]]>Hello everyone. This is Dr. Dennis Barley at Barley Wellness here in Fairhaven, Massachusetts. Today we’re going to talk about a condition called Carpal Tunnel Syndrome.
The hand and wrist of your body contains 27 moveable bones. And there can be a compression in something called the carpal tunnel. What is the carpal tunnel, we have eight bones in our in our wrist four, a row of four and another row of four. And underneath is a tendon or something called the flexor retinaculum. It forms a tunnel, the bones are arched here on top of your wrist. And here is that tunnel, in through that tunnel is the median nerve and artery and vein that median nerve does innervate, the thumb, the forefinger, the middle finger, and the median medial part medial part of the ring finger also, what does carpal tunnel feel like it can feel like a burning a numbness or tingling, strong pain into the wrists could be both wrists. And as a chiropractic office, we need to find out whether indeed you do have carpal tunnel syndrome.
So the first thing that we’ll do is check will do is sit down with you and consult with you to find out how this happened. What’s it doing to your daily life, and what we can do about it if indeed we discovered that it is carpal tunnel, we will then do an examination after the consultation to find out whether you have carpal tunnel. And from there, we’ll check not just the wrist, but we’ll check the elbow for any impingement, the shoulder for any impingement. And the cervical spine for an impingement, that medium median nerve is innervated by the C-5-6-7. And the first thoracic T-1 nerve roots that innervate that make up that median nerve that travels into the into the wrist and into the hand. So we’ll do a complete examination to see whether you have any impingements not just at the wrist but elsewhere. And then we’ll we’ll get involved with our treatment protocols. And we’ll tell you a little bit more about that.

]]>So we’re gonna go over some therapies today that we do in our office for people with Carpal Tunnel Syndrome.
One of the therapies is working on the soft tissue. And what I mean by that is the forearm musculature, and musculature leading into the hands and wrist. It’s a critical component of treating carpal tunnel syndrome, as it helps the muscles function more normally, many muscles and tendons pass through this carpal tunnel that can negatively affect get negatively affected by inflammation. So when we utilize these therapies like ultrasound, or muscle therapy, using an instrument or not, and also something called Alpha stim, we’re helping alleviate those tight muscles reducing tension and restoring normal function back into that area of the wrist that’s giving the patient problems, non instrument assisted myofascial work utilizes utilized by us at barly wellness, we alleviate muscle muscle tension, fascial, intendant, adhesions and dysfunction, which can improve the the symptoms of carpal tunnel, and also reduce some of the inflammation.
So the first thing we’ll do or illustrate is ultrasound. And with my hand model, we’re just going to basically use the upper forearm muscles here, okay, through the these muscles in the in the underside of the arm, so the extending extensor muscles and the flexor muscles, and we’ll apply a gel on the skin and then do mild short term ultrasound and ultrasound therapy applied to this area can may may induce a reduction in inflammation, will will apply some, you know gel again, and use moving ultrasound to reduce inflammation here, there was a study actually by a fellow named Evan Beckler and colleagues and they conducted a controlled clinical trial using ultrasound for carpal tunnel syndrome. And they found that it did indeed reduce inflammation. So that’s one therapy we can use as ultrasound.
Another therapy we can use is basically just utilizing the muscles themselves using some some what we call stripping here using our hands, and then actually applying an extension of the wrist. And getting deeper into those those muscles where they’re tightness, we can feel some trigger points, and we’ll work hard on those trigger points. If if using the your hands is is not as effective, then we may use something an instrument with gentle, gentle rubbing back and forth on these areas to reduce any Myo fascial tension, that is the sleeve around the muscle that can constrict and cause irritation. So use that. And the last thing we have is something called Alpha stem. And it’s kind of a it’s a relatively unknown procedure.
Alpha-Stim is actually a non drug FDA cleared medical device that is painless and easy to use therapy. And we use this at Barley Wellness here in Fairhaven. A patient can actually take this device home and utilize it there. It’s fast x acting, it’s long lasting and has cumulative effects. It works by restoring the balance of the engineering using a painless electro micro current waveform on this waveform returns your body back to homeostasis or this these muscles back to homeostasis. Essentially we’ll take the probes and apply them gently here. There’s no There’s no discomfort, patient feels basically nothing. And we’ll apply around the wrist, turn the wrist over and do the same. The bottom of the wrist, we’ll do a cross section and then we’ll come across and do cross the wrist and on top of the wrist.
Those are some of the therapies that you and you will get to naturally treat carpal tunnel syndrome. Like I said before, carpal tunnel syndrome many times is a surgical fix. ]]>Hey everyone, this is Dr. Dennis Barley at Barley Wellness here in Fairhaven, Massachusetts. We’re going to talk about how we take care of Carpal Tunnel Syndrome, a painful wrist, hand, forearm discomfort.
Here at Barley Wellness, we do things more naturally typically carpal tunnel syndrome may be treated with surgery. And at Barley Wellness, what we will do is do a consultation to see how you develop this, this discomfort and how it’s affecting your life. We’ll also dive deep into what’s creating the cause of it.
So we’ll do a complete examination to see what’s causing your carpal tunnel syndrome. From there, we will look to many areas that could potentially cause problems in the wrist. That being because the carpal tunnel which are eight bones in the wrist, that are the top of the wrist here, okay, so here, these eight bones, they slide they they’re they form a tunnel with this soft tissue underneath the bottom side of the wrist. And if that tunnel collapses, or there’s inflammation in that tunnel, it can affect an artery of vein, a nerve in there, the median nerve specifically, that can cause irritation, pain, numbness and tingling, tingling and burning in your hands, specifically your thumb, your forefinger your middle finger in the this part of your ring finger.
So we will look to the spine because the median nerve is innervated by seat and the nerve roots at C-5 through T-1. So we’ll look here in the upper back and the spine, the neck to see if you have any impingement there. We’ll also look to see if there’s any impingement along that we call the kinetic chain, not just from the neck, but down into the shoulder, you could have an impingement in here and we’ll check for that also in the elbow, and of course the wrist. So we’ll sit first with you do a consultation to find out how this is affecting you. Then we’ll examine you will examine all those areas to see where the carpal tunnel is coming from. And then from there, we will we will work with specific chiropractic adjustments for your spine for your for your extremities.
Some other work we’ll do is some soft tissue work, we’ll use some an instrument and I’ll go over these in another video but we’ll do some instrumentation on muscles that are tight on muscles that have trigger points in them. Some myofascial irritation, so we use some instrument assisted soft tissue work, we’ll do soft tissue work with with our hands only. And then we’ll we’ll we’ll use some specific modalities that may help like alch ultrasound, which has been shown that creates an anti inflammatory effect and has been shown to help carpal tunnel syndrome.
And then we’ll use something called Alpha stem which is a non drug FDA medical device that is a painless, easy to use therapy that you can use here at the office or also it’s available for home use. It’s long lasting, it’s it has a cumulative effect, and it restores the natural balance to the injured area. It’s typically painless and painless microcurrent in a in a specific form. And there’s a lot that can be done for migraines. Working on the spine, that’s particularly the upper cervical spine. We see here, here’s the skull and the top few vertebra, the nerves come out in between the vertebrae to come back up towards your head. And if there’s a fixation or a misalignment of those vertebra, causing some irritation, those nerves that could be a big contributing factor to you having migraines.

]]>This is Dr. Sheridan from Barley Wellness. And we’re going to talk about migraines and the role that trigger points play in your treatment of migraines. And we’re going to talk specifically about three muscle groups, the suboccipital oils, the SEM muscle, and the upper trapezius muscle, and how they play a role in treating your migraines.
Oftentimes, when you come to the chiropractor, we’re focusing on your whole body. But in particular, with migraines, we’re looking at the top part of your cervical spine, where the nerves go back up towards your head. And when those areas are misaligned, they cause some irritation, and those muscles tend to tighten up. And they will get knots in them, if you will, what we call trigger points. And we will be working on those as well as adjusting that area of your spine. But there’s some of these things that you can probably do at home. And we’re looking at the suboccipital muscles here, and this tiny little muscles that turn your head and position your head, and they can give you a presented with a headache in that shaded area that goes right behind your eye. And then that’s the SEM muscle where it goes down the side of your neck. And that can come up around your eye and into your into your face a little bit.
So muscle goes from behind here down to the collarbone. And then your upper trap muscle which is right around here would give you it’s tough to see the shaded area, but it gives you that area right up over your ear. And that’s often come with a headache or a migraine.
So these are areas that we would be working on. But you could do a little bit at home too, if you wanted to, you could just feel along and gently palpate that area. And if you had a tight spot, you could kind of squeeze it. Apply some pressure, try and breathe through it slowly. For 10 to 60 seconds, you would be putting some pressure there, what you’re looking for is for the tension or the discomfort to decrease a little bit. That would be for the SEM muscle.
A good way to do the suboccipital muscles would be to just take your thumbs like this, and feel for the back of your skull and follow it to yours. And if you find a tight spot, and if it recreated that feeling behind your eye, that’d be the spot to put some pressure on and just try and breathe through it and through your nose out through your mouth. And again, 10 to 60 seconds. If it releases a little bit, that’s great. Try and hydrate when you after you do these things to get fluids in your back into your muscles. And the upper trap is kind of tough, but you can kind of hang on it with your fingers here and just press breathe through it again 10 to 60 seconds if you want to tip your head away a little bit that might help also.
Those are just some simple things you can try and do while you’re at home. They’re also things that we would incorporate into your care here.

]]>Hey everyone, its Dr. Dennis Barley at Barley Wellness here in Fairhaven, Massachusetts. Today I’m going to talk about a problem that you may have in your wrist called Carpal Tunnel Syndrome.
Carpal Tunnel Syndrome is created when the tunnel collapses, that the eight bones in your wrist, called carpal bones are supported by something called the flexor retinaculum, or a transverse ligament, and the median nerve and there’s an artery and vein that go through this tunnel. If the tunnel collapses, it can put pressure on that nerve, who are most susceptible, susceptible or at high risk for carpal tunnel?
Those are desk workers, you know, people who are frequently on the computer. Frequent taking typing hours after hours, day after day, week after week can cause repetitious motion and that error can cause some real issues with the muscles of the forearm and also cause problems in with inflammation in the wrist. So proper desk organ ergonomics or how you position yourself at the desk and where your keyboard is, is vitally important. Second group of people who may have suffer from carpal tunnel syndrome are those meat and fish processing workers. They’re in a cold environment, they’re lifting heavy, and many times Holding, holding things tightly, can create pressure again in an inflammation in the wrist of pressure and overuse syndrome in the muscles and then create inflammation, the risks that can create carpal tunnel and then diabetics. Diabetes is a metabolic condition where there is a lot of inflammation. Many times that inflammation in the carpal tunnel will create diabetic neuropathy and so that has to be diagnosed and addressed.
So those are three high risk groups that may suffer from carpal tunnel syndrome. And carpal tunnel syndrome, of course, is a burning tingling or numbness, weakness in the wrist, forearm and the hands.

]]>Hi, my name is Dr. Theresa Soucie from Barley Wellness, I want to talk to you a little bit about cervicogenic vertigo or vertigo in general.
True vertigo was a sense of the room is spinning around you. A lot of people come in and say they’re dealing with dizziness and actual dizziness is like feeling lightheaded. Feeling faint, that’s more what dizziness is or you feel disoriented. Vertigo is, when the room is spinning. There’s a few different reasons for you to have vertigo.
One of the most common ones is called benign positional vertigo, it comes from the inner ear, there’s little canals that go in different directions, and there’s fluid in there. And as the fluid whirls, it tells your brain, what position your body is in space. Sometimes there’s a little bit of settlement that crystallizes out of that. So when you move your head at whirls and stops, and you get that feeling like you’re have vertigo, because your brain can’t tell what position your head is in space. We can treat that here at Barley Wellness, we do a procedure called Epley Maneuver, and it helps that settlement settle down.
One of the more common forms of vertigo is cervicogenic vertigo. Not a lot of people know about that. The top three vertebra in your neck actually have a lot of nerve endings in it to tell your brain what position your head is in space because you have a reflex called the righting reflex that wants to keep your eyes level on the horizon. When the joints in the upper neck aren’t moving well. The position that your brain perceives doesn’t agree with your eyes and you get that vertigo feeling. So oftentimes, we can help that resolve by doing chiropractic adjustments and giving you exercises. One of the things that we do do when you come in is we try to figure out what the cause is for your vertigo. So we do an orthopedic and neurologic exam, we take x rays.
There are a couple other reasons why someone might have vertigo. Sometimes those little canals I talked about earlier, become inflamed, and that usually need some anti-inflammatories that’s called Meniere’s disease. You can also get vertigo with some sickness, some sinus problems.
So when you come in, we definitely figure out what the causes of your vertigo. Oftentimes if you have those misalignments and part of your neck you made your symptoms may vacillate you may get vertigo, you may feel like you’re lightheaded, you may feel like you feel a little disoriented and clumsy. Some people have a hard time focusing with their eyes. Some people get ringing in their ears, all that and even migraines can be related to your neck not moving well. And we treat that condition every day. Thank you!

First of all, it’s safe. It’s safe for the mom and the baby. And it’s just going to help with a better delivery. And what we want to talk about is when to start chiropractic during your pregnancy. And if you can get here before you’re pregnant, that’d be great. So we can get you in shape before you before you start your journey. But during that first trimester would be a great time too. That is when your body’s going through those initial changes and increasing the hormone relaxin in your body. And that’s when the joints start to loosen a little bit, we want to make sure that nothing gets too out of alignment.
Another time that chiropractic comes in handy is when the baby might be breech. There’s a great technique called the Webster technique in chiropractic that helps the baby allow the baby to turn later in the in the third trimester so we can have a safe delivery. And it’s just about chiropractics about helping your body during that time, and even postpartum to come back together again. And be as healthy as you can be during that process and heal up after you have a fantastic baby at the end.

]]>Hello everyone, Dr. Dennis Barley from Barley Wellness here in Fairhaven, Massachusetts. We are speaking today about plantar fasciitis and why do we get plantar fasciitis will be what I’ll be speaking about.
Now, there are many different risk factors that predispose people to developing plantar fasciitis. First and foremost is weight. If you’re overweight, or you have a BMI of over 30, which puts you into the obese category, you are now six times more likely to develop plantar fasciitis than those people who have a BMI below that that number. Age is a risk factor with people between 40 and 60 years of age most likely develop that plantar fasciitis this painful condition, probably due to decrease in tissue elasticity. Pregnant women also are prone to plantar fasciitis because of the excess weight. And there are other risk factors, such as people who spend a lot of time on their feet.
So occupations where you know, you’re spending a lot of time on your feet as there’s a condition where you could develop plantar fasciitis. If you’re in the wrong type of shoes, or you’re in work boots all day long, that can be an issue. Your foot mechanics can be an issue also. So foot foot mechanics and the types of shoes that we were play a role in whether you develop plantar fasciitis or not. For example, if your heel pronates naturally meaning your heel turns inward when you’re walking, you’re more likely to develop plantar fasciitis due to a gradual breakdown of the bottom of the foot. these risk factors are taken into consideration when we take a look at what’s happening to you and how you’ve developed plantar fasciitis.
So, again, who gets plantar fasciitis? Many times runners who are pounding on their on their feet. You know frequently pregnant women because of the excess weight, overweight people in those in occupations who are on their feet for longer periods of time.

]]>Hi everyone, Dr. Dennis Barley here at Barley Wellness. Kind of giving you a home self diagnosis for plantar fasciitis.
There are three ways to tell if you have plantar fasciitis.
Usually you feel plantar fasciitis first thing in the morning when you step out of bed. It typically is very painful once you’re getting up walking around going down the stairs. Eventually with more walking, it tends to feel a little bit better. However, if you sit down to breakfast, you sit down for a long period of time and you go to stand up again. It gives you that pain.
The other thing you can do is something called the heel squeeze test. When you squeeze the heel between your be freed between your fingers or four fingers and your thumb, if it yields pain, there’s a possibility that you have plantar fasciitis.
And then the stretch test if you stand holding on to both handrails on an on a step and you put your toes on the step your heels hanging off and let them drop down below the step. If you notice that those muscles are tight, there are two muscles of the calf one’s called the gastrocnemius. That’s the one you see on the outside and there’s a flat one closer to the bone called the soleus. If those muscles are tight and they yield heel plank pain, that’s a potential that you do have plantar fasciitis.
