This pioneering program includes a full-time staff person and provides crisis intervention, mental health support, grief and bereavement support, and collaborative case management to help address social determinants of health. "I'm excited to be a judge because of what this technology represents for our industry," said Bell. "This concept is about engaging people where they're at, using a technology with a very low barrier to entry to give them a way to manage their care on their own terms and in their own homes."
