(This is, for some reason, omitted from the main body of the Handbook for Virginia Notary Handbook provided by the SOC, but it is contained at the end where the full Code of Virginia “Virginia Notary Act” is printed. Additionally, I confirmed it by looking up the Code of Virginia directly and calling the SOC. Unfortunately, Virginia proscribes no standard form for an affidavit!)
