond outdated frameworks and provide a more personalized, accurate understanding of where a woman falls within the pre-, peri-, or postmenopausal spectrum.

]]>Metabolic health is deeply tied to mitochondrial function, the "powerhouses" of our cells that drive energy production. When mitochondrial health is compromised, it can lead to a range of issues, from poor energy levels to chronic diseases. Factors such as oxidative stress, poor diet, lack of exercise, and environmental toxins all play a role in mitochondrial dysfunction.
In this episode, we'll explore:
What metabolic health really means and why it's essential for overall well-being.
How practitioners can simplify the concept for patients and emphasize the importance of lifestyle factors like diet, exercise, and stress management.
The role of VO2 Max as a powerful yet underutilized tool for assessing cardiovascular and metabolic health.
VO2 Max, which measures the body's ability to utilize oxygen during exercise, is one of the best indicators of mitochondrial and cardiovascular health. Despite its value, it's not used enough in clinical practice. We'll discuss why this metric matters and how it can be integrated into patient care to provide a more comprehensive picture of health.
Join us for this insightful conversation as we redefine what it means to achieve and maintain metabolic health.

In his New York Times best selling book Fat Chance: Beating the Odds Against Sugar, Processes Food, Obesity, and Disease, Robert documents both the science and the politics that have led to the current pandemic of obesity and chronic disease. In the Fat Chance Cookbook, Robert provides practical examples for applying healthy eating principles with recipes by Cindy Gershen.
Dr. Lustig is a neuroendocrinologist, with basic and clinical training relative to hypothalamic development, anatomy, and function. Prior to coming to San Francisco in 2001, he worked at St. Jude Children's Research Hospital in Memphis, TN. There, he was charged with the endocrine care of many children whose hypothalami had been damaged by brain tumors, or subsequent surgery, radiation, or chemotherapy. Many patients who survived became massively obese. Dr. Lustig theorized that hypothalamic damage led to the inability to sense the hormone leptin, which in turn, led to the starvation response. Since repairing the hypothalamus was not an option, he looked downstream, and noted that these patients had increased activity of the vagus nerve (a manifestation of starvation) which increased insulin secretion. By administering the insulin suppressive agent octreotide, he was able to get them to lose weight; but more remarkably, they started to exercise spontaneously. He then demonstrated the same phenomenon in obese adults without CNS lesions.
The universality of these findings has enabled Dr. Lustig to weave these threads together into a novel unifying hypothesis regarding the etiology, prevention, and treatment of the current obesity epidemic, and the role of our environment in the biochemical changes that promote weight gain. This has led him to explore the specific role of fructose (half of sucrose and high-fructose corn syrup) as a specific mediator of both chronic disease, and continued caloric consumption. His acclaimed YouTube video, "Sugar: The Bitter Truth" continues its popularity with the lay public.
A native of Brooklyn, New York, Dr. Lustig went to Stuyvesant High School in Manhattan, graduated from Massachusetts Institute of Technology in 1976, and received his M.D. from Cornell University Medical College in 1980. He completed his pediatric residency at St. Louis Children's Hospital in 1983, and his clinical fellowship at UCSF in 1984. From there, he spent six years as a post-doctoral fellow and research associate in neuroendocrinology at The Rockefeller University. He has also been a faculty member at the University of Wisconsin-Madison, and the University of Tennessee, Memphis. In 2013, Dr. Lustig received his Masters in the study of Law from University of California, Hastings to enable him to impact the food industry through policy change.
Dr. Lustig has authored 125 peer-reviewed articles and 73 reviews. He has mentored 20 pediatric endocrine fellows, and trained numerous other allied health professionals. He provides endocrinologic support to several protocols of the Children's Oncology Group. He is the former Chairman of the Ad hoc Obesity Task Force of the Lawson Wilkins Pediatric Endocrine Society, a member of the Pediatric Obesity Practice Guidelines Subcommittee of The Endocrine Society, a member of the Obesity Task Force of the Endocrine Society, a member of the Pediatric Obesity Devices Committee of the U.S. Food and Drug Administration, a member of the Bay Area Board of Directors of the American Heart Association, and a member of the Steering Committee of Health Foods, Healthy Kids of the Culinary Institute of America. He also consults for several childhood obesity advocacy groups.
Dr. Lustig lives in San Francisco with his wife Julie and two daughters. Spare time (what little there is) is spent cooking, theater-going, and traveling.

Key cellular events of SMCs include phenotypic switching, oxidative stress, inflammation and formation of extracellular vesicles. The regulation of vitamin K-dependent protein biosynthesis, its sites of action and its putative relationship with remodeling and calcification is insufficient to understand the pathogenesis of vascular ageing at a level that allows the development of novel diagnostic tools and therapeutic strategies.

In the past decade, Dr. Brenner has made multiple seminal contributions to NAD+ metabolism, which include engineering a yeast strain to convert inexpensive NAD+ precursor vitamins into NR, solving the crystal structure of human Nrk1, developing the methods for quantitative NAD+ metabolomics, and demonstrating the activity of oral NR in animal models of fatty liver disease; obesity; type 2 diabetes; diabetic and chemotherapeutic neuropathy; heart failure; and central brain injury. Dr. Brenner also led the first clinical trial of NR, which established safe oral availability in humans. He is currently focusing on the maternal and neonatal effects of oral NR and translating animal discoveries into evidence-based safe, human clinical practice for human conditions of metabolic stress.
