September 23, 2024 – Covisus: 3.4 Tagless UDI tracking of non-sterile implants with no changes to implants, trays or packaging.

As noted in the US FDA website, “the FDA established the unique device identification (UDI) system to adequately identify medical devices sold in the Untied States from manufacturing through distribution to patient use”. This directive requires device labelers, who are typically the device manufacturers, to include UDI information on device labels and packages. All classes of medical devices shipped in sterile packages are in full compliance with these rulings. Non-sterile implantable devices (class II and III), such as orthopedic plates and screws, are shipped in packaging that include UDI information. However, at a medical facility, the implants are removed from their packaging, placed in surgical trays and sterilized prior to surgery. During this process, the UDI information is lost and is not traced to a patient’s medical records. While many larger implantable plates are directly marked with their UDI information (Direct Part Marking -DPM), close to 60 million implantable screws and countless plates used annually in surgeries across the country are not marked as they are too small. This results in a major non-compliance to FDAs requirement. As a result, the agency is restricted in its ability to perform post-market surveillance and manage recalls of most non-sterile implants. In addition to risking patient lives, health facilities are often stuck with large inventories of non-sterile implantable devices with unknown provenance that require complex and expensive dispositioning.

We report on a commercially available system developed for the Department of Defense to meet their Intrinsic Unique Device Identifier (IUID) requirements (DFARS 252.211-7003) and its applicability to meet FDA’s Unique Device Identifier (UDI) requirements. This system uses an imaging assembly mounted onto an automated robotic platform. At a rate of approximately 1 second per part, it rapidly captures the unique surface texture of an implantable device and from it creates an immutable unforgeable intrinsic “fingerprint” that acts as a system generated item specific token. No direct part marking, RFID or other labels are required. A proof-of-concept study with this device was performed at a hospital on multiple non-sterile neuro surgical kits from two different manufacturers. At the hospital Central Supply, all screws and plates from the kits were scanned with the imaging system that generated an item specific digital token to which the devices UDI information was linked. This information was stored in a database that can be linked with a hospital’s ERP system in the future. Trays were then sterilized and either sent to surgery or to a hospital representative who simulated surgical conditions. Following use, the washed trays were then returned to Central Supply and verified with the imaging system. The reconciliation reports generated by the imaging system were cross checked with either the surgical report or simulated study report. Our study showed that the UDI information associated with all screws and plates were retained through the entire process with minimal disruption to the workflow at Central Supply and no work/overhead on surgical staff. In addition to being in compliance with FDA UDI requirements, we found multiple scenarios where the imaging system was able to flag non-conforming implants that could improve overall quality during kit preparation. This “value beyond compliance” is expected to enhance patient safety.

The U.S. Customs and Border Protection’s (CBP) Office of Trade will host the Forced Labor Technical Expo in Washington D.C. Abstract: With support from the Defense Microelectronics Activity (DMEA), we are developing DTEK 3.0 to support the ongoing Department of Defense (DOD) challenge to ensure the authenticity and security of microelectronics parts within its supply chain. The automated machine vision system is designed to capture SAE AS 6171/2 EVI test methods as well as the surface features of a microelectronic component and using machine learning algorithms to determine if the component is suspect or not. The technology is based on our proven products currently being used within the DOD supply chain to protect it from counterfeit components. A vTag® is a ‘virtual tag’. It functions like a tag but without any modifications to the part. Like a human fingerprint or iris scan, a vTag® uses intrinsic surface features of an item to uniquely identify it from all other items of its kind. The intrinsic features of an item are micro-scale roughness, formed randomly during the fabrication of the item. They are well known in the scientific literature1to be completely unique and impossible to replicate. The pharmaceutical industry is a massive global level industry. By the year 2023 it is projected that $1,520 billion will be spent on medicines. When it comes to the pharmaceutical industry, the U.S. dominates. North America alone holds responsibility for the largest revenue in pharmaceuticals. With that being said, it is hopeful that the U.S. regulations would be some of the strictest with pharmaceutical serialization for traceability and counterfeit detection. The U.S. currently has no centralized database for serialization and traceability.

Currently, there is a pharmaceutical regulation in the United States that demands pharmaceutical serialization on a package level. The intentions behind this regulation is that returned medicines will be closely monitored for counterfeits during the return process between wholesalers, hospitals and pharmacies. Item level traceability will be implemented in the pharmaceutical industry across the EU with a centralized regulation database. With the U.S. taking in the largest portion of revenue, it would be beneficial for a similar approach to be taken.

Item level traceability has potential to be beneficial and successful in the United States. Pharmacies receive inventory from pharmaceutical wholesalers and separate pills individually. Fake pills

Counterfeit pharmaceuticals are one of the most falsely reproduced items in the world and there is a new recurrence of fatal health issues linked to fake pain killers. These pain killers are round blue pills with an “M” on one side and “30” on the other to be disguised as 30mg oxycodone. The fatal ingredient inside these fake pills is fentanyl which is an opioid drug that is much stronger than opioids like morphine.

In Santa Cara County there have been 9 documented overdose deaths linked to these fake oxycodone pills since January of 2019. A statement on the issue was released by Santa Cara County that mentions the extent of circulation is unknown. 

Fatal incidents related to the consumption of these fake pills have also been reported in Yakima, WA. A report was released by Yakima NBC on the serious matter with hopes to bring attention to the case.

Marty Brueggemann, the chief medical officer at Virginia Mason Memorial, says that this is not his first time hearing about drugs laced with fentanyl in the area. “The thing with fentanyl that makes it potentially dangerous is it’s incredibly potent and so it’s about 50 to 100 times more potent than morphine, and that’s where we kinda get into trouble in some places.” said Brueggemann. 

There have been statements that these laced drugs normally derive from production facilities in Mexico. This time, there has been no designated place of origin for these fake pills. Those involved with investigating the fatal laced pills are still working towards an answer to where these came from. 

In the mean time, officials urge people to only purchase and accept any form of pharmaceuticals from a licensed medical professional through a licensed pharmacy.
