imageSeventeen years have passed since the first report of the synthesis of peptides with antimicrobial activity in the skin of African clawed frog, Xenopus laevis. Since that time more than 200 such peptides have been isolated from the skins of other frogs belonging to different families. With very few exceptions, the frog skin antimicrobial peptides are cationic, relatively hydrophobic and have the propensity to form an amphipathic α-helix in a membrane-mimetic environment. By making selective changes to the molecular charge, hydrophobicity and secondary structure of the naturally occurring peptides, analogues have been developed that show increased potency against microorganisms but reduced toxicity towards mammalian cells. Because of their non-specific mechanism of producing cell death, pathogens develop resistance to the antimicrobial peptides at much lower rates than to conventional antibiotics. The magainins, dermaseptins, ranalexin, brevinins, ranatuerin-1 and buforin-2 (and their analogues) have received particular attention with regard to possible clinical applications. However, despite showing broad-spectrum activity against strains of antibiotic-resistant bacteria and potent activity against certain pathogenic fungi and protozoa, their therapeutic potential remains to be realized. No anti-infective peptide based upon their structures has yet been adopted in clinical practice. New clinical applications need to be found if progress in the field is to continue.

imagePlesiomonas shigelloides is a Gram-negative, motile and oxidase-positive bacterium that is widely distributed in nature. It is also a significant pathogen causing mainly intestinal diseases in humans. Diarrhoea is the leading symptom of most of these infections and may occur as a watery, invasive and chronic form. Food- and water-borne outbreaks of intestinal infections due to Plesiomonas have been reported. P. shigelloides also causes a variety of extraintestinal infections with high mortality rates; sepsis and meningitis represent the most common extraintestinal forms of disease. In spite of its close phylogenetic relationship to other Enterobacteriaceae, P. shigelloides is biochemically distant from other species of this family. One biovar, but more than 100 serovars have been described. P. shigelloides is thermo-, alkali-, acido- and halotolerant and has been discussed as a ‘natural’ vaccine against shigellosis. In the laboratory, Plesiomonas appears inconspicuous on the surface of several agar plates, and some enteric media are known to inhibit its growth. Plesiomonas shows unusual antibiotic susceptibility patterns, its susceptibility to some agents is highly dependent on inoculum size. The formation of extensive cell filamentation at high bacterial densities in the presence of certain β-lactam antibiotics is distinctive.

Now in preview, you can migrate PostgreSQL databases to Azure Database for PostgreSQL with minimal downtime by using the Azure Database Migration Service (DMS).   • The US Dollar Index (DXY) is trading in a bull channel but the price has actually made no progress since late May.
  • DXY is forming a head-and-should pattern and although the shape of it is not perfect the bearish dynamics at play remains the same. DXY has still quite a lot of resistance to go through if the bears want to take control: the bull trendline from May 14, 94.43 support (August 28 swing low) and the 100-day simple moving average (SMA). Interestingly, the RSI, the MACD and the Stochastics have turned bearish. 
  • A bear breakout below 94.43 is needed to confirm a neutral to bearish bias while a bull breakout above 96.65 would be considered as the resumption of the main bull trend.

          Outcome Analysis of Helmet Therapy for Positional Plagiocephaly Using a Three-Dimensional Surface Scanning Laser      Cache   Translate Page      
imageIntroduction: A dramatic rise in positional plagiocephaly has been noted over the last decade. Methods for treating and following outcomes are varied. We present our results from a passive soft helmet molding therapy using a surface scanning laser to provide objective outcomes. Methods: One hundred seventy-five infants with positional plagiocephaly were treated at our institution over a 6-month period. The helmets used were soft foam helmets modified to fit each patient. Results were measured using a three-dimensional surface scanning laser and objective measurements of asymmetry. Compliance was also documented. Results: The scanning laser allowed for objective measurements of symmetry over time and was useful in following outcomes. Improvement in head shape was noted after about 4 months of therapy. Patients who were more compliant with therapy achieved better results. Conclusion: Use of the three-dimensional scanning laser and objective scoring system shows significant improvement in patients who were compliant with helmet therapy.

imageThe objective of this study was to review the medical records of patients with a history of mandibular angle fracture who were attended at the Service of Oral and Maxillofacial Surgery and Traumatology of FOAr/UNESP in the last 5 years. The data collected were subjected to chi-squared test (significance level of 5%). The authors reviewed 19 medical records. The main cause was physical aggression (58.00%), but with no statistical difference in relation to the other etiologies (P > 0.05). Regarding the type of fixation, one 2.0-mm system plate associated with one 2.4-mm system plate and the fixation using only two 2.0-mm system plates were used in 7 patients each. The fixation method with a monocortical plate at the upper border was used in 5 patients. However, there was no statistically significant difference in the frequency of complications among the 3 fixation methods used (P > 0.05). In 52.64% of the patients, the third molar was removed intraoperatively. Despite this, there was no statistically significant difference in the frequency of complications when the third molar was in the fracture line or when it was removed postoperatively (P > 0.05). The complications observed were dehiscence associated with pain (44.44%), trismus (22.22%), infection (22.22%), and presence of bone spicules (11.11%). However, no statistical differences were observed between the frequency of different types of complications (P = 0.779). In the sample studied, there were no differences in the frequency of complications among the fixation methods applied.

imageAbstract In adults, paranasal sinus agenesis is an uncommon anomaly that appears mainly in the frontal sinus. The aim of this study was to investigate the prevalence of agenesis of the frontal sinus using computed tomography in a population of Iranian individuals. Computed tomographic scans in the axial and coronal planes of the frontal sinus of 565 patients were examined for evidence of the absence of the frontal sinus. A bilateral agenesis of the frontal sinus was seen in 8.32% of cases, and unilateral absence of the frontal sinus was observed in 5.66%. The information provided in the current study is useful for some craniofacial surgeries and improves the anthropometric knowledge of humanity. There are limited studies on agenesis of the frontal sinus in the literature, but comparing our findings with the available data, there were differences regarding the incidence of this condition. This may be related to geographic and racial features. More studies on other population are required.

imageStudy Design A comprehensive survey of literature on the proposed mechanisms and treatment of pain and spasticity after spinal cord injury (SCI) was completed. Objectives To define the current understanding of these entities and to review various treatment options. Summary of Background Data. The neurophysiologic basis of spasticity after SCI is well established. The mechanism of neuropathic pain after SCI remains conjectural, although considerable new data, much of it from animal models, now add to our understanding of this condition. Methods. A comprehensive search and review of the published literature was undertaken. Results. Treatment options for spasticity are effective and include oral medication (baclofen, tizanidine), intrathecal baclofen, and rarely, surgical rhizotomy or myelotomy. Selected patients with post–SCI pain can respond to surgical myelotomy (DREZ lesions) or intrathecal agents (e.g., morphine + clonidine), but the majority continue to suffer. Conclusions. Medical and surgical treatments for spasticity are established and highly successful. Management of post-SCI pain remains a clinical challenge, as there is no uniformly successful medical or surgical treatment.

imageStudy Design. Evidence-based literature review. Objective. To provide updated evidence-based recommendations regarding spinal cord decompression in patients with acute spinal cord injury (SCI). Summary of Background Data. It is controversial whether early decompression following SCI conveys a benefit in neurologic outcome. Methods. MEDLINE search of experimental and clinical studies showing the effect of decompression on neurologic outcome following SCI. We focused on articles published within the last 10 years, with a particular emphasis on research conducted within the past 5 years. Results. A total of 66 articles were retrieved. Animal studies consistently show that neurologic recovery is enhanced by early decompression. There was 1 randomized controlled trial that showed no benefit to early (<72 hours) decompression. Several recent prospective series suggest that early decompression (<72 hours) can be performed safely and may improve neurologic outcomes. A recent systematic review showed that early decompression (<24 hours) resulted in statistically better outcomes compared to both delayed decompression and conservative treatment. Conclusions. There are currently no standards regarding the role and timing of decompression in acute SCI. We recommend urgent decompression of bilateral locked facets in a patient with incomplete tetraplegia or in a patient with SCI with neurologic deterioration. Urgent decompression in acute cervical SCI remains a reasonable practice option and can be performed safely. There is emerging evidence that surgery within 24 hours may reduce length of intensive care unit stay and reduce post-injury medical complications.

Photographic records and films were made for documentation of represented muscles at rest and contraction. Results: Using the body painting technique, each of the muscles involved in facial expression and generation of hyperkinetic wrinkles can be faithfully reproduced on the model’s face. The documentation of the exact position of the points of application, the distribution of the feature points in the muscular area, the proper angulation and syringe grip, as well as the correlation of the points of application with the presence of hyperkinetic wrinkles, could be properly registered, providing professional training with information of great practical importance, development of highly effective treatments, and low complication rates. Conclusion: By making it possible to interrelate anatomy of a function, body painting is proposed in the present study as an innovative method, which in a demonstrative and highly didactic manner presents great potential as a teaching tool in the application of botulinum toxin A.

Hypospadias are one of the most common inherent genital anomalies in boys. Many surgical procedures have been published for total phallic reconstruction aiming at a functionally and aesthetically pleasing. We present a case of reconstruction of the penis in a severe hypospadias in a 40-year-old man by transferring the original glans to the forearm flap and a stiffening procedure with an osteocutaneous fibular flap 3 months after first surgery. Three months postoperatively, the final result was an acceptable sexual intercourse, normal voiding, and quite normal appearance. Microvascular staged phalloplasty preserving original glans in a severe hypospadia could be considered a surgical option for micropenis. Technical difficulties and microsurgical advanced skills are the main drawback of this approach.

          Why Containers are the Future      Cache   Translate Page      

Software deployment has been a major problem for decades. On the client and the server.

On the client, the inability to deploy apps to devices without breaking other apps (or sometimes the client operating system (OS)) has pushed most business software development to relying entirely on the client's browser as a runtime. Or in some cases you may leverage the deployment models of per-platform "stores" from Apple, Google, or Microsoft.

On the server, all sorts of solutions have been attempted, including complex and costly server-side management/deployment software. Over the past many years the industry has mostly gravitated toward the use of virtual machines (VMs) to ease some of the pain, but the costly server-side management software remains critical.

At some point containers may revolutionize client deployment, but right now they are in the process of revolutionizing server deployment, and that's where I'll focus in the remainder of this post.

Fairly recently the concept of containers, most widely recognized with Docker, has gained rapid acceptance.

tl;dr

Containers offer numerous benefits over older IT models such as virtual machines. Containers integrate smoothly into DevOps; streamlining and stabilizing the move from source code to deployable assets. Containers also standardize the deployment and runtime model for applications and services in production (and test/staging). Containers are an enabling technology for microservice architecture and DevOps.

Virtual Machines to Containers

Containers are somewhat like virtual machines, except they are much lighter weight and thus offer major benefits. A VM virtualizes the hardware, allowing installation of the OS on "fake" hardware, and your software is installed and run on that OS. A container virtualizes the OS, allowing you to install and run your software on this "fake" OS.

In other words, containers virtualize at a higher level than VMs. This means that where a VM takes many seconds to literally boot up the OS, a container doesn't boot up at all, the OS is already there. It just loads and starts our application code. This takes fractions of a second.

Where a VM has a virtual hard drive that contains the entire OS, plus your application code, plus everything else the OS might possibly need, a container has an image file that contains your application code and any dependencies required by that app. As a result, the image files for a container are much smaller than a VM hard drive.

Container image files are stored in a repository so they can be easily managed and then downloaded to physical servers for execution. This is possible because they are so much smaller than a virtual hard drive, and the result is a much more flexible and powerful deployment model.

Containers vs PaaS/FaaS

Platform as a Service and Functions as a Service have become very popular ways to build and deploy software, especially in public clouds such as Microsoft Azure. Sometimes FaaS is also referred to as "serverless" computing, because your code only uses resources while running, and otherwise doesn't consume server resources; hence being "serverless".

The thing to keep in mind is that PaaS and FaaS are both really examples of container-based computing. Your cloud vendor creates a container that includes an OS and various other platform-level dependencies such as the .NET Framework, nodejs, Python, the JDK, etc. You install your code into that pre-built environment and it runs. This is true whether you are using PaaS to host a web site, or FaaS to host a function written in C#, JavaScript, or Java.

I always think of this as a spectrum. On one end are virtual machines, on the other is PaaS/FaaS, and in the middle are Docker containers.

VMs give you total control at the cost of you needing to manage everything. You are forced to manage machines at all levels, from OS updates and patches, to installation and management of platform dependencies like .NET and the JDK. Worse, there's no guarantee of consistency between instances of your VMs because each one is managed separately.

PaaS/FaaS give you essentially zero control. The vendor manages everything - you are forced to live within their runtime (container) model, upgrade when they say upgrade, and only use versions of the platform they currently support. You can't get ahead or fall behind the vendor.

Containers such as Docker give you some abstraction and some control. You get to pick a consistent base image and add in the dependencies your code requires. So there's consistency and maintainability that's far superior to a VM, but not as restrictive as PaaS/FaaS.

Another key aspect to keep in mind, is that PaaS/FaaS models are vendor specific. Containers are universally supported by all major cloud vendors, meaning that the code you host in your containers is entirely separated from anything specific to a given cloud vendor.

Containers and DevOps

DevOps has become the dominant way organizations think about the development, security, QA, deployment, and runtime monitoring of apps. When it comes to deployment, containers allow the image file to be the output of the build process.

With a VM model, the build process produces assets that must be then deployed into a VM. But with containers, the build process produces the actual image that will be loaded at runtime. No need to deploy the app or its dependencies, because they are already in the image itself.

This allows the DevOps pipeline to directly output a file, and that file is the unit of deployment!

No longer are IT professionals needed to deploy apps and dependencies onto the OS. Or even to configure the OS, because the app, dependencies, and configuration are all part of the DevOps process. In fact, all those definitions are source code, and so are subject to change tracking where you can see the history of all changes.

Servers and Orchestration

I'm not saying IT professionals aren't needed anymore. At the end of the day containers do run on actual servers, and those servers have their own OS plus the software to manage container execution. There are also some complexities around networking at the host OS and container levels. And there's the need to support load distribution, geographic distribution, failover, fault tolerance, and all the other things IT pros need to provide in any data center scenario.

With containers the industry is settling on a technology called Kubernetes (K8S) as the primary way to host and manage containers on servers.

Installing and configuring K8S is not trivial. You may choose to do your own K8S deployment in your data center, but increasingly organizations are choosing to rely on managed K8S services. Google, Microsoft, and Amazon all have managed Kubernetes offerings in their public clouds. If you can't use a public cloud, then you might consider using on-premises clouds such as Azure Stack or OpenStack, where you can also gain access to K8S without the need for manual installation and configuration.

Regardless of whether you use a managed public or private K8S cloud solution, or set up your own, the result of having K8S is that you have the tools to manage running container instances across multiple physical servers, and possibly geographic data centers.

Managed public and private clouds provide not only K8S, but also the hardware and managed host operating systems, meaning that your IT professionals can focus purely on managing network traffic, security, and other critical aspects. If you host your own K8S then your IT pro staff also own the management of hardware and the host OS on each server.

In any case, containers and K8S radically reduce the workload for IT pros in terms of managing the myriad VMs needed to host modern microservice-based apps, because those VMs are replaced by container images, managed via source code and the DevOps process.

Containers and Microservices

Microservice architecture is primarily about creating and running individual services that work together to provide rich functionality as an overall system.

A primary attribute (in my view the primary attribute) of services is that they are loosely coupled, sharing no dependencies between services. Each service should be deployed separately as well, allowing for indendent versioning of each service without needing to deploy any other services in the system.

Because containers are a self-contained unit of deployment, they are a great match for a service-based architecture. If we consider that each service is a stand-alone, atomic application that must be independently deployed, then it is easy to see how each service belongs in its own container image.

This approach means that each service, along with its dependencies, become a deployable unit that can be orchestrated via K8S.

Services that change rapidly can be deployed frequently. Services that change rarely can be deployed only when necessary. So you can easily envision services that deploy hourly, daily, or weekly, while other services will deploy once and remain stable and unchanged for months or years.

Conclusion

Clearly I am very positive about the potential of containers to benefit software development and deployment. I think this technology provides a nice compromise between virtual machines and PaaS, while providing a vendor-neutral model for hosting apps and services.

Obwohl er darum bat, keinen Senf auf seinen Burger zu bekommen, bekam er Senf.“

Der Reddit-Nutzer schreibt, dass der Kunde eine Tabelle erstellte, in die er alle Fehler eintrug, die Angestellte von McDonald's bei seinem Essen machten. Mackenzie Shelton, laut ihrer Quora-Biografie eine Angestellte bei McDonald's, berichtet von einem Trick, wie man ein frisches Ei auf seinem Frühstücks-Burger bekommen kann. Bittet um ein „rundes Ei“, so Shelton. „Es ist das beste Ei, das wir haben und definitiv echt!“

Die Ernährungswebseite Serious Eats“ bestätigte, dass die Bitte tatsächlich dazu führen kann, dass „euer zusammengeklapptes Spiegelei ohne zusätzliche Kosten durch ein frisches Ei ersetzt wird.“ 



Angestellte werden oft mit ungewöhnlichen Bitten konfrontiert

Manchmal haben Kunden merkwürdige Bitten an die Angestellten von McDonald's. Mike Bowerbank, der sagt, er habe bei McDonald's gearbeitet, schreibt auf Quora, dass eine Frau einmal den McDLT ohne Fleisch haben wollte. Ganesh Satyanarayana schreibt auf Quora, dass er kurz nach Ladenschluss auf eine „sichtbar schwangere“ Kundin stieß. „Sie fragte mich ruhig, ob wir noch geöffnet hätten, da sie Gelüste nach einem Burger hätte und alle anderen Filialen bereits geschlossen hätten“, so Satyanarayana. „Mein Herz schmolz sofort, so dass ich sie reinließ und ihr sagte, dass ich sie noch bedienen könnte, sobald ich mit dem Putzen fertig wäre.“ Daraufhin antwortete sie: „Oh, ich will kein Fleisch auf meinem Burger, ich will nur Essiggurken auf einem getoasteten Brötchen.“ Satyanarayana schreibt dazu: „Ich schwöre bei Gott, dass ich an einen Scherz dachte.“

Hello everyone! Today we are announcing the release of update 1809 for the Technical Preview Branch of System Center Configuration Manager. This release gives you the ability to visualize your CMPivot results using the Azure Log Analytics render operator. You can now choose from the following types of visualizations: bar chart, column chart, pie chart and time chart. Visualizations help you analyze query results and discover key insights, so you can more quickly answer business questions, troubleshoot issues, and respond to security incidents.

CMPivot now also includes the ability to query and pivot on hardware inventory class information. This not only significantly enhances what can be queried out-of-the-box, but also enables a standardized way to extend CMPivot by adding new hardware inventory class definitions. Even better, CMPivot will immediately render data from the last hardware inventory scan while simultaneously pulling live data from online clients -- giving you a view across online and offline devices!

 

Other improvements to CMPivot include support for scalar functions and scalar operators, and a query summary which displays the status of the query on the device where it was executed. Try it out and let us know what you think about it.

こんにちは、さとうなおきです。今回の「週刊アジュール」では、2018年8月26日~9月1日の1週間に発表されたMicrosoft Azureの新機能から、筆者の独断と偏見で選んだトピックについて紹介していきます。



          Accuracy of Cause of Death Determination Without Forensic Autopsy Examination      Cache   Translate Page      
imageMedical examiners and coroners commonly determine cause and manner of death without an autopsy examination. Some death certificates generated in this way may not state the correct cause and manner of death. From the case files of the Department of Forensic Medicine in Sydney, Australia, the authors retrospectively reviewed investigative information of all cases in a 6-month period that were initially considered natural deaths (429). The authors, blinded to autopsy results, accepted 261 cases as appropriate for certification without autopsy and assigned a cause of death to each. Per standard local practice, all cases had been autopsied. The actual causes of death as determined by autopsy were then revealed and compared with the presumed causes of death. Most presumed and actual causes of death were cardiovascular (94% and 80%, respectively). The majority of presumed causes of death were listed as ASCVD as the cases lacked features of a more specific cardiovascular process. A large majority of cases had a presumed cause of death of ischemic heart disease based on individual case details. The actual causes of death demonstrated a large breadth of cardiovascular and noncardiovascular disease processes, even though ischemic heart disease accounted for 62% of deaths. The presumed cause of death was completely wrong in 28% of cases. A nonnatural manner of death was present in 3% of cases. This study demonstrates that experienced forensic pathologists may generate erroneous death certificates for cases that are not autopsied.

imageTo examine the pattern of injuries in cases of fatal shark attack in South Australian waters, the authors examined the files of their institution for all cases of shark attack in which full autopsies had been performed over the past 25 years, from 1974 to 1998. Of the seven deaths attributed to shark attack during this period, full autopsies were performed in only two cases. In the remaining five cases, bodies either had not been found or were incomplete. Case 1 was a 27-year-old male surfer who had been attacked by a shark. At autopsy, the main areas of injury involved the right thigh, which displayed characteristic teeth marks, extensive soft tissue damage, and incision of the femoral artery. There were also incised wounds of the right wrist. Bony injury was minimal, and no shark teeth were recovered. Case 2 was a 26-year-old male diver who had been attacked by a shark. At autopsy, the main areas of injury involved the left thigh and lower leg, which displayed characteristic teeth marks, extensive soft tissue damage, and incised wounds of the femoral artery and vein. There was also soft tissue trauma to the left wrist, with transection of the radial artery and vein. Bony injury was minimal, and no shark teeth were recovered.In both cases, death resulted from exsanguination following a similar pattern of soft tissue and vascular damage to a leg and arm. This type of injury is in keeping with predator attack from underneath or behind, with the most severe injuries involving one leg. Less severe injuries to the arms may have occurred during the ensuing struggle. Reconstruction of the damaged limb in case 2 by sewing together skin, soft tissue, and muscle bundles not only revealed that no soft tissue was missing but also gave a clearer picture of the pattern of teeth marks, direction of the attack, and species of predator.
          Diagnostic Problems Associated with Cadaveric Trauma from Animal Activity      Cache   Translate Page      
imageAnalysis of a series of deaths between 1986 and 2001 resulting from natural disease, accidents, suicides, and homicide, where postmortem animal activity had traumatized bodies, was undertaken at the Forensic Science Center in Adelaide to demonstrate the range of lesions that may occur and problems in interpretation that result. Tissue damage had been caused by a variety of animals, including fly larvae, ants, birds, dogs, rodents, sea lice, and sharks. Postmortem animal activity had disguised injuries, modified wounds, and created the appearances of inflicted injury. Problems with identification occurred after postmortem facial trauma, and loss of organ parenchyma had interfered with, or precluded, the precise determination of the manner of death in some cases. Specific kinds of tissue and organ damage may occur after death, necessitating careful assessment of lesions in a search for characteristic features of animal activity. The pattern of lesions may enable identification of the particular species of animal involved.
          High-Speed Documented Experimental Gunshot to a Skull-Brain Model and Radiologic Virtual Autopsy      Cache   Translate Page      
imageThe authors documented and evaluated experimental gunshots to a skull-brain model with high-speed photography and subsequent radiographic examination for comparison of the morphologic findings in the model. The artificial skull was a polyurethane ball constructed in layers, with a porous diploe sandwiched between a tabula externa and a tabula interna. The brain itself was simulated with gelatin 10% at 4°C, a material well known in wound ballistics. Gunshots were fired at the model from a distance of 10 m and documented with high-speed photography (up to 50 million frames/sec). Subsequently, a complete examination of the artificial skull was performed, including spiral computed tomography (with two-dimensional and three-dimensional reconstructions) and classic skull autopsy. The high-speed photographs clearly showed the dynamic development of the skull fracture system from an external perspective. The subsequent radiographic examination of the entire head volume created two-dimensional reformations in any plane and three-dimensional reconstructions of the gunshot injury of the polyurethane skull-brain model, especially the wound channel and the fracture system. Thanks to the model and high-speed photographs, the dynamic development of the morphology of a gunshot wound could be documented and studied. The data from computed tomography, using two-dimensional and three-dimensional postprocessing with a perspective view, were very similar to those from classic head autopsy, but derived in a hands-off and nondestructive manner. This examination method leads the way to radiographic digital autopsy or virtual autopsy.

imageAssisted living (AL) residences are residential long-term care settings that provide housing, 24-hour oversight, personal care services, health-related services, or a combination of these on an as-needed basis. Most residents require some assistance with activities of daily living and instrumental activities of daily living, such as medication management. A resident plan of care (ie, service agreement) is developed to address the health and psychosocial needs of the resident. The amount and type of care provided, and the individual who provides that care, vary on the basis of state regulations and what services are provided within the facility. Some states require that an RN hold a leadership position to oversee medication management and other aspects of care within the facility. A licensed practical nurse/licensed vocational nurse can supervise the day-to-day direct care within the facility. The majority of direct care in AL settings is provided by direct care workers (DCWs), including certified nursing assistants or unlicensed providers. The scope of practice of a DCW varies by state and the legal structure within that state. In some states, the DCW is exempt from the nurse practice act, and in some states, the DCW may practice within a specific scope such as being a medication aide. In most states, however, the DCW scope of practice is conscribed, in part, by the delegation of responsibilities (such as medication administration) by a supervising RN. The issue of RN delegation has become the subject of ongoing discussion for AL residents, facilities, and regulators and for the nursing profession. The purpose of this article is to review delegation in AL and to provide recommendations for future practice and research in this area.

Даже несмотря на отсутствие у меня симпатий к сделавшей её корпорации, должен признать, что платформа обладает рядом удобств по сравнению с конкурентами. Но нет бочки мёда без ложки дёгтя, и этой ложкой в Microsoft Azure являются имена компонентов системы. Если в AWS переименовать компонент — дело двух кликов, то в Azure имена намертво привязываются к компонентам, и изменить их без танцев с бубном порой невозможно. Поэтому очень важно при работе с Azure изначально создать правильную структуру.

          Effects of Partner Support on Resting Oxytocin, Cortisol, Norepinephrine, and Blood Pressure Before and After Warm Partner Contact      Cache   Translate Page      
imageObjective: We examined whether the magnitude of plasma oxytocin (OT), norepinephrine (NE), cortisol, and blood pressure (BP) responses before and after a brief episode of warm contact (WC) with the spouse/partner may be related to the strength of perceived partner support. Methods: Subjects were 38 cohabiting couples (38 men, 38 women) aged 20 to 49 years. All underwent 10 minutes of resting baseline alone, 10 minutes of WC together with their partner, and 10 minutes of postcontact rest alone. Results: Greater partner support (based on self-report) was related to higher plasma oxytocin in men and women across the protocol before and after WC. In women, higher partner support was correlated with lower systolic blood pressure (SBP) during solitary rest after WC but not before. Also, higher OT in women was linked to lower BP at baseline and to lower NE at all 4 measurements. Conclusion: Greater partner support is linked to higher OT for both men and women; however, the importance of OT and its potentially cardioprotective effects on sympathetic activity and BP may be greater for women. Hurricane Florence is coming to the east coast


The deadly hurricane Florence is making it’s way into the East Coast of the US at 16 MPH, right now measured as a category 3 hurricane of 335 miles in diameter and expected to grow back into a category 4 on Thursday, this dangerous phenomenon is being described as “The storm of a lifetime” and is threatening the lives of 10 million people.

US president Donald Trump said that this will be one of the biggest hurricanes to ever hit the US, and has urged civilians to heed the instructions of of state and local officials, he added “It is imperative that everyone follow local evacuation orders. This storm is extremely dangerous. Be SAFE!”

The warnings and watches range from Virginia to South Carolina. A Storm Surge Warning and a Hurricane Warning are both in effect for South Santee River, South Carolina to Duck, North Carolina and the Albemarle and Pamlico Sounds, including the Neuse and Pamlico Rivers. A Storm Surge Watch and a Hurricane Watch is in effect for Edisto Beach, South Carolina to South Santee River, South Carolina.

A Tropical Storm Warning and Storm Surge Watch is in effect for north of Duck, North Carolina to the North Carolina/Virginia border. A Tropical Storm Watch is in effect for north of the North Carolina/Virginia border to Cape Charles Light, Virginia and for the Chesapeake Bay south of New Point Comfort.

Florence had the strength of a category 4 hurricane and was over 400 miles in diameter this Wednesday morning, but lost some momentum over the day, however Federal Emergency Management Agency (FEMA) administrator Jeff Byard warned that despite its slight weakening, Florence will still be an extremely dangerous hurricane, and that it will likely regain it’s power tomorrow morning, and touch ground on Thursday night or on Friday.

“Do you want to get hit with a train or do you want to get hit with a cement truck,” Byard asked.

Florence will pass over two nuclear power plants — the Brunswick Nuclear Plant, which is located 30 miles south of Wilmington, as well as the Shearon Harris Nuclear Plant in New Hill, about 23 miles from Raleigh according to the proyected route.

I'm currently getting up to speed with the Azure SignalR Service (Preview), and have looked at the aspnet core and straight .net samples.

One of the interesting samples is:

Which allows a connection to the SignalR Hub from a .net console to console.

I would like to utilize this, and have modified it with reference to the Azure SignalR Swagger (https://github.com/Azure/azure-signalr/blob/dev/docs/swagger/v1-preview.json) for definition of services available and am able to broadcast to all clients, or to specifics.

However I am having a problem registering the connecting users to groups for the service to send messages to specific groups.

I can see from the Swagger that I can send messages to groups, in a similar way to users (by posting to /api/v1-preview/hub/{hub}/group/{group}) however, I cannot see anything outside of a Hub class - which this sample would not have, to do the equivalent of 

Groups.AddToGroupAsync(userName, groupName);

which works in the aspnet core examples when within a hub, but I am unable to find a non-hub equivalent.

Some direction on how I go about adding a user to a group outside of the hub would be very helpful.

Microsoft announced some changes to their Visual Studio Team Services production today, primary of which was a name change. VSTS now becomes Azure DevOps, which is made up of Azure Repositories, Azure Pipelines, Azure Boards, Azure Test Plans and Azure Artifacts. This update also included a new UI design and URLs. The full announcement can be found here

The most exciting part of this for me, however, was the announcement of unlimited, free, Azure Pipelines for building open source projects hosted on Github. By enabling Azure Pipelines for your Github projects, you can run builds for as many minutes as you want, for free. The only limitation is that you can only have 10 concurrent builds running at the same time. This is a great new feature for open source projects.

I host many Open Source projects on Github, so I thought I would take a look at enabling Azure Pipelines on one of these. The project I chose was the VS Code Azure Snippets project - https://github.com/sam-cogan/arm-snippets-vscode/ .

Enable Azure Pipelines

The first thing we need to do is enable Azure Pipelines for our Github account. There is an app already available in the Github Marketplace, so we can go and search for that and select it.

Scroll down to the bottom of the page, and you can see the option to "install for free."

Once you click on that, you'll then be shown the price for your order, free!

Click on "Complete Order", and you'll then be asked to grant permissions to your Repos for Azure Pipelines. You can grant it access to all your repositories or a specific repository.

You've now authorised the app on your account; now you need to link it to an Azure Pipelines workspace. You'll be presented with a window asking you to select a project or create a new one. If you already have a VSTS or Azure DevOps workspace setup you can choose to use your existing ones (you'll still get the free builds for your open source projects), if not you can use this wizard to create one.

For this demo, we will go ahead and create a brand new organisation and project.

It will take a minute to create you a project; then it will take you to New Pipeline wizard to create your first pipeline. The first thing you need to do is select the repository you want to use.

Once selected, the wizard will parse your project and then suggest a template based on the language in use. In my project, it offered Node, as some of the VS Code plugin processes uses Node.

You can accept the template it offers, or one of the other suggest templates, search all templates, or start with a simple template. Given that I already had a build process defined for my project, I elected to use the simple template.

You will then be presented with your template in YAML format, with the option to Save and Run this.

Once you hit Save, it will kick off an initial build with your default template, and you can confirm that it all works, at least with the default options.

Creating your Pipeline

Now that you've set everything up you need to go ahead and edit your pipeline so that it does what you want. This is where I feel Microsoft have let things down a little. In the full version of VSTS/Azure DevOps, you get a graphical workflow editor to create your build as well as the option to edit the YAML file behind this manually. In the free pipelines it seems (as far as I can tell) they have limited you to just using the YAML file option, I could not find any way to enable the visual designer.

So, to define your build, you are going to need to edit the azure-pipelines.yaml file that was created in your repository to add the steps you need in your process. The syntax to use for all types of steps is documented here . In my project I need to do the following:

Get my code from Github Run NPM Install Move some files to the appropriate locations Run a PowerShell Script Run VSCE to create the extension Publish an artifact of the extension package

My azure-pipelines file looks like this:

The build definition is split into two sections, setup and tasks. In my setup section I do the following:

Indicate that the task should pull from this git repo Run a clean before pulling Use a hosted build agent Demand the build agent support .net and npm Define a name, which is the build number for the pipeline resources: - repo: self clean: true queue: name: Hosted demands: - npm - DotNetFramework name: 1.3$(Rev:.r)

Then I define my tasks inside the "steps" section to undertake each of my defined tasks.

Now we have defined a build, we need to run it. Builds in your pipeline will be triggered in 3 ways

Manually by clicking on the build button Automatically when you commit changes directly the master branch (which you shouldn't be doing) or when you merge a pull request into the master branch Automatically when you submit a pull request

You can also configure more triggers for builds (such as commits on other branches) in your build settings.

Once a build has been triggered, it shows up in the build pane, and you can view its status, If you click on a specific build you can then view the individual tasks and any logs or errors associated with them.

Hopefully, your build runs through the first time. Once it completes, any artifacts are available on the build results summary page. You can also retrieve your build status page from the menu on the build page if you want to display this in your repo. This will be updated automatically.

Summary

This is just a swift run through of how you can hook up your Github projects to Azure Pipelines and take advantage of the completely free builds. In my view, this could be a handy tool for Open Source projects. I am a bit disappointed with the lack of the visual designer; hopefully, this will get added in the future, but aside from that this is a very nice new feature. We've only scratched the surface of what you can do with Azure Pipeline in this article (we haven't even touched the release management components), so do go and test it out for yourself.

Today Microsoft announced Azure DevOps , which is partly a rebranding of the existing Visual Studio Team Services but also has some exciting news.

The gist of the rebranding is that Azure DevOps is now a suite of service, where each service cna be acquired and used separately from the other ones. If you only want to use source control (and use some other CI/CD system) that’s fine.

Do you have your code over at GitHub and want to use the CI/CD services in Azure DevOps? Works perfectly! By breaking the whole suite down into smaller services, it will make it easier for customers to find the best fit for their needs, without having to invest in the whole suite. Of course, you will still be able to easily get the whole suite when creating new accounts.

Mohammad: Do you think people who dismiss the cloud as a “fad” or just don’t take it serious enough to learn about it (i.e. Azure, AWS, etc), will be in a tough spot to find a job 5 years from now?

John:The trend I am seeing is for more of a hybrid data platform environment where there is a diverse mix of SQL Server both on-premises and in cloud platforms. I am seeing more and more companies look at the Platform as a Service offerings that are on offer such as Azure SQL Database, Amazon RDS for SQL Server, and Azure SQL Database Managed Instance. The latter option with its high degree of parity with the retail SQL Server product makes the jump from managing full blown SQL Servers to PaaS semi-manged platforms a lot more viable.

Another trend that I am seeing is that companies are moving to a more heterogenous data platform environment where it is more than just SQL Server in play. While SQL Server and relational engines rule the roost for many line of business applications there is a big move to document database platforms like Cosmos and MongoDB, as well as other technologies like Spark, Databricks, Hadoop, Casandra, etc. The key thing here being to use the right technology for the right role in the data platform solutions, a great example being the Modern Data Warehousing pattern using Lambda architecture. You can find out more by watching this fantastic session by Simon Whiteley ( t | b ) from Adatis thanks to GroupBy conference.

Mohammad: Do you ever see the traditional SQL Server DBA role being replaced/eliminated?

John: I can never see the DBA being eliminated, I do however see the role evolving with the technology that we use for data platform systems. Unlike Storage or Networking which are being made self-serve by cloud and appliance vendors, there is always going to be a need to manage data. It is easy to setup a data system that collects vast quantities of data. However, it is difficult to do it well and make it perform while incurring minimum overheads in cost or resource use. I see the role of the DBA becoming more of a Data Platform Engineer where we need to go beyond the relational engine of SQL Server and will need to guide businesses in when to use and when not to use SQL Server. Along with needing to learn core cloud platform skills around networking, identity, and storage in addition to adding other data platform technologies to our skillset.

Mohammad:What are you most proud of doing/accomplishing for the SQL Server community so far in your career?

John: There is no one event that sticks in the mind when it comes to the community. The one thing that I feel proud of is that I have been able to help people understand the topics that I speak and blog about. When I was starting out I used to attend the user group events held at Microsoft’s offices in Reading and that was how I got really interested and learned my trade. The skills and advice I got from the speakers and other attendees is what helped me get where I am, being part of that and helping others is what is important to me.

Mohammad: What non-technical/non-fiction book/s would you recommend? If you only read technical books…what do you recommend?

John: I very rarely read nonfiction or non-technical books these days, though the two that I return to every now and then are ‘ The Sword & the Mind ’ and Descartes Meditations on First Philosophy . Neither of them are going to be in any best seller lists any time soon, but they are a challenging read and each time I go back to them I gain an appreciation on how to focus my mind, form better arguments, and alter the way I think to meet new challenges.

Mohammad: For someone who’s career focus has been on one aspect of SQL Server (i.e. Database Engine), do you think it would be wise for them to become a “jack of all trades” by starting to learn, SSRS/IS/Azure, etc. or remain focused on their area of expertise? In another words, which would you say is more valuable? mile wide / inch deep or inch wide / mile deep?

John: I would say that it depends (can you tell I was a DBA earlier in my career?). I think that there is a strong need to expand a skillset to have an appreciation and solid foundation in networking, storage, and identity management that will roll into working with cloud platforms whether it is Azure, Amazon, or Google. I would also recommend building on this foundation in areas that you find interesting to achieve a higher understanding of those areas. Even the database engine is so large now that it is difficult to know all of that. I would point out security, database design principals, and performance tuning as areas that are likely to be very important over the coming years.

          Azure AD Application Proxy Overview, Deployment Process and Limitations      Cache   Translate Page      
One of my customers came to me enquiring about publishing some on-premises applications through the cloud using Azure AD Application Proxy.  This feature of Azure is relatively new and there is currently limited documentation about this available online.  I put some time aside to learn the technology and get a deep understanding on how the technology works.

In this article I will be running through a high level overview of how Azure AD Application Proxy works and then go through a step by step for a basic deployment of Azure AD Application Proxy to publish Outlook Web App.

Azure AD Application Proxy Overview

Azure AD Application Proxy is essentially a reverse proxy solution which is hosted completely in the Microsoft Azure cloud that allows companies to provide secure remote access to on-premises web based applications.  Traditionally companies needed to setup client side VPN connections or use demilitarized zones to provide secure remote access to on-premises web applications - especially for some web applications which may not be built secure for direct publishing to the Internet.  There are numerous security benefits for using Azure AD Application Proxy such as leveraging rich authorization controls and security analytics in Azure, two factor authentication, DDOS protection, no inbound connections to your internal network and much more.  I'm not going to go into all the security benefits of Azure AD Application Proxy in this article but you can do some further reading on the Microsoft website about this here:


It is important to mention, Azure AD Application Proxy is regional based.  Microsoft will automatically select the Azure Datacentre to host this service for you based on the country you specify when signing up to Microsoft Azure.  When you enable Application Proxy, the Application Proxy service instances for your tenant are chosen or created in the same region as your Azure AD tenant, or the closest region to it.

I have put together a high level overview of how Azure AD Application Proxy works below which I will be going through below (click to enlarge).


Without going too deep, there are essentially four components that make Azure AD Application Proxy work:
  • Azure AD Application Proxy Apps
  • Connectors
  • Connector Groups
  • Backend Applications
I will cover each of these below.


Azure AD Application Proxy Apps

Azure AD Application Proxy Apps sit in Microsoft Azure along side all your Software as a Service (SaaS) that you have published through Azure AD.  The primary difference between Application Proxy applications and standard Web Based Cloud applications, is Proxy Apps will redirect you to the server on-premises.

In Microsoft Azure, you can see the Application Proxy Applications with all your other Enterprise Applications.  Simply login to https://portal.azure.com and select Azure Active Directory --> Enterprise Applications.


When we open the Enterprise Applications, we can see an Azure AD Application Proxy application I created which sits along with all my other SaaS applications that I have associated with this Azure AD Tenancy.


Users can access the application directly by going directly to the Application Home Page URL or by logging into their application portal where all their SaaS applications are published.  This portal is under:

http://myapps.microsoft.com

As you see my test user user1@avantlab.com.au has only been presented with a few applications:


Connectors

Connectors facilitate traffic flow from the Azure AD cloud to your on-premises applications.  They are services which are installed on servers on your internal network running Server 2012 R2 or Server 2016.

Connectors are stateless and hold no configuration data on the machine apart from settings needed to connect to the Azure AD cloud service and its authentication certificate required for authenticating itself against the cloud.  They automatically update their configuration information every few minutes from the cloud.

When installed on an on-premises server, the connector will simply show as services in the Windows MMC console.


All connectors do not require any inbound firewall or Network Address Translation (NAT) rules publishing the connectors to the Internet.  They only initiate outbound connections to Azure creating a stateless connection to the cloud which is used to receive inbound application requests.  You can move the member servers running these connectors to different datacentres or public IP addresses, and because they are creating outbound connections it will not disrupt the on-premises web applications they publish.

As the Connectors do not require any inbound ports open from the Internet, they do not need to be located within a Demilitarized Zone and can go directly on servers on your internal network.  It is recommended deploying the connectors as close to your servers as possible to minimise latency.

Connectors can service multiple Azure AD Proxy Applications on your internal network and can be made fully redundant.

For production environments, it is always recommended to have multiple (at least two) Application Proxy Connectors on an on-premises environment to provide high availability to published on-premises applications.

Microsoft Azure automatically maintains the on-premises connectors not only the configuration, but updates.  Provided the "Microsoft AAD Application Proxy Connector Updater" service is running, your connectors will be automatically updated and patched.  If you have multiple connectors on your environment associated with a connector group, updates will only target one connector at a time in the group to prevent downtime to your environment.  We will talk about connector groups next.

Lastly it is important to mention, Connectors can run on member servers that are not domain joined however if you want Single Sign-On (SSO) working for applications that use Integrated Windows Authentication, they need to be setup on domain joined servers.  I will be showing you how to deploy Windows Integrated Authentication for an Azure AAD Application Proxy connector later in this post.

Connector Groups

All Connectors are associated with a Connector Group in Azure.  If you do not first define a Connector Group, all connectors you create will get assigned to a "Default" Connector Group.

A few key things to take away from Connector Groups:
  • Every AAD Proxy App you create must be assigned to one Connector Group (one to one relationship)
  • One Connector Group can contain Many Connectors (one to many relationship)
  • One Connector Group can service many applications (one to many relationship).
Connector Groups ensure on-premises application requests are load balanced across all on-premises Connectors in the group.  No configuration is done to setup load balancing between the connectors, this is done all automatically after the connectors are installed.

For most companies, only one Connector Group will be required with a few connectors as this will look after all your internal applications.  But why would you want to create multiple Connector Groups?  There is a few reasons.
  • Multiple Datacentres
  • Network Isolation
  • IaaS Deployments
  • Multi-Forest Deployments
  • Multiple Companies in a Single Azure AD Tenant

Multiple Datacentres

If you have multiple datacentres, you will most likely want to have a separate connector group with connectors in each datacentre.  This is to ensure latency is minimised between the connector and the application to provide the best experience for the users.

Network Isolation

If you have any network isolation between applications on your internal network such as Access Control Lists or Network Segmentation, you will want to ensure separate connector groups are created for each segment.  Remember the connectors associated with the connector group need internal connectivity to the application published.

Infrastructure as a Service Deployments

If you have any member servers in an IaaS cloud running internal web applications, you will most likely want to put connectors in the IaaS tenancy to minimise latency and a separate connector group.

Multi-Forest Deployments

If you have multiple on-premise Active Directory forests, you want a connector group for each Active Directory forest so that Kerberos Constrained Delegation works correctly for each forest.

Multiple Companies for a single Azure AD Tenancy

Some IT Departments for parent companies may run the on-premises networks for many child companies which are associated against a single Azure Tenancy.  You will want a connector group for each child company as they will most likely each have their own Active Directory forest.

Backend Applications

Backend applications are the on-premises products you wish to publish to the cloud via Azure AD Application Proxy.  I displayed in my diagram Exchange or SharePoint, but you can also publish other applications from Microsoft or third party vendors - as long as the entire application is web based.

Azure AD Application proxy Access Workflow

Before I get stuck into going through the steps for configuring Azure AD Application Proxy, I want to quickly touch base on the 6 steps which occur when a user accesses an application published in Azure AD Application Proxy.  This is taken from TechNet but is important to understand.
  1. The user accesses the application through the Application Proxy service and is directed to the Azure AD sign-in page to authenticate.
  2. After a successful sign-in, a token is generated and sent to the client device.
  3. The client sends the token to the Application Proxy service, which retrieves the user principal name (UPN) and security principal name (SPN) from the token, then directs the request to the Application Proxy connector.
  4. If you have configured single sign-on, the connector performs any additional authentication required on behalf of the user.
  5. The connector sends the request to the on-premises application.
  6. The response is sent through Application Proxy service and connector to the user.


Azure AD Application Proxy Deployment How-To

I have gone through and documented the steps for publishing Exchange 2016 on-premises Outlook Web App via Azure AD Application Proxy so you get an understand what is involved in setting up this technology.  In this deployment I have a single connector which is installed on my Exchange 2016 server in a default connector group.

For a production deployment, you will want to ensure the connectors are installed on dedicated servers and that you have more then one connector server for redundancy.

In this lab deployment also is using Password Hash Synchronisation between the on-premises environment and Azure AD.  If your company does not want to store your user password hashes in Azure AD, you can also use Passthrough Authentication with the Azure AD Connect tool.

Before we start configuring the Azure Application Proxy, you need to have Azure AD Connect setup and synchronising.  You will most likely already have this in place, but if you have never done this before, I have documented the steps.  Its very straight forward!

Setting up Azure AD Connect for Password Hash Synchronisation

First of all download the Azure AD Connect tool from:

Once the tool is downloaded, run it and accept Microsoft's End User Licensing Agreement (EULA).



When you get to this screen you will be asked if you want to use Password Hash Synchronisation or Passthrough Authentication.  If your company does not want user passwords in Azure AD, you need to use Passthrough authentication.  For this lab I'm using Password Hash Synchronisation.

I have also enabled Azure Active Directory Seamless Single Sign-On (Azure AD Seamless SSO) which provides single sign-on to Azure AD services by sending the Kerberos requests directly through to Azure AD using a Computer Account AZUREADSSOACC.  If you want to understand how Azure AD Seamless SSO works, please refer to these articles here:



Enter an account with Azure AD Global Administrator rights into the wizard to connect to Azure AD.


Select the Active Directory forests you want to synchronise.  The tool can synchronise multiple forests to Azure AD.


Select the on-premises attribute you want Azure AD to use as the username.  I ususally make this the userPrincipalName of the on-premises accounts, and I make sure the UPN is the users email address.  This can be done by setting up a UPN Suffix in Active Directory Domains and Trusts and assigning the UPN on each user account to match the UPN Suffix.

Users in my test lab have the following UPN suffixes:

user1@avantlab.com.au
user2@avantlab.com.au
user3@avantlab.com.au etc.


You can synchronise sub organisational units or the entire domain.  In this lab I'm synchronising the entire domain structure.


Azure was configured to manage the source anchor (the unique identifier) between users on premises and in Azure AD.  If you have a complex network with a lot of users you will need to think about what attribute you want to use as the source anchor.


We are not filtering any users in this lab, we are synchronising everything.


No additional features were need in this lab environment for Azure AD Application Proxy.  You may need additional features for other requirements.


You need to make sure your domain is verified in the Azure Portal by adding a TXT record to your public DNS.


The wizard will show if its verified.


Enter Domain Admin credentials for an account on-premises which is required for Azure AD Seamless SSO.  You will want to use a service account for this with a super complex password.


Kick off an initial synchronisation once the wizard is finished.


You will see the wizard created the AZUREADSSOACC for Azure AD Seamless SSO.


Close the wizard.


Upgrading Azure AD

By default your Azure AD subscription will be "Azure AD Free" which can run Microsoft Office 365.  In order to use Azure AD Application Proxy you must be running at least "Azure AD Basic", "Azure AD Premium P1" or "Azure AD Premium P2".

For a comparison between the various flavours of Azure AD, please see the following Microsoft website:


If you upgrade from Azure AD Free to a higher flavour, you will need to wait 24 hours for the changes to take effect within Azure.

Setting up Azure AD Application Proxy

The following steps need to be taken to setup Azure AD Application Proxy.

Navigate to Azure AD --> Enterprise Applications.


Select New Application


Select On-premises application.


If you haven't already upgraded from Azure AD Free you will get this warning.


Click the link and select Free Trial next to Azure AD Premium for lab purposes.  If your running a business, you will need to procure Azure AD Basic or higher before proceeding.


Activate the Azure AD Premium P2 trial in the lab and wait 24 hours for the activation to complete.


Next download the Application Proxy Connector application to install on the member servers.


You will need to agree to a license agreement before download can commence.


Make sure you run the setup on a Server 2012 R2 or Server 2016 computer.  In my lab to minimise the amount of virtual machines I have dedicated to this exercise, I installed the connector directly on an Exchange 2016 server.  For production deployments you should deploy the connector on a dedicated server and have more then one for redundancy.

Click through the steps below.


Sign-in to your Azure AD Tenancy with an Azure AD Global Tenancy account when prompted.


Let the tool install and connect to Azure AD.


Click close when it was successful.


Notice on my Exchange 2016 server, it added the following two services:
  • Microsoft AAD Application Proxy Connector
  • Microsoft AAD Application Proxy Connector Updater

After the connector is installed, go back to Azure AD and add the on-premises application.
  • Give the application a name, this is what users will see in the myapps.microsoft.com portal.
  • Enter the Internal URL of the application.  This is the FQDN of the web based application on your internal network.
  • Select a DNS name for the service, you can use a Microsoft domain or lab purposes or your own domain which you will need to take care of the DNS changes yourself.  As this is a lab I just used the Microsoft msappproxy.net domain.
  • Under Pre-Authentication you can select from Azure Active Directory or Passthrough Authentication.  Azure Active Directory requires the password hashes to exist in Azure AD.
  • As we did not create a connector group it will just use the Default group.  For production deployments I recommend you create a Connector Group first and give it a descriptive name such as the datacentre/location where the connectors will exist.


After you create the application, you will need to go into the properties of it and assign a couple of users the ability to access the application through Azure AD.  This is done through Users and Groups.


Lastly we need to setup Single Sign-on.  This ensures once a user signs into Azure AD through https://myapps.microsoft.com or going directly to the external facing URL of the application, they automatically get logged into the application and don't require signing in twice.  There are many Single Sign-on methods for Azure AD Application proxy including:
  • Password-based Sign-on
  • Linked Sign-on
  • Integrated Windows Authentication
  • Header-based Sign-on

For Exchange Server, we used Integrated Windows Authentication for Outlook Web App as this is best for Exchange.  By default, Exchange is setup to use Forms based authentication for the web apps so we need to change this before configuring Azure AD.

As you see below I changed my Exchange Virtual Directories and restarted IIS so that we are using Windows Integrated Authentication for both ECP and OWA.  Forms based authentication is now disabled.


Next we need to configure Kerberos Constrained Delegation on the servers running the connectors.  Find the member server computer objects in Active Directory Users and Computers for your Connector machines and go to the Delegation Tab.  You will need to enable Advanced Features in Active Directory Users and Computers to see this.

Under Delegation, enter the computer objects for all your servers which you want to setup delegation for.  These are the servers running applications.  You can add multiple web servers here... Exchange, SharePoint and any other web servers you have internally that you want to publish.

The screenshot is off putting because the Connector and the Application are both on the same server, hence why it was delegated to itself.

Once Active Directory is setup, configure the Internal Application SPN of your internal server.  This is generally going to be:

http/fqdn-of-internal-app

Select the login identity we configured earlier in Azure AD Connect, which was User Principal Name.


Next navigate to the public facing URL of your published application.  In my lab its:

FYI: If your reading this a month later, I properly repurposed my lab and the address no longer exists.


After signing in to the application, we see that we are automatically passed through to Exchange OWA on-premises without being prompted for credentials again.

新加坡政府目前指定国内最大电子支付商 NETS 为国内餐饮行业统一电子支付商,目前覆盖的支付范围包括咖啡店、大排档和公司食堂。推行的商户每月则将获得0.5%的商户贴现率 (MDR)。

NETS 未来将支持20种不同的无现金支付选项。其中,首批支持的10种支付方式分别是美国运通 American Express、CEPAS 优惠卡、EZ-Link、Liquid Pay、Mastercard、NETS、NETS FlashPay、UnionPay、Visa和微信支付。

第二批支持的10种支付方式则将于2019年8月底上线,其中包括 Active Wallet、支付宝、Bev Eat、Ezi Wallet、GrabPay、iAPPS、Honestbee、Qixiang Technology、Singtel Dash和Smart Gateway。

*Mobile TeleSystems (MTS)* has claimed a Russian first with the launch of services over its NB-IoT network in 20 cities across the Federation including Moscow, St. Petersburg, Nizhny Novgorod, Novosibirsk, Kazan and Vladivostok. It plans to offer NB-IoT coverage in all large cities nationwide by the end of 2018. The NB-IoT infrastructure and service rollout is supported by technology solutions from *Huawei*, *Nokia*, *Ericsson*, *Samsung* and *Cisco*, with MTS targeting sectors such as logistics/transport, energy/utilities, mining, manufacturing, retail, health, smart cities and smart housing. The mobile operator highlighted that the NB-IoT network will significantly reduce costs of implementing IoT/M2M solutions due to advantages over existing M2M standards, including increased network capacity, high radio-sensitivity, long service/battery life and low IoT module cost. In another pan-Russia announcement, fixed network operator *ER-Telecom* disclosed that its LoRaWAN-based industrial IoT network has expanded services to 52 cities, stretching from the Pacific port of Vladivostok to the European exclave of Kaliningrad. ER-Telecom – operating under the brands Dom.ru and Dom.ru Business – has partnered platform provider *Actility* for the rollout, targeting 60 cities by the end of this year. IoT solutions on the LoRa network are aimed at the energy sector as well as smart city applications, housing/communal services, other urban services and transport companies among others, offering tailored solutions for ER’s business clients. In the US, *AT&T* is narrowing down the launch window for its upcoming NB-IoT network, which will augment its existing low power, wide area (LPWA) IoT network based on LTE-M technology. AT&T now aims to launch NB-IoT connectivity services in the second quarter of 2019, Mobile World Live reports. Chris Penrose, president of IoT Solutions at AT&T, noted that cellular LPWA IoT services are finally starting to live up to the hype surrounding their early rollouts, saying that ‘use cases are taking off,’ and adding: ‘It’s about how we now truly scale this industry.’ *Vodafone Group* is doubling the number of LTE cell sites supporting the NB-IoT standard, with its existing NB-IoT networks in Germany, Italy, the Netherlands, the Czech Republic, Greece, Ireland, Spain, Turkey, South Africa and Australia to be densified while new networks will be commercially launched in the UK, Romania and Hungary, TechRadar reports. Vodafone – which claims around 74 million M2M/IoT connections worldwide – says there is strong enterprise demand for NB-IoT-powered applications, with its IoT director Stefano Gastaut stating: ‘NB-IoT gives businesses access to 5G capabilities a year before we expect large-scale consumer availability and I believe this will be a catalyst in the widespread use of IoT by enterprises.’ Indian IoT specialist *Unlimit*, part of the *Reliance Group*, has teamed up with state-owned telco *Bharat Sanchar Nigam Limited (BSNL)* to offer IoT services to enterprise customers across India in sectors including automobile, digital manufacturing, logistics, transportation, public sector enterprises and agriculture. Unlimit will leverage BSNL’s pan-India cellular network to provide solutions including IoT device management, managed connectivity, application software and advanced analytics. Jurgen Hase, CEO at Unlimit, said: ‘With the addition of BSNL’s connectivity, we are further expanding our services and capabilities to help scale essential IoT projects in India and contribute significantly in the digitisation of the rural society.’ Anupam Shrivastava, BSNL chairman, added: ‘By combining our pan-India coverage, last mile network access, and bandwidth with Unlimit’s range of services, we will help enterprises accelerate the pace of their new innovations and fast-track the digital transformation process.’ *BlackBerry* has unveiled ‘BlackBerry Spark’, described as the only Enterprise of Things (EoT) platform designed for ultra-secure hyperconnectivity from the kernel to the edge. BlackBerry Spark is aimed at manufacturers of complex ‘things’ such as autonomous vehicles and industrial equipment with the highest levels of security and safety-certification, as well as consumer-friendly interfaces to complex processes and AI, such as voice-activated speakers with built-in privacy protection. The platform is also designed for enterprises to leverage AI and manage smart 'things' regardless of operating system (Android, iOS, Linux, QNX, Windows) as well as snap-in existing platform services such as Android Things, AWS, Azure and Watson. In another launch with eyes firmly on security, US operator *Sprint*, in partnership with its Japanese parent *SoftBank Group*’s member companies *Packet* and *Arm*, has unveiled the ‘Curiosity IoT’ platform, enabling enterprises to manage IoT devices and connectivity over the air across multiple SIM profiles. Sprint says that via Curiosity IoT ‘intelligence from device data will be generated instantly through the dedicated, distributed and virtualized core, built together with the new operating system. And the ultimate level of security will be provided from the chip to the cloud.’ Lastly, *Cubic Telecom* has launched its Global Connectivity Management Platform offered ‘as-a-Service’ to IoT device manufacturers worldwide, with the first devices hosted on the platform already being managed in Europe, the USA and South Korea. Cubic says its unique platform solution brings standards-based remote SIM provisioning and cloud-native advanced connectivity management functions such as zero-touch device registration and connectivity activation across different mobile networks, regions and regulatory conditions.           Caudal Septal Stabilization Suturing Technique to Treat Crooked Noses      Cache   Translate Page      
imageAim: To rotate the nasal axis and septum to the midline using an L-strut graft and a novel caudal septal stabilization suturing technique to treat crooked noses. Patients and Methods: Thirty-six patients were included in the study. First, an L-strut graft was prepared by excising the deviated cartilage site in all patients. Second, multiple stabilization suturing, which we describe as a caudal septal stabilization suturing technique with a “fishing net”-like appearance, was applied between the anterior nasal spine and caudal septum in all patients. This new surgical technique, used to rotate the caudal septum, was applied to 22 I-type and 14 C-type crooked noses. Correction rates for the crooked noses were compared between the 2 inclination types with angular estimations. Results: Deviation angles were measured using the AutoCAD 2012 software package and frontal (anterior) views, with the Frankfurt horizontal line parallel to the ground. Nasal axis angles showing angle improvement graded 4 categories as excellent, good, acceptable, and unsuccessful for evaluations at 6 months after surgery in the study. The success rate in the C-type nasal inclination was 86.7% (±21.9) and 88% (±16.7) in the I-type. The overall success rate of L-strut grafting and caudal septal stabilization suturing in crooked nose surgeries was 87.5% (±18.6). “Unsuccessful” results were not reported in any of the patients. Conclusions: L-strut grafting and caudal septal stabilization suturing techniques are efficacious in crooked noses according to objective measurement analysis results. However, a longer follow-up duration in a larger patient population is needed.

I have created one AIF services with HTTP adapter in AX 2102 R3

This service is working fine.

We are developing Apple iOS app. so mobile developer requires this service in single WSDL file. When I click the singel WSDL im receiving the following error.

The server was unable to process the request due to an internal error. For more information about the error, either turn on IncludeExceptionDetailInFaults (either from ServiceBehaviorAttribute or from the <serviceDebug> configuration behavior) on the server in order to send the exception information back to the client, or turn on tracing as per the Microsoft .NET Framework SDK documentation and inspect the server trace logs.

Can anyone help me how to get single WSDL file from AX web service for iOs app development, 

Is there any other alternative way to provide the service to mobile app developer without using Azure.


          aspnet core 2.1 logging - I can see it in streaming log, but not in blob storage      Cache   Translate Page      

I'm trying to configure logging for an aspnet core 2.1 app.

Packages configured (Microsoft.ASpNetCore.App and Microsoft.lExtensions.Logging.AzureAppServices)

I can see my output logs when I connect to the log stream, but I do not see the {app-name}{timestamp}/yyyy/mm/dd/hh/{guid}-applicationLog.txt in blob storage.

In blob storage, I see 27b318-8196.applicationLog.csv where the output of my WebJobs in the same service run.

In Program.cs I added the following line in CreateWebhostBuilder to no avail
.ConfigureLogging((hostingContext, logging) => logging.AddAzureWebAppDiagnostics())

I'm not sure what I'm doing wrong here (as logging is working - I get a file and can connect via the stream, just no logging to blob storage)

imageThis study investigates the clinical utility of squamous cell carcinoma antigen (SCC-Ag), circulating immune complexes (CIC), and immunoglobulins (IgA, IgG, IgM) in the diagnosis, monitoring, and prognosis of 117 squamous cell carcinoma of the head and neck (SCC-HN) patients having local and/or systemic treatment. Serum marker levels were measured in a prospective study. SCC-Ag was positive in 28.2% of patients, the CIC in 63.2%, the IgA in 11.1%, the IgG in 15.4%, and the IgM in 9.44%. Statistically significant correlation was found between the initial SCC-Ag levels and tumor localization, whereas the CIC levels were increasing significantly with progressing disease stages. It was also found that the significant decrease of SCC-Ag, IgA, and CIC levels at the end of treatment was correlated with an increased incidence of disease-free status. The initial values of IgG and the disease stage were significantly correlated with a favorable treatment outcome. The pretreatment elevated SCC-Ag and IgM serum values showed a significant trend to predict a disease progression. Using a Cox proportional hazards model the IgG serum values, the primary site, and the disease stage were significant predictors for time to progression. The significant decrease of SCC-Ag, IgA, and CIC values at the completion of treatment was correlated with an increased incidence of disease-free status. This study indicates that only the estimation of SCC-Ag and in some degree the IgM and/or IgG is a potential tool for monitoring the efficacy of treatment or disease recurrence in SCC-HN.

The successful candidate will work within the newly formed Security Architecture team responsible for: Developing and maintaining the security reference architecture for the security technology domains (eg Authentication, Authorization, Monitoring, Assurance etc.) Creating Target State Architecture in collaboration with the SMEs in engineering as well as other partners across the firm to deliver the core security capabilities and their application across major infrastructure and application environments including the cloud Architecting/designing/documenting strategic cross-discipline security solutions for an array of business problems Evaluating/assessing strategic solutions to solve business problems and/or to feedback into the Target State Architecture Skills Required: Proven architectural and design background Proven background in Security and/or delivering Security solutions Excellent communication skills coupled with a collaborative approach Excellent understanding of Enterprise Architecture Experience delivering and/or running scalable infrastructure Skills Desired: Experience using an Architectural Frameworks (TOGAF, EA, SABSA, etc.) Experience working in a large, regulated corporate entity Experience delivering Microsoft office365 & Azure services Cryptography knowledge - ie Azure Key Vaults Experience integrating with Cloud (SaaS) applications and/or services Experience with network and mobile security Broad experience across one or more of core security technology areas: Identity and Access Management, Data Security, Security Monitoring, Cryptography, Assurance, Incident Response.

The primary outcome was mean daily frequency of VMS. Secondary outcomes were VMS interference with daily life, sleep quality, depressive symptoms, somatic and other symptoms, anxiety, and quality of life.Results:The VMS frequency declined by 36.7% at 6 months in the acupuncture group and increased by 6.0% in the control group (P < 0.001 for between-group comparison). At 12 months, the reduction from baseline in the acupuncture group was 29.4% (P < 0.001 for within-group comparison from baseline to 12 months), suggesting that the reduction was largely maintained after treatment. Statistically significant clinical improvement was observed after three acupuncture treatments, and maximal clinical effects occurred after a median of eight treatments. Persistent improvements were seen in many quality of life-related outcomes in the acupuncture group relative to the control group.Conclusions:We found that a course of acupuncture treatments was associated with significant reduction in VMS, and several quality-of-life measures, compared with no acupuncture, and that clinical benefit persisted for at least 6 months beyond the end of treatment.           Lines of Thinking - September 2018: Eugenio Montale       Cache   Translate Page      

If you are traveling along the coast of Liguria in Italy you will likely be drawn to the seaside villages of Cinque Terra. It has always been one of my favorite places, full of opportunity for heart-exerting hikes, stirring vistas, reading, drawing, writing, talking with friends next to the sea, and quiet contemplation almost anywhere along the way.

These days, the five communities are connected by train and a well-worn hiking path, but for many years, the interior villages were fishing/farming ports reachable only by boat or winding mountain roads. A temperate (Mediterranean) climate makes it possible to grow citrus and olives in the steep terraced hillsides facing the unbroken expanse of the Liguria Sea. This was the adopted patria of the Italian poet, Eugenio Montale, who was born in Genoa further to the north. His family spent holidays in the area and much of Montale’s early poetry was influenced by his study and experience of the Cinque Terra’s beautiful but isolated landscapes, a rugged geology into which human habitation had doggedly inserted itself over millennia.

Montale was awarded the Nobel Prize for Literature in 1975 and was a frequent subject of translation by English speaking writers, other poets especially, despite the fact that his poems were, according to Harry Thomas, editor of Montale in English, “notoriously difficult” to translate. 

Here are two Montale poems, the second one rendered by two different translators.

Cinque Terra is a popular summer tourist and vacation spot and you will see why if you visit. The off-season past early November and into the harder weather months through early spring are also interesting times to visit and walk the far less-crowded trails. Sometime ago, I remember you could arrange for local poet guides to take you to places Montale captured in his work and there read for you. Or, you could just go with a volume of Montale and your imagination.

Hear me a moment. Laureate poets
seem to wander among plants
no one knows: boxwood, acanthus,
where nothing is alive to touch.
I prefer small streets that falter
into grassy ditches where a boy,
searching in the sinking puddles,
might capture a struggling eel.
The little path that winds down
along the slope plunges through cane-tufts
and opens suddenly into the orchard
among the moss-green trunks
of the lemon trees.

Perhaps it is better
if the jubilee of small birds
dies down, swallowed in the sky,
yet more real to one who listens,
the murmur of tender leaves
in a breathless, unmoving air.
The senses are graced with an odor
filled with the earth.
It is like rain in a troubled breast,
sweet as an air that arrives
too suddenly and vanishes.
A miracle is hushed; all passions
are swept aside. Even the poor
know that richness,
the fragrance of the lemon trees.

You realize that in silences
things yield and almost betray
their ultimate secrets.
At times, one half expects
to discover an error in Nature,
the still point of reality,
the missing link that will not hold,
the thread we cannot untangle
in order to get at the truth.

You look around. Your mind seeks,
makes harmonies, falls apart
in the perfume, expands
when the day wearies away.
There are silences in which one watches
in every fading human shadow
something divine let go.

The illusion wanes, and in time we return
to our noisy cities where the blue
appears only in fragments
high up among the towering shapes.
Then rain leaching the earth.
Tedious, winter burdens the roofs,
and light is a miser, the soul bitter.
Yet, one day through an open gate, 
among the green luxuriance of a yard,
the yellow lemons fire
and the heart melts,
and golden songs pour
into the breast
from the raised cornets of the sun.

Used Code First Migration to update properties and then publish to azure.

...in every tab / property set of web app.  That is, no tabs / properties work and give error above.  I have absolutely no access to any of my live data on azure.

The project builds fine when running on local host - no errors - just not in azure where my live data is.

I tried to run down method of project.  Same error in azure.

I need to research this more tomorrow when I get back into office but was hoping someone might be able to give me some insight and point me in the right direction.
