CartilatistTM is studied based on the off the shelf stem cell technology that has invented by Dr. Pham at Stem Cell Institute, University of Science, VNUHCM, Vietnam since 2012. The product contains 5 millions of adipose derived stem cells for allogenic graft.

Figure 1. The Cartilatist TM products are cryo-bags with 5 millions of adipose derived stem cells.

With the state of art technology, adipose derived stem cells in CartilatistTM are selected with high potential of immune modulation, and low immunogenecity.

Van Hanh General Hospital will receive this technology from CIPP and make the mass production at Stem Cell Unit, Van Hanh General Hospital.

The first Cartilatist products will be launched in Vietnam market from 2020.

Currently, physicians mostly prescribe analgesics, hyaluronic acid and neridronate for osteoarthritis. However, these only help ease symptoms, reduce pain and control inflammation, but cannot help in articulation reproduction.

PRP, or platelet-rich plasma, can improve some symptoms, but it is also ineffective after a long period of supervision.

Scientists have combined the PRP injection with stem cells. Recent publications show that mesenchymal stem cells and PRP have better effects in improving cartilage damage and VAS, the scale used to measure pain, than PRP alone.

The stem cells selected for production are carried out under strict control in accordance with GMP-WHO (good manufacturing practice – the World Health Organization) guidances. The production cost is reasonable as products are made in large quantities.

However, the method also has problems: the treatment effect is unstable, and stem cell quality significantly falls in elderly patients and those who suffer from chronic diseases. The treatment cost is very high.

The off-the-shelf stem cell therapy has been developed in an attempt to solve the problems. Scientists can find many advantages of the therapy.

The stem cells selected for production are carried out under strict control in accordance with GMP-WHO (good manufacturing practice – the World Health Organization) guidances. The production cost is reasonable as products are made in large quantities.

The Stem Cell Institute is focusing on Cartilatist, a stem cell product derived from human adipose tissue, used to treat osteoarthritis and degenerative spinal conditions.

Each treatment dose is contained in a small glass tube containing 5-10 million stem cells and stored at -40 to -860C. Adipose stem cells are collected and isolated and then proliferate under a special process which allows for selection of the most suitable stem cells.

The proliferation process runs in large bioreactors under strict control.

The Cartilatist technology completed at the pilot production level was transferred to the Van Hanh General Hospital Co., Ltd for commercial production in July 2018 in Vietnam and ASEAN member countries.

The Stem Cell Institute is now considering transferring the Cartilatist technology to the second partner in Europe to exploit the European and American markets.

The first off-the-shelf stem cell product in the world was available in South Korea under the name Cartistem in 2012.

In 2012-2017, six off-the-shelf stem cell products were commercialized and launched on the world market, including Cartistem, Prochymal, HS Temcell, Darvadstrocel, Osteocel plus and Trinity Evolution.

The first clinical application of stem cells in Vietnam was performed in 1995 that medicine doctors at Blood Transfusion Hospital, Ho Chi Minh city used hematopoietic stem cells for leukemia treatment.

However, more than 20 years (1995-2017), all treatments using stem cells in Vietnam is autologous, non-expanded stem cell transplantation.

In March 28th 2017, Ministry of Health approved the first allogenic expanded mesenchymal stem cell therapy for type 1 diabetes mellitus.

This approval permits Van Hanh General Hospital (Ho Chi Minh city, Vietnam) to carry out the clinical trial phase 1b/2a that using allogenic expanded mesenchymal stem cells derived from umbilical cord tissue in combination with bone marrow concentrates to cure type 1 diabetes mellitus.

According to the approval letter, this clinical trial will recruit 30 patients with type 1 diabetes mellitus. These patients will be intravenously transplanted a combo of umbilical cord tissue derived mesenchymal stem cells and bone marrow concentrates.

At the present, OA is treated by some drugs to reduce their symptoms but can not regenerate the knee cartilage. Stem cell therapy is the one most promising therapy for cartilage regeneration.

In this strategy, Stem Cell Institute (SCI) has developed the autologous adipose stem cell therapy for knee osteoarthritis since 2007. The product was developed from this therapy is 2 kits: ADSC Extraction Kit for adipose stem cell extraction; and New PRP Pro Kit for Platelet rich plasma preparation.

SCI collaborated with Geneworld Ltd. (Saigon Hightech Park, Ho Chi Minh city, Vietnam) to produce these commercial products.

Figure 1. The timeline for the autologous adipose derived stem cell therapy development for knee osteoarthritis at SCI.

The first clinical trial using this therapy to treat knee osteoarthritis was performed at Van Hanh Hospital and 115 Hospital in 2013. The results showed that adipose derived stem cells in combination with PRP gave the excellent improvements in symptoms and cartilage regeneration.

In 2016, this therapy was approved by Ministry of Health as the routine treatment for knee osteoarthritis. And it becomes the first approved mesenchymal stem cell transplantation at Vietnam as a routine treatment.

Although the disease is chronic, it significantly reduces the patient’s quality of life. At present, stem cell therapy is considered to be an effective approach for treating this condition. Mesenchymal stem cells (MSCs) show the most potential for stem cell therapy of osteoarthritis. In fact, MSCs can differentiate into certain mesodermal tissues such as cartilage and bone. However, mesenchymal stem cell transplantation is the expensive treatment, recent years, off-the-shelf stem cell technology is produced to produce stem cells as drugs that can be widely used in patients. The first technology is introduced in Canada and Korea in 2012. To date, there are some countries owned this technology included Canada, USA, Korea, Japan, Australia.

The first ever the key laboratory of stem cell research and application (SCL), University of Science, Vietnam National University Ho Chi Minh city successfully developed this technology to produce the produce “Cartilatist” that used in the treatment of knee osteoarthritis. The first idea about Cartilatist was suggested in 2009 by Dr. Pham at SCL. However, this technology has just finalized at Stem Cell Institute (SCI) (VNU-HCM University of Science) when the last technology about cryopreservation without cryoprotective agents was successfully studied.

Cartilist is stem cells based drugs that produced from allogenic human adipose tissue-derived stem cells. These cells were expanded for long-term to get enough the stem cells. Finally, these cells were induced to able to stimulate the endogenous stem cells to grow and differentiate into chondroblasts that can participate the cartilage regeneration. Finally, they were frozen in the particular condition without cryoprotective agents and can be directly injected into patients’ knee without any further manipulations.

In the early evaluation, Cartilatist exhibited the significant improvement in knee functions. This product will be evaluated in the more extensive clinical trials. Scientists at SCI hope that this product can be commercialized in 2022. At present, this technology was transferred to Stem Cell Unit, Van Hanh General Hospital to produce and commercialize in Viet Nam.
