1. Sukari.
Sukari nyingi katika damu huweza kupelekea kuharibiwa kwa kuta za mishipa ya damu. Kisha kuta hizi hushindwa kuzalisha homoni aina ya Nitrogen Oxide ambayo husaidia kutanua mirija ya damu kwa ajili ya kupitisha damu kwa ufasaha. Ikitokea katika mirija ya damu inayotiririsha katika uume kushindwa kutanuka vema, ni wazi kuwa uume utapungua uimara wake.

2. Lehemu (Cholesterol).
Lehemu huganda kwenye kuta za mirija ya damu mithiri ya mafuta yanavyoganda katika chupa. Hali hii hupelekea kupungua kipenyo (diameter) cha mirija ya damu na kushindwa kupitisha damu vizuri.

3. Mgandamizo wa damu au Presha (Hypertension).
Katika hali hii moyo hushindwa kusukuma damu vizuri na hivyo kupungua kwa kasi na ujazo wa damu katika maeneo yaliopo mbali na moyo, ikiwa ni pamoja na viungo vya uzazi.

Pia, Mishipa ya fahamu (nerves) au Neva za fahamu huweza kuharibiwa na kupunguza ufanisi wake.Katika hali hii, maeneo yanayohudumiwa na neva hizo hupungua hisia na wakati mwengine kupata ganzi ya kudumu. Uume pia huweza kuathiriwa kwa namna hiyo.

Abstract
BACKGROUND: The Xpert MTB/RIF assay is an automated molecular test that has improved the detection of tuberculosis and rifampicin resistance, but its sensitivity is inadequate in patients with paucibacillary disease or HIV. Xpert MTB/RIF Ultra (Xpert Ultra) was developed to overcome this limitation. We compared the diagnostic performance of Xpert Ultra with that of Xpert for detection of tuberculosis and rifampicin resistance.
METHODS: In this prospective, multicentre, diagnostic accuracy study, we recruited adults with pulmonary tuberculosis symptoms presenting at primary health-care centres and hospitals in eight countries (South Africa, Uganda, Kenya, India, China, Georgia, Belarus, and Brazil). Participants were allocated to the case detection group if no drugs had been taken for tuberculosis in the past 6 months or to the multidrug-resistance risk group if drugs for tuberculosis had been taken in the past 6 months, but drug resistance was suspected. Demographic information, medical history, chest imaging results, and HIV test results were recorded at enrolment, and each participant gave at least three sputum specimen on 2 separate days. Xpert and Xpert Ultra diagnostic performance in the same sputum specimen was compared with culture tests and drug susceptibility testing as reference standards. The primary objectives were to estimate and compare the sensitivity of Xpert Ultra test with that of Xpert for detection of smear-negative tuberculosis and rifampicin resistance and to estimate and compare Xpert Ultra and Xpert specificities for detection of rifampicin resistance. Study participants in the case detection group were included in all analyses, whereas participants in the multidrug-resistance risk group were only included in analyses of rifampicin-resistance detection.
FINDINGS: Between Feb 18, and Dec 24, 2016, we enrolled 2368 participants for sputum sampling. 248 participants were excluded from the analysis, and 1753 participants were distributed to the case detection group (n=1439) and the multidrug-resistance risk group (n=314). Sensitivities of Xpert Ultra and Xpert were 63% and 46%, respectively, for the 137 participants with smear-negative and culture-positive sputum (difference of 17%, 95% CI 10 to 24); 90% and 77%, respectively, for the 115 HIV-positive participants with culture-positive sputum (13%, 6·4 to 21); and 88% and 83%, respectively, across all 462 participants with culture-positive sputum (5·4%, 3·3 to 8·0). Specificities of Xpert Ultra and Xpert for case detection were 96% and 98% (-2·7%, -3·9 to -1·7) overall, and 93% and 98% for patients with a history of tuberculosis. Xpert Ultra and Xpert performed similarly in detecting rifampicin resistance.
INTERPRETATION: For tuberculosis case detection, sensitivity of Xpert Ultra was superior to that of Xpert in patients with paucibacillary disease and in patients with HIV. However, this increase in sensitivity came at the expense of a decrease in specificity.

Abstract
BACKGROUND: Millions of HIV-infected people worldwide receive antiretroviral therapy (ART) in programmes using WHO-recommended standardised regimens. Recent WHO guidelines recommend a boosted protease inhibitor plus raltegravir as an alternative second-line combination. We assessed whether this treatment option offers any advantage over the standard protease inhibitor plus two nucleoside reverse-transcriptase inhibitors (NRTIs) second-line combination after 144 weeks of follow-up in typical programme settings.
METHODS: We analysed the 144-week outcomes at the completion of the EARNEST trial, a randomised controlled trial done in HIV-infected adults or adolescents in 14 sites in five sub-Saharan African countries (Uganda, Zimbabwe, Malawi, Kenya, Zambia). Participants were those who were no longer responding to non-NRTI-based first-line ART, as assessed with WHO criteria, confirmed by viral-load testing. Participants were randomly assigned to receive a ritonavir-boosted protease inhibitor (lopinavir 400 mg with ritonavir 100 mg, twice per day) plus two or three clinician-selected NRTIs (protease inhibitor plus NRTI group), protease inhibitor plus raltegravir (400 mg twice per day; protease inhibitor plus raltegravir group), or protease inhibitor monotherapy (plus raltegravir induction for first 12 weeks, re-intensified to combination therapy after week 96; protease inhibitor monotherapy group). Randomisation was by computer-generated randomisation sequence, with variable block size. The primary outcome was viral load of less than 400 copies per mL at week 144, for which we assessed non-inferiority with a one-sided α of 0·025, and superiority with a two-sided α of 0·025. The EARNEST trial is registered with ISRCTN, number 37737787.
FINDINGS: Between April 12, 2010, and April 29, 2011, 1837 patients were screened for eligibility, of whom 1277 patients were randomly assigned to an intervention group. In the primary (complete-case) analysis at 144 weeks, 317 (86%) of 367 in the protease inhibitor plus NRTI group had viral loads of less than 400 copies per mL compared with 312 (81%) of 383 in the protease inhibitor plus raltegravir group (p=0·07; lower 95% confidence limit for difference 10·2% vs specified non-inferiority margin 10%). In the protease inhibitor monotherapy group, 292 (78%) of 375 had viral loads of less than 400 copies per mL; p=0·003 versus the protease inhibitor plus NRTI group at 144 weeks. There was no difference between groups in serious adverse events, grade 3 or 4 adverse events (total or ART-related), or events that resulted in treatment modification.
INTERPRETATION: Protease inhibitor plus raltegravir offered no advantage over protease inhibitor plus NRTI in virological efficacy or safety. In the primary analysis, protease inhibitor plus raltegravir did not meet non-inferiority criteria. A regimen of protease inhibitor with NRTIs remains the best standardised second-line regimen for use in programmes in resource-limited settings.

Diagnostic accuracy of Xpert MTB/RIF Ultra for tuberculous meningitis in HIV-infected adults: a prospective cohort study.

Abstract
BACKGROUND: WHO recommends Xpert MTB/RIF as initial diagnostic testing for tuberculous meningitis. However, diagnosis remains difficult, with Xpert sensitivity of about 50-70% and culture sensitivity of about 60%. We evaluated the diagnostic performance of the new Xpert MTB/RIF Ultra (Xpert Ultra) for tuberculous meningitis.
METHODS: We prospectively obtained diagnostic cerebrospinal fluid (CSF) specimens during screening for a trial on the treatment of HIV-associated cryptococcal meningitis in Mbarara, Uganda. HIV-infected adults with suspected meningitis (eg, headache, nuchal rigidity, altered mental status) were screened consecutively at Mbarara Regional Referral Hospital. We centrifuged CSF, resuspended the pellet in 2 mL of CSF, and tested 0·5 mL with mycobacteria growth indicator tube culture, 1 mL with Xpert, and cryopreserved 0·5 mL, later tested with Xpert Ultra. We assessed diagnostic performance against uniform clinical case definition or a composite reference standard of any positive CSF tuberculous test.
FINDINGS: From Feb 27, 2015, to Nov 7, 2016, we prospectively evaluated 129 HIV-infected adults with suspected meningitis for tuberculosis. 23 participants were classified as probable or definite tuberculous meningitis by uniform case definition, excluding Xpert Ultra results. Xpert Ultra sensitivity was 70% (95% CI 47-87; 16 of 23 cases) for probable or definite tuberculous meningitis compared with 43% (23-66; 10/23) for Xpert and 43% (23-66; 10/23) for culture. With composite standard, we detected tuberculous meningitis in 22 (17%) of 129 participants. Xpert Ultra had 95% sensitivity (95% CI 77-99; 21 of 22 cases) for tuberculous meningitis, which was higher than either Xpert (45% [24-68]; 10/22; p=0·0010) or culture (45% [24-68]; 10/22; p=0·0034). Of 21 participants positive by Xpert Ultra, 13 were positive by culture, Xpert, or both, and eight were only positive by Xpert Ultra. Of those eight, three were categorised as probable tuberculous meningitis, three as possible tuberculous meningitis, and two as not tuberculous meningitis. Testing 6 mL or more of CSF was associated with more frequent detection of tuberculosis than with less than 6 mL (26% vs 7%; p=0·014).
INTERPRETATION: Xpert Ultra detected significantly more tuberculous meningitis than did either Xpert or culture. WHO now recommends the use of Xpert Ultra as the initial diagnostic test for suspected tuberculous meningitis.

The US department of state has cautioned its citizens to “reconsider travel to Nigeria because violent crime, such as armed robbery, assault, carjacking, terrorism, civil unrest, kidnapping, rape and piracy, is common throughout the country.”

In a travel advisory recently released on its website, which contains information on the safety and security of US travellers, Nigeria was listed among 35 countries with a “K” indicator that indicates where its citizens could be at risk of getting kidnapped or taken hostage.

“The new “K” indicator is part of our ongoing commitment to provide clear and comprehensive travel safety information to U.S. citizens so they can make informed travel decisions,” the department said.



“The Travel Advisories for 35 countries have been updated to include a “K” indicator for the risk of kidnapping and/or hostage taking: Afghanistan, Algeria, Angola, Bangladesh, Burkina Faso, Cameroon, Central African Republic, Colombia, Democratic Republic of the Congo, Ethiopia, Haiti, Iran, Iraq, Kenya, Lebanon, Libya, Malaysia, Mali, Mexico, Niger, Nigeria, Pakistan, Papua New Guinea, Philippines, Russian Federation, Somalia, South Sudan, Sudan, Syria, Trinidad and Tobago, Turkey, Uganda, Ukraine (in Russian-controlled eastern Ukraine), Venezuela, and Yemen.”

While Nigeria was placed on level 3, the US department of state issued a level 4 travel alert for Borno, Yobe and Adamawa states.

“The security situation in northeast Nigeria is fluid and unpredictable, particularly in Borno and Yobe States and northern Adamawa State,” the department said.

“Terrorist groups based in the Northeast target churches, schools, mosques, government installations, educational institutions, and entertainment venues. Approximately two million of Nigerians have been displaced as a result of the violence in northeast Nigeria.”

Samples taken in the QDL wetland in the upstream region of the Heilong River showed higher values for EFs of heavy metals than those taken in HXZ after 1980 CE This pattern could indicate that human activities started to lead to the deposition of more heavy metals into the QDL wetland starting in 1980 CE However, the EFs of heavy metals in the HXZ wetland in the downstream region of the Heilong River almost showed stable patterns in all stages, with only a gradual increasing from 1965 CE ( Fig. 4a). This pattern might be due to the fact that heavy metal concentrations can decrease gradually with the water flow because of the self-purification of the water body and sedimentation processes ( Ostroumov, 2002;Vagnetti et al., 2003). Alle ore 11 di questa mattina, nella Sala Clementina del Palazzo Apostolico, il Santo Padre Francesco ha ricevuto in Udienza i volontari dell'Associazione italiana per la Donazione di Organi, Tessuti e Cellule (AIDO). Dopo l’indirizzo di saluto della Dott.ssa Flavia Petrin, Presidente dell’AIDO, il Papa rivolge ai presenti il discorso che pubblichiamo di seguito:

Sono lieto di accogliere tutti voi, volontari dell’Associazione Italiana Donatori di Organi (AIDO), qui convenuti in rappresentanza di migliaia di persone che hanno scelto di testimoniare e diffondere i valori della condivisione e della donazione, senza nulla chiedere in cambio. It seems this site is pretty well-dominated with information regarding gay life in W. Africa. Perhaps, we can start more dialogue on similar topics in E. Africa. I lived in Tanzania when I was young and returned there for work just after university. Now, I am back in the US after completing my graduate degrees, but continue to work in E. Africa. I know Uganda is a very bleak scene nowadays for gays and lesbians. But, there is a growing advocacy for gay and lesbians especially in the capital. There are opportunities to meet gay men in some locales (ie. Matteo's after 10pm on Fri/Sat) in Kampala now especially near Makerere University (ie. T Cozy bar on Sunday nights). I will be happy to share a few experiences with others interested and who hopefully can share some as well.

High level international condemnation has just pushed the President to send the bill for review, but Ugandan allies say only a worldwide outcry could tip Parliamentarians away from discrimination, alarming them with global isolation.

We have just days left -- sign the petition to oppose Uganda's anti-gay law below and send it on to friends and family and it will be delivered to Uganda's politicians, donors and embassies around the world.

This is Part II of a two part pancocojams series about the Nigerian (Igbo) praise & worship song "Ekwueme" as sung by Prospa Ochimana featuring Osinachi Nwachukwu.

THE CUSTOM OF "NATION SIGNING IN" ON DISCUSSION THREADS FOR CERTAIN YOUTUBE MUSIC VIDEOS
"Nation signing in" (nsi) occurs when a person writes his or her nation's name, country code top-level domain [initials], country telephone code (iso) etc. in a discussion thread for a YouTube music video with or without any other text or emojis (small digital images or icons).

Commenters who sign in their nation's name (and/or other group identifiers) may also implicitly or explicitly expect or demand that other commenters from that nation or other affiliation follow his or her lead and also sign in and represent* that nation on that discussion thread. In addition, or separately, a commenter may ask or expect that people from other nations "rep" ("represent') their nation by writing a response to that comment. Much less often, instead of their nation, I've found comments from people representing their ethnic group (such as Luo), city/state* (such as Burbank, California), region (such as the Caribbean), or continent (such as "Europe").

I've noticed this custom in the discussion threads of a number of popular contemporary non-religious YouTube music videos from West Africa, South Africa, East Africa, and Central Africa. I'm currently testing my theory that this custom is also found in the discussion threads of popular contemporary religious YouTube videos from at least two of African regions (West Africa and South Africa). By contemporary, I mean within the last 10 years. By popular, I mean videos with viewer hits totally multiple millions.

In testing this theory, I'm somewhat randomly selecting a few YouTube contemporary religious music videos from South Africa and from Nigeria that have multiple million hits. After watching the videos, I'm "reading" (mostly scanning) the discussion threads of those videos to see if I find any nation signing comments. Based on my very limited informal research which is still ongoing, it appears to me that the custom of writing "nation signing in" may also be a common feature on large discussion threads of contemporary non-religious African (YouTube) music videos from West Africa, South Africa, East Africa, and Central Africa (as well as collaborations between singers from different regions of Africa and elsewhere.)

It also appears that nation signing in comments are also a common feature on large discussion threads for contemporary religious (YouTube) music videos in West Africa and South Africa (as well as combinations of those two regions' singers. This custom may also be found in other YouTube discussion threads of other African music from other regions and/or YouTube discussion threads for non-African music. Stay tuned. Next I'm going to check out contemporary Caribbean music to see if there are any nation signing in comments on any of those YouTube discussion threads.

It appears to me that one difference between religious and non-religious signing in comments on YouTube discussion threads for African music videos, is that some commenters include testimonies (religious statements about their experience/s with God and/or how this song blesses them) with their religious nsi comments.

Waziri Mkuu ambaye alikuwa mgeni rasmi kwenye ufunguzi wa michuano hiyo, aliambatana na Rais wa Shirikisho la Soka na barani Afrika (CAF), Ahmad Ahmad kukagua timu zilizofungua dimba na kisha akatangaza uamuzi huo wa Serikali.

Timu zilizofungua dimba leo ni Serengeti Boys ya Tanzania na Nigeria. Katika mchezo wa leo Nigeria imeibuka kidedea baada ya kuifunga Tanzania mabao 5-4. Hadi mapumziko timu ya Nigeria ilikuwa ikiongoza kwa mabao 3-1.

Mabao ya Nigeria yalipatikana dakika ya 20, 29, 36, 71 na 78 wakati mabao ya Tanzania yalipatikana dakika ya 21, 51, 56 na 60 ambapo magoli mawili kati ya hayo, yalipatikana kwa njia ya penati.

Bao la kwanza la Tanzania lililofungwa na Alphonce Mabula Msanga lilitinga kimiani dakika ya 21. Bao la pili, lilipatikana dakika ya sita ya kipindi cha pili na lilifungwa na Kelvin Pius John.

Dakika tano baadaye, Morice Michael Abraham aliipatia Tanzania bao la tatu lililofungwa kwa njia ya penati. Kabla vijana wa Nigeria hawajakaa sawa, dakika nne nyingine, Edmund Godfrey John alitinga kimiani bao la nne, ambalo pia lililifungwa kwa penati.

Timu ya Tanzania iko kundi A na imepangwa na timu za Nigeria, Uganda na Angola. Michuano hii inatarajiwa kumalizika Aprili 28. Endapo timu hiyo ya Taifa ya vijana ya Serengeti Boys itaibuka na ushindi katika michezo yake miwili, itakuwa imekata tiketi ya kushiriki mashindano ya dunia kwa vijana wa umri wa miaka 17 yatakayofanyika mwakani huko Brazil.
