The Health Resources & Services Administration (HRSA) reported that these purchases reached $53.7 billion, meaning 340B covered entities spent $9.8 billion more on 340B drugs than they did in 2021. This release provided an opportunity to take a closer look at why that figure increased. Putting 340B Purchases in Perspective In addition to understanding the drivers behind the 340B purchase figures, it is important to understand what the HRSA totals mean. The purchase totals pertain to the tens of billions of dollars that drug companies take into their coffers for selling 340B-eligible drugs. They do not quantify how much hospitals received in savings for those discounted drugs, as implied by some drug industry critics of 340B. It is not at all a given that an increase in the amount of 340B purchases translates into a corresponding increase in 340B savings. The 340B value depends on both the price of each drug and the 340B discount for that drug. These are factors that hinge on whether a drug is brand-name or generic, how the drugmaker has hiked prices over time, and what drug discounts the company has offered other payers. With the understanding of how 340B purchases differ from discounts, one can turn to the key reasons why drugmakers are selling more of these drugs each year. A Continued Shift Toward Outpatient Care A major element behind total 340B drug purchases has nothing to do with how hospitals use 340B. Instead, it reflects a continued shift away from providing hospital care in the inpatient setting. In recent years, hospital outpatient revenue exceeded inpatient revenue for the first time. This was the latest development in a trend that accelerated during the COVID-19 pandemic. Other research notes that the shift toward outpatient care – and the use of 340B-eliglble drugs that go along with it – are due to several key reasons. They include advances in outpatient care and technology, changes in reimbursement for care, and patient preferences. High Prices for New Drugs Other big drivers of 340B purchases are the often-astronomical prices that drugmakers set for new products that they are bringing to market. High prices are an issue because the 340B discount for new brand-name drugs is 23.1%, meaning hospitals are paying more than three-quarters of those starting prices. Drugs that have been on the market for years and have undergone large price hikes have deeper 340B discounts. A recent study found that net launch prices for new drugs skyrocketed from a median of $1,376 in 2008 to $159,042 in 2021. Recent launch prices for individual drugs have been so high in several cases that they have generated nationwide media coverage. Many of these new products are drugs that are infused or injected in the hospital outpatient setting, which make them 340B-eligible for hospitals that qualify. That contributes to total 340B purchases beyond any additional uptake of drugs that have been on the market for years. The Rise of Specialty Drugs Increasing numbers of patients are being prescribed specialty drugs to treat their diseases and conditions. These drugs often come with special handling requirements and more complex instructions. They also often come with very high price tags. Those high price tags have helped drive 340B drug spending because the mix of drugs that providers prescribe is rapidly shifting toward specialty drugs. These include some of the costliest products to treat cancer, multiple sclerosis, autoimmune disorders, and other complex diseases. When 340B first came into being in 1992, virtually no specialty drugs existed. Data show that by 2010 they made up 27% of pharmaceutical spending, and by 2022 that figure was a whopping 51%. In the decade from 2012 to 2022, spending on specialty drugs grew from $81 billion to $219 billion. Massive uptake in the use of just a few blockbuster drugs can move the needle rapidly on the amount spent on 340B drugs. 340B Health calculations of pricing data demonstrate that just five specialty drugs introduced within the past decade (Keytruda, Darzalex, Dupixent, Biktarvey, and Trikafta) alone accounted for a $2.3 billion increase in 340B purchases from 2019 to 2020. ]]> Ideas for 340B Changes Could Lead to Legislation /2023/12/ideas-for-340b-changes-could-lead-to-legislation/ Wed, 06 Dec 2023 17:28:45 +0000 /?p=9751 Print Article Amid complaints about unilateral drug company restrictions on the 340B drug pricing program and mounting pressure from industry-backed advocates for even more 340B changes to favor drugmakers, a bipartisan group of six U.S. senators have taken a proactive step by asking for ideas to ensure “stability” for 340B. This move came in the form of a request for information (RFI) that prompted a raft of responses. Some of the feedback could form the basis for legislation to amend 340B and possibly set the stage for a comprehensive discussion on its future. Those ideas include several proposals by 340B hospitals and other covered entities to strengthen 340B. Also in the mix, however, are proposals by the pharmaceutical industry and its allies that would reduce 340B benefits for providers. The RFI is from six longtime supporters of 340B, Sens. John Thune (R-S.D.), Debbie Stabenow (D-Mich.), Shelley Moore Capito (R-W.Va.), Tammy Baldwin (D-Wis.), Jerry Moran (R-Kan.), and Ben Cardin (D-Md.). Several of the senators have been critical of the actions of a group of drug companies that have been limiting hospitals’ ability to partner with community and specialty pharmacies to deliver discounted drugs to their patients. The senators’ stated goal is to focus on both “stability and oversight” of 340B. Hospitals and Pharmacists United In response, 340B Health offered a series of proposals to strengthen 340B and protect it against the harmful actions of drug companies. These include: Restoring the 340B benefit for drugs dispensed through community and specialty pharmacies and prohibiting drug companies from imposing conditions that restrict 340B. Allowing covered entities the ability to directly challenge unlawful drug company 340B restrictions or conditions in federal courts. Prohibiting discriminatory policies by payers and pharmacy benefit managers (PBMs) for drugs that 340B hospitals administer or dispense to their patients. Prohibiting states from mandating that providers use or do not use 340B drugs for Medicaid patients. Eliminating statutory provisions that increase drug costs for hospitals, such as the orphan drug loophole and group purchasing organization (GPO) prohibition. 340B Health urged the lawmakers to reject proposals to impose unnecessary and burdensome reporting requirements on 340B hospitals. It asked the senators to encourage the Health Resources & Services Administration (HRSA), which administers 340B, to exercise its existing authority to conduct greater 340B oversight over drug companies and wholesalers, including through more frequent audits and by fully operationalizing the 340B administrative dispute resolution (ADR) process. Finally, 340B Health urged Congress to include protections against HRSA taking any administrative actions to narrow the scope of 340B. Similar comments went to the six senators from such 340B advocates as the American Hospital Association (AHA), the American Society of Health-System Pharmacists (ASHP), America’s Essential Hospitals (AEH), the Association of American Medical Colleges (AAMC), the Catholic Health Association of the United States, and the Children’s Hospital Association. Drug Industry Attacks Given its multi-year, multi-million-dollar campaign to weaken 340B, it was not surprising that the drug industry and its allies weighed in with proposals to shrink the scope of 340B and exclude many hospitals and their patients from its benefits. The Pharmaceutical Research & Manufacturers of America (PhRMA) said in its comments that it believes 340B should serve a much narrower purpose than Congress intended. The group says Congress should limit 340B to helping “low-income, uninsured, and other vulnerable patients obtain the outpatient medicines they need,” the letter states. This proposal is contrary to the expressed intentions of Congress when it debated and approved the 340B statute in 1992. In a committee report during that process, Congress said the purpose of 340B was to “stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services.” As a federal judge explained in a recent 340B court decision: “340B entities are able to stretch these scarce Federal resources because they receive their drugs at a discount and are reimbursed by insurers at the non-discounted price of the drug… . This allows 340B entities to provide more services to a larger population of under-served patients.” To reduce the scope of 340B to fit its narrow vision, PhRMA would have Congress make the 340B definitions of a qualified hospital and a covered patient significantly more restrictive than they are today. Such changes would dramatically reduce the benefit of 340B to providers, directly impacting patient care. The law requires 340B providers to serve all patients, including those with low incomes and those living in underserved rural parts of the country. PhRMA-backed requirements for hospitals and other covered entities to pass the 340B discounts directly to patients would eliminate their ability to invest the savings into providing more comprehensive services. Other drug industry proposals include limits on hospital-pharmacy partnerships and hospital child sites as well as requirements for hospitals to report information to the government on matters unrelated to 340B, such as their provision of charity care. Drug industry partners submitted similar proposals, including groups that PhRMA funds. What Comes Next? The senators who requested these ideas have not set a timeline for next steps. RFIs can lead to hearings, congressional letters to the administration, or proposed legislation. While it is not certain that these ideas and others being considered on Capitol Hill will find their way into legislation, lawmakers might introduce such measures in the coming months. One-quarter of patients at the health system’s three hospitals in the Wichita area are on Medicaid or are uninsured, like Chandler was when he came in. But his diagnosis was much more uncommon – aplastic anemia, a rare blood disorder that can require hundreds of thousands of dollars’ worth of treatments that are out of reach for someone without health coverage. So his caregivers marshaled 340B resources to help. Ashley Martin is a clinical oncology pharmacy specialist for Ascension Via Christi Hospitals. She describes how the system used its access to 340B pricing to provide steep discounts to Gary on the infused drugs that he needed from his hematologist. Thanks to 340B, he received these lifesaving treatments for a year-and-a-half from his initial diagnosis and again after he suffered a relapse of his disease. “Without the 340B program, we would not have been able to provide the care that Mr. Chandler needed,” said Martin, who shared this patient’s story as one of our newest Faces of 340B profiles. “He is currently doing very well.” 340B Help for All Those Who Need It Chandler’s story is one of the many patient health successes that are due to the dedicated health providers at Ascension Via Christi and their ability to serve patients in need using 340B resources. These patients include not just those living in or near Wichita but those living in more rural areas that the system also serves. Patients who cannot afford their medications benefit from 340B through the system’s charity pharmacy services, which provide free medications to patients with low incomes. Patients with cystic fibrosis have dedicated pharmacist services funded in part through 340B. And through their access to 340B pricing, outpatient pharmacies located at the hospitals provide affordable medications to discharged patients who are heading home. All that 340B-supported care results in better patient adherence to medication regimens and lead to better health outcomes for those patients, Ashley said. The findings help enrich a deep and growing evidence base for the vital role 340B is playing in American health care. Researchers from the University of Arkansas and the University of Colorado published their findings in Health Affairs, one of the nation’s most respected policy journals. The researchers wanted to see whether becoming 340B-eligible led rural hospitals to add critical services to prevent and treat cancer. They focused on rural general acute care hospitals that lacked oncology services in 2011. 340B’s Support for Rural Health Congress created 340B in 1992 to help hospitals and clinics serving large percentages of low-income patients “stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services.” As part of the Affordable Care Act of 2010, Congress voted to expand 340B eligibility to critical access hospitals (CAHs), sole community hospitals (SCHs), and rural referral centers (RRCs), among others. Many of those types of hospitals, which tend to serve rural areas, began to enroll in 2011. To determine the impact of 340B eligibility on cancer care availability, the researchers looked at three sets of hospitals – the early adopters that enrolled between 2012 and 2015, the late adopters that enrolled in 2016-18, and those that did not enroll. The researchers found the 340B hospitals were 8.3 percentage points more likely to have added oncology services by 2020. “After 340B enrollment there was a clear and persistent increase in the probability of offering oncology services at newly participating hospitals,” the researchers write. Cancer Care Struggles in Rural America It has been well-documented in numerous published studies that the 46 million Americans who live in rural parts of the country face significant challenges when it comes to cancer care and prevention. More than 46 million Americans, or 15% of the U.S. population, live in rural areas as defined by the U.S. Census Bureau. Access to cancer care and prevention is especially vital for residents of rural communities, which are often underserved and struggle to gain access to needed care. The National Cancer Institute has found that rural residents face serious disadvantages compared to urban residents, including socioeconomic deprivation, limited access to quality health care, and risk factors for cancer. While there are many reasons for these disparities, including behavioral and environmental factors, the lack of access to cancer care and prevention is a key factor. Rural Americans must travel farther to see a cancer specialist. In fact, about one in five rural residents must travel 60 miles or more to obtain care. Because cancer care often requires multiple visits for treatment, rural cancer patients have a harder time accessing and receiving needed treatment. More Progress Needed The impact of these disparities is illustrated by research finding that while cancer incidence rates in urban and rural areas are about the same, cancer mortality rates in rural communities are statistically higher. While cancer mortality rates have been dropping in the U.S., a study looking at the annual, age-adjusted mortality rates for all types of cancer during 2006-15 found rates declined more slowly for rural parts of the country (-1.0%) than in urban areas (-1.6%).  The authors of the latest study point to another factor in access to cancer care in rural America. The newly participating hospitals that added oncology services were disproportionately located in Medicaid expansion states and in counties with lower uninsurance rates. The ACA allowed states to expand Medicaid eligibility to include individuals with income up to 138% of the federal poverty line (or a little more than $20,000 a year). According to the Kaiser Family Foundation, 40 states and the District of Columbia have adopted the Medicaid expansion and 10 states have not. Most of the states that have not expanded eligibility for Medicaid are in the southeast quadrant of the country, which the American Cancer Society has found to have some of the highest cancer mortality rates in the country. ]]> New Research Finds No Evidence 340B Discounts Affect Prescribing /2023/06/new-research-finds-no-evidence-340b-discounts-affect-prescribing/ Thu, 15 Jun 2023 18:49:55 +0000 /?p=9634 Print Article June 15, 2023— Newly published research illustrates that while 340B hospitals tend to treat patients who are sicker and in greater need of care, they do not prescribe more expensive drugs than non-340B hospitals after those differences are taken into account. The latest evidence appears in the peer-reviewed Journal of the American Medical Association (JAMA). Researchers examined Medicare Part D data for fiscal year 2020 to see if physicians in 340B-eligible hospitals were more likely to prescribe more expensive brand-name drugs versus the lower-priced generic products available on the market. After examining drugs prescribed for patients of 340B-eligible hospital providers versus those ordered by non-340B hospital prescribers, the researchers report they “found no meaningful difference in overall generic prescribing rates.” In FY 2020, generic drugs made up 86.6% of Medicare Part D prescriptions from non-340B hospital prescribers and 86.4% of Part D prescriptions from 340B-eligible hospital prescribers. But after adjusting for differences in therapeutic class mix, the figure for 340B-eligible hospital providers was 86.6% – identical to the non-340B hospital rate. The researchers looked more closely at the various classes of drugs covered by Medicare Part D and found 11 classes for which 340B-eligible prescribers had statistically significant higher generic prescribing rates and 17 classes where non-340B prescribers had statistically significant higher generic prescribing rates. In three-quarters of those cases, the differences were less than two percentage points. The researchers concluded that such variation was more evident for drug classes that are not prescribed as much, for which a difference in numbers of prescriptions translates into larger percentage-point differences. For cardiovascular agents, the drug class with the highest use, 340B-eligible prescribers had a 95.9% generic prescribing rate compared to 95.7% for non-340B prescribers. Important Data for Policymakers This new research is critically important as policymakers on Capitol Hill debate proposals to restrict the size of 340B. One of the arguments frequently put forward in favor of such legislation is that 340B hospitals have a financial incentive to prescribe branded drugs when a less expensive generic product is available. Under the 340B statute, eligible covered entities can purchase outpatient drugs at a discount and use the savings to “stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services.” 340B requires a minimum discount of 23.1% for outpatient brand-name drugs and 13% for generic products. Some have suggested that the higher savings for branded versus generic drugs could induce 340B hospital prescribers to favor the costlier products. An important factor in comparing 340B and non-340B hospitals is the type of patients treated by each. For example, the JAMA researchers noted 340B-eligible patients with Medicare are more likely to be dual eligible and/or disabled. Most dually eligible patients qualify for both Medicare and Medicaid because they are living with low incomes. Disabled people who are eligible for Medicare tend to be younger than the traditional group of beneficiaries who are ages 65 or older. Those differences in types of patients also are associated with differences in their health needs and the complexity of their care. A Growing Body of Evidence In January 2020, the independent Medicare Payment Advisory Commission (MedPAC) issued another important report investigating these factors. MedPAC looked at the use of oncology drugs for Medicare Part B patients diagnosed with five forms of cancer (breast, colorectal, prostate, lung, and leukemia and lymphoma). They found that while there were variations in spending by 340B and non-340B hospital prescribers, they were “unable to attribute these findings to incentives created by 340B discounts.” Another study also published in JAMA confirmed that there is no drug spending difference between 340B and non-340B hospitals once beneficiary-level risk factors and hospital characteristics are taken into account. Like the new JAMA study, MedPAC’s researchers noted important differences in patient mix and hospital size. For example, they noted that 340B hospitals are more likely to be larger teaching hospitals and caring for patients who are young, disabled, and receiving assistance designated for low-income Medicare beneficiaries. The 19 drug companies that have imposed restrictions on safety-net hospital access to 340B drug pricing program discounts collectively brought more than $660 billion in revenues in 2021. The banner year for the companies occurred as hospitals reported increasing losses of 340B savings from the drugmaker-imposed cuts. The company reported significant growth in its pharmaceutical sales last year for top-selling drugs used to treat various cancers, inflammatory diseases, and schizophrenia. Also contributing to this total figure were sales of the COVID-19 vaccine the company produced under its subsidiary Janssen. J&J’s restrictions on 340B did not take effect until May 2, so the 2021 revenue figures account for what the company brought in while offering full discounts on all eligible drugs. Pfizer was close behind J&J, bringing in a record $81.3 billion in revenue last year. This performance was driven largely by sales of Comirnaty, the COVID-19 vaccine that Pfizer and BioNTech produced jointly, and Paxlovid, the Pfizer medication for patients with COVID infections. At the same time Pfizer was preparing to release its report, the company announced it would become the 13th drugmaker to impose pricing conditions for safety-net hospitals on 340B drugs dispensed at community-based pharmacies. To continue receiving discounted prices through their community pharmacy partnerships, hospitals must hand over pharmacy patient claims data the company is demanding. The policy, which took effect March 1, targets Xeljanz, a drug used to treat certain types of arthritis, as well as 11 oral oncology drugs that the company makes for patients living with cancer. Pfizer brought in more than $2.5 billion in Xeljanz sales alone last year. Following Pfizer in the revenue column is AbbVie, which reported that it brought in more than $56 billion in 2021. This figure was driven largely by sales of the immunosuppressive medication Humira, which at more than $20 billion in net revenues is the world’s top-selling drug. AbbVie implemented its restrictive 340B policy on Feb. 1, the day before its financial report came out. The policy, which also conditions discounted pricing through community pharmacies on the submission of patient claims data, applies to a list of two dozen drugs that includes Humira. The company has warned that it might consider expanding that list to restrict access to discounts on more of its drugs. One of the newest drug companies to announce 340B restrictions is high up on the 2021 revenue list as well. GlaxoSmithKline (GSK) pulled in almost $47 billion last year, nearly $6 billion of which came from the sales of inhaler products used to treat patients with respiratory diseases such as asthma and chronic obstructive pulmonary disease (COPD). In a policy that took effect April 1, GSK is targeting these respiratory drugs – including bestsellers Advair, Breo, and Ventolin – with 340B pricing restrictions for hospitals that do not turn over claims data. Cuts Are Leaving Deep Marks 340B restrictions by 10 drug companies – AbbVie, Amgen, Bausch Health, Biogen, Bristol Myers Squibb, Exelixis, Gilead, GSK, J&J, and Pfizer – did not take effect until after the start of 2022. Collectively, those companies alone brought in nearly $400 billion in revenues even while offering unrestricted 340B discounts to safety-net hospitals, health centers, and clinics. Hospitals have been reporting significant financial pain caused by the 340B restrictions. In July 2020, Eli Lilly became the first drug company to start denying discounts on drugs dispensed at community pharmacies. Those restrictions now have been in place long enough that they have been noted on numerous Lilly financial reports as a significant contributor to the company’s bottom line. Lilly reported more than $28 billion in revenues in 2021 on the strength of top sellers Trulicity, Humalog, and Jardiance. Those are diabetes drugs for which safety-net hospitals are sustaining some of their largest losses of 340B savings under drug company restrictions. The 19 restrictive policies involve many of the best-selling drugs in the world needed to treat patients with diabetes, cancer, respiratory diseases, and other chronic illnesses. When a company refuses to offer a discount on one of these drugs, resources that hospitals say should be going toward the health care safety net instead shows up as part of a revenue column on these financial reports. Price Hikes, Like Clockwork As drug companies were putting the finishing touches on their earnings reports, they were also engaging in another annual tradition – price hikes. Companies typically raise prices in two batches, once at the beginning of the year and again in the middle of the year. January 2022 was no exception. The Campaign for Sustainable Drug Pricing (CSRxP), which closely monitors drug pricing and promotes policy solutions designed to lower drug prices, reported on the first batch of 2022 price hikes in January and again in July. CSRxP noted 785 such increases on brand-name drugs at the beginning of the year, with the average increase approaching 5%. The group reported a second round of increases for more than 100 brand-name drugs in the first week of July alone. Some of the more notable pricing jumps involve drug companies that are among the 19 now limiting 340B discounts. For instance, Pfizer increased its prices on more than 100 of its drugs, including a 16.8% boost on the price of the frequently prescribed drug Solu-Cortef, which treats a wide range of conditions. GSK raised prices on more than 30 of its drugs, including 7% hikes for cancer drug Zejula and anti-seizure drug Lamictal. BMS hiked the price tags on more than a dozen of its most-prescribed drugs. Gilead raised prices more than 5% on its HIV drugs Biktarvy and Descovy. Some drug companies have limited their 340B pricing restrictions only to certain drugs, but they could expand those lists at any time. The earnings reports and the news about price increases together formed another reminder that the pharmaceutical companies are the ones that determine the prices of drugs, based largely on what they expect the market will bear. At Riverside Regional Medical Center in Virginia, one discharged patient with diabetes was readmitted to the hospital within days due to delayed access to an insulin medication for which drug companies had restricted 340B pricing.  Cynthia Coffey Prior to the drug company 340B restrictions, the patient paid less than $10 out of pocket for insulin he received at the hospital’s community pharmacy, according to Cynthia Coffey, Riverside’s director of retail pharmacy. Once the 340B restrictions went into effect, the patient’s out-of-pocket cost for insulin skyrocketed to $191 each time he filled his prescription at the community pharmacy located in the hospital. That price was just too much for the patient. “The patient chose to not fill the medication,” Coffey said. “Unfortunately, two days later, the patient was readmitted into the hospital due to uncontrolled blood sugars.” Coffey shared this patient’s story as one of our newest Faces of 340B profiles. The story is an example of the barriers the hospital’s patients face from 19 drug companies’ unlawful restrictions on access to 340B pricing. It is not the only example of harm that Riverside Health System’s patients have experienced from these actions. Disruptions to Medication Regimens   Riverside Shore Memorial Hospital is another hospital in the Riverside Health System. Located on the Eastern Shore of Virginia, the hospital serves a community that is physically separated from the rest of Virginia by a bay. The only way to access the Eastern Shore by land is to travel over an 18-mile bridge/tunnel that includes a toll. The hospital also serves a high percentage of patients living with low incomes. 340B savings for Riverside Shore Memorial fund the hospital’s comprehensive oncology services, including radiation and infusion treatments. If the hospital did not offer these services, patients on the Eastern Shore would need to drive 90 minutes to access these vital services. The hospital notes that without access to 340B savings, the hospital would be at risk of closing its doors due to its razor-thin operating margins. David Jones The 340B community pharmacy restrictions have reduced these savings for Riverside Shore Memorial and harmed patient access to medications. David Jones, MD, the chief medical officer at Riverside Shore Memorial, is another of our newest Faces of 340B profiles. He notes that given the geographic barriers Eastern Shore patients face, the hospital’s 340B community pharmacy partnerships enable patients to fill their prescriptions much closer to home. This access improves medication adherence. Jones specializes in pulmonary medicine and cares for many patients with asthma and chronic obstructive pulmonary disease (COPD). After the drug company 340B restrictions started, he saw his patients run into barriers to affording vital inhaler medications. “I have a patient with COPD and because he is underinsured, he is unable to afford the basic maintenance inhalers he needs,” Jones said. “Due to the restrictions, he was unable to obtain one of those medications that he had previously been on. As a result, he had increasing symptoms of shortness of breath, wheezing, he required additional office visits, and we had to supplement his medications with other medications that had increasing side effects and were not as effective for him.” More Emergencies on the Horizon? 340B community pharmacy partnerships are critical to Riverside Health System’s efforts to provide comprehensive care to uninsured and underinsured patients, Coffey said. Many of the patients who receive low-cost medications dispensed at Riverside Regional Medical Center’s community pharmacy cannot access the medications through drug company assistance programs. She and Jones are both concerned about what will happen to their patients if drug company restrictions on 340B pricing continue and worsen in 2023. “I feel certain that patients that have relied on these discounts to obtain the most basic of medications won’t have access to it,” Jones said. “This will result in increasing emergency department visits and most likely hospitalizations. While those hospitals say the end to the cuts is welcome news, they continue to wait for repayments to remedy the nearly five years’ worth of pay cuts that many 340B hospitals have absorbed. January 2023 will mark the return to a full year of 340B hospitals receiving the same Medicare reimbursements for outpatient drugs that all hospitals receive under the outpatient prospective payment system (OPPS). Beginning in 2018, many 340B hospitals were hit with a cut of nearly 30% in Medicare Part B payments under the OPPS for drugs they purchased at 340B prices. The Centers for Medicare & Medicaid Services (CMS) is now restoring payments to their prior levels following a prolonged legal battle that ended with a unanimous U.S. Supreme Court agreeing the government had overstepped its authority in making the cuts. How 340B Hospitals Got Here The Trump administration put the cuts in place as part of its final OPPS rule for calendar year 2018. CMS offset the estimated $1.6 billion in annual cuts with corresponding increases in payments for non-drug services under Medicare “budget-neutrality” rules. A group of 340B hospitals and national organizations went to court to block the cuts and won an early decision in their favor. But an appeals court reversed that decision, bringing the case to the Supreme Court. In June 2022, the high court unanimously decided that the pay cuts were indeed unlawful. In July, CMS announced in a proposed rule that it intended to end the cut and restore full outpatient drug pay rates starting Jan. 1, 2023. A lower federal court ordered the current payment cuts to end immediately, a change that CMS implemented in late September. On Nov. 1, the government issued a final rule setting Medicare outpatient payment rates for 2023 and confirming the cuts would end. “We are very pleased to see that CMS has restored equity to the Medicare outpatient prospective payment system,” 340B Health President and CEO Maureen Testoni said in a statement. “For 30 years, 340B has supported hospitals in serving patients living with low incomes, and full Medicare payments for those services are essential for the health care safety net.” The latest development also is likely to have wider benefits for 340B hospitals beyond what they receive from Medicare for drugs they purchase. Private insurance plans often follow Medicare’s lead when setting their own payment rates. Medicare’s elimination of the differential payments eliminates a major precedent that private payers had been citing for imposing discriminatory payment rates of their own on 340B hospitals. Undoing Past 340B Cuts The official change in Medicare rules to restore the full statutory payment rates to 340B hospitals does not settle the issue of providing remedies for past cuts. When the Supreme Court decided those cuts were unlawful, it obligated CMS officials to find a way to provide relief for hospitals that had been affected by the reduced payments. 340B Health and other hospital advocates are calling for lump-sum repayments of all losses, plus interest, to hospitals for the additional amounts they should have received since 2018 had the unlawful cuts not been in effect. These comments note that CMS officials can make those repayments on a hospital-by-hospital basis without adjusting payment rates for any other outpatient services. It is not clear when the government and hospitals will resolve the repayment question. In its final 2023 OPPS rule, CMS pledged to devise a plan through a separate rulemaking process, which is expected to take months to unfold. The government says it plans to propose a remedy, consider public comments, and issue a final rule sometime next year. As 340B hospitals await clarity on how they might be repaid for Medicare cuts, they are preparing for another year of caring for patients in need. По его словам, совокупность геополитических, экономических и административных факторов создаёт предпосылки для масштабного перемещения людей в регионе. Эксперт подчёркивает: это не обязательно означает хаотичный или неконтролируемый наплыв, но региону придётся оперативно адаптировать социальную, экономическую и инфраструктурную политику к новым реалиям. Одной из ключевых причин, по мнению Куницына, станет изменение миграционных потоков в соседних регионах и странах. В течение 2026 года ужесточение контроля и экономические трудности в ряде центральных и южных областей подтолкнули части трудовых мигрантов к поиску альтернативных маршрутов и рабочих мест. Урал, оставаясь относительно промышленно развитым регионом с большим спросом на трудовые ресурсы в строительстве, сельском хозяйстве и добывающих отраслях, выглядит привлекательной целью. Tsitsikamma Coastal National Park is one of the most scenic parts of the Garden Route.  It is incredibly relaxing to stroll on the wooden boardwalks through the coastal forest, with stunning views over the Ocean. These boardwalks lead to the suspension bridge over the Storms River Mouth, which makes for great photographic opportunities. If you are up for modest accommodation, we highly recommend an overnight stay in one of the ocean-view wooden cabins. Some of them are located right at the shore and rocks, and you would have great views over the Ocean from your own patio. The National Park is a beautiful, restful experience. Whether it is a calm sunny day or whether it is stormy weather when the waves are crashing on the shore in an impressive display of the Ocean. The only way to see the inaccessible Storms River Gorge and Waterfall are with the Spirit of Tsitsikamma boat cruise that starts in the National Park on the ocean side, and then enters the Storms River. Another favourite activity here is doing the Kayak-Lilo excursion – a great way the natural beauty of Tsitsikamma.  Discover the gems of activities around Storms River Village  There are many activities to enjoy in the Storms River Village / Tsitsikamma area. Some of these are ziplining across a waterfall which is an easy 1-hour activity to enjoy whether you are 3 or 80 years old. Birding in the Overberg : “Beauty” (Blue Crane Birds) and the “Beast” (Cape Vultures) It is not unusual to see Blue Cranes, South Africa’s elegant national bird, chatting to their sheep friends next to the farm roads. Almost half of South Africa’s Blue Crane population of about 25000 birds are found in the Overberg. As these stunning birds are under threat, seeing so many of them along the Overberg roads is a privelige we hope to continue enjoying in future. The only remaining colony of Cape Griffon Vultures in the Western Cape, live in the steep cliffs of the Potberg Mountains in the De Hoop Nature Reserve. They now flourish where their numbers were previousdly dwindling. The farmers and community have been instrumental in protecting the colony. 8. The adorable African Penguin Colony at Stoney Point in Betty’s Bay is a delightful experience Traveling in Cape Town’s direction, just before Clarence Drive, we stop at Stoney Point in Betty’s Bay and stroll along the wooden boardwalk to the controlled access point where, for a nominal entrance fee, one can observe the African Penguin Colony. We love their former name, the “jackass penguins”; this aptly links them to donkeys because of the braying sound they make . The adorable penguins, all dressed up in their “tuxedos”, must rate as one of the cutest birds we’ve ever seen. This is a wonderful opportunity to see them in their natural habitat, where they are not enclosed, and free to come and go as they please. They share their “home” with cormorants and many other sea birds. A jacket is advisable as the temperature can be cool with a strong wind. 9. The charming back-roads, meandering through the farmlands, create a wonderful ambiance We love traveling along the back-roads that pass the farms in the Overberg. Away from the hustle and bustle, there’s time to stop and take pictures and to enjoy the views of the rolling farmlands and the beautiful scenery. 10. Bright yellow Canola Fields and Blue Mountains make for great photo opportunities During the late winter and early spring months, the magnificent bright yellow canola fields stretch as far as we can see. We tried to capture it on camera, but there’s nothing that comes close to seeing the bright-coloured fields “live”. Behind the rolling hills of the Overberg, the backdrop of rugged blue mountains makes a stunning picture and it is easy to see why they have inspired many South African songs. 11. Cape Agulhas (or known as Cape L’agulhas), the iconic meeting place of the “Two Oceans” There’s something special and magical knowing we are standing on the southern-most tip of Africa, that iconic point on the African map. When we face the ocean, the Atlantic Ocean is on our right ; the indian Ocean on our left. This is the southern-most point that the early seafarers had to circum-navigate when they were searching for safe passage to the spice islands of the East. 12. Clarence Drive, our magnificent scenic coastal drive, is one of the most scenic roads you’ll experience. Clarence Drive is considered one of the most scenic drives in the world (as per our guests’ opinion) and the stunning scenic splendour entices you to stop at the viewpoints to try and capture the beauty with a photograph. The road is hugged snugly by the mountain range on one side and the ocean on the other. Whether it’s a sunny or cloudy day, the scenery is truly spectacular and we’ll never tire of dricing along this beautiful road. A walk through the bustling city-bowl of Cape Town would certainly contradict this statement, but a short drive outside of any city centre, and deep into the space of the nature reserve will reveal an altogether different story. Find yourself off the grid and away from the tall office-block buildings, the smells of freshly tarred roads and the collective city sounds of revving cars, loud music spilling out of apartment windows and everything else that is decidedly urban – and the wildness of Africa could feel just a little too close for comfort. But when it comes to the wild, it attracts more than tourists and holidaymakers or hunters and poachers, as a curious and captivating figure seems to have emerged: the wildlife whisperer. Kevin Richardson ‘The Lion Whisperer’, Lawrence Anthony ‘The Elephant Whisperer‘ and Mike ‘Sharkman’ Rutzen have become global names for their phenomenal and seemingly crazy efforts with wildlife – all of whom were born and bred on the colourful shores of South Africa! Each of these men have captivated the world in one way or another, and for animal lovers and haters alike, their plights with wildlife creatures have been inspiring; have a look: The Lion Whisperer Who said cats hate water, anyway? Lions, as the apex predator of the savannah, are typically looked at as rather dangerous – understandably so, considering that their jaws can crush bones and an adult’s paw is almost the size of a human head. But imagining Africa without Lions seems almost impossible, since the two are so mutually exclusive. “Lion’s live on less than 20% of their former habitat due to destruction, human-predator conflict, poaching, hunting, and illegal trade” – Kevin Richardson. Richardson has developed unique and awe-inspiring relationships with Africa’s big cats – bonds which are over 16 years in the making. He’s appeared on several local and international news networks, and his philosophy of building loving and caring ties of trust between himself and these predatory cats is beautifully endearing. Looking content & comfortable. His efforts to shine a spotlight on the plight of Lions in both the wild and in captivity has captured plenty of attention around the world. The way he interacts with these animals is astounding; they look like nothing more than oversized kitty-cats when Richardson is around them. This video will have you gasping and gawking in delight: () The Elephant Whisperer Anthony interacting with the elephants he loved. Lawrence Anthony’s remarkable story and his work with African elephants is widely-known across the world. The relationships he developed with Africa’s largest land animal, his legendary conservation efforts that included establishing conservation groups and nature reserves, as well as his intimate approach to rehabilitating traumatized elephants is well-documented in his co-written book, The Elephant Whisperer. A heart-warming, tear-jerking, all round inspiring novel. This man was remarkable; after being called upon by a conservation group to rescue a group of nine, destructive elephants in KwaZulu-Natal, Anthony successfully managed to rescue the herd and save them from being shot. He provided them with a home at Thula-Thula nature reserve, where he eventually earned their trust and became a part of the family. What’s more – after Anthony’s passing in March 2012, two herds laboriously trudged across acres of the Thula-Thula Reserve and loitered on their friend’s compound for two days before retreating back into the wilderness. It’s remarkable, because these gentle giants are well-known for mourning their dead and the herds had, presumably, gone to the Anthony compound to pay their respects. The Sharkman He’s tallied over 800 hours of free-diving with sharks, and discovered that massaging a white shark’s nose causes tonic mobility. Another local wildlife whisperer, Mike Rutzen, is fast approaching legendary status for his interaction with Great White sharks. Footage of his nail-biting free dives have folks labelling him as crazy, while at the same time, alerting us all to the gross misconceptions that have shrouded these apex predators since the pilot screening ofJaws. Rutzen started out as a fisherman catching yellowtail, and recalls white sharks launching themselves on the deck of their fishing boat to get at the freshly caught fish. Close to 20 years later, and Rutzen’s come a long way; his shark cage diving business, White Shark Diving Unlimited, is one of the popular diving operators in Gansbaai and his business has hosted many, many celebrities over the years. “My business gives the great whites a value alive, whereas previously their only value was their fins and jaws.” – Mike Rutzen Rutzen pictured here with actor Brad Pitt. This past summer’s extreme weather has brought home the intensity of the climate crisis for many of us around the world: from the catastrophic floods in Nepal, to the lethal heatwaves across Europe and India, to the choking wildfires in North America, France, and Spain.  As if that isn’t enough, we are seeing these extreme weather crises going hand-in-hand with information crises. The danger of heatwaves is being downplayed. Flooding is getting blamed on foreign neighbors. Wildfires are being attributed to a land-clearing conspiracy. As fossil-fueled extreme weather continues, it is also fueling an information crisis, bringing climate denial, AI slop content, political propaganda, extremism, and online rage bait. This has ramifications that endanger lives, divide societies, and undermine climate action.  We must protect ourselves and those around us not only from the physical impacts of extreme weather but also from the disinformation that comes with it. The good news is, we can keep ourselves safe by anticipating and short-circuiting lies and distortions.  A new toolkit ‘Weathering Extremes,’  by Jennie King  from CCS lays out some practical steps forward:     A Spanish military unit battles wildfires in August 2026, which displaced more than 300,000 people across Spain and France. (Military Emergencies Unit, Spanish Ministry of Defense)   Understanding the Threat Wildfires, floods, and other extreme events bring a host of problematic claims and narratives. Experts sometimes talk about false information that is spread knowingly (disinformation), or  false information spread unknowingly (misinformation), or even  cherry-picked facts that are spread to manipulate (malinformation). But the most important thing is the impact this kind of information has, not the mindset of the person clicking “share.”  In the wild, we might encounter three main categories of deception around extreme weather. These can look different from traditional denialism in climate debates. But they have a variety of harmful impacts. The first category is environmental, which attacks science or cherry picks facts to separate a disaster from its climate-related causes. To use wildfires as an example, this could mean saying this wildfire was started by an arsonist, not climate change. That may be true, but it obscures the fact that climate change is making wildfires more likely and intense, regardless of how they start. Over time, this can weaken our understanding of climate change, and slow down climate action.  The second category is political, which attacks disaster response efforts, often to grind an unrelated partisan or geopolitical axe. For example, Russia falsely claimed the wildfire wouldn’t have been so bad if the US hadn’t wasted resources supporting Ukraine, or the US President misleadingly argued that the LA wildfires are the fault of his political rival’s water policies. These claims can polarize governance, undermine disaster relief or recovery, or empower authoritarian leaders. The third category is hateful and extremist, which blames disasters on minorities or promotes conspiracy theories. These deceptions might look like the wildfires were caused by Jewish weather control or that foreigners are stealing all the disaster aid. Such falsehoods can divide societies or endanger the safety of vulnerable people.    The Perpetrators and Motives These claims are mostly about directing blame, whether to “natural” causes, to conspiracies, to minorities, to political rivals or foreign enemies–basically, anything but climate change. So what is really behind the lies? It comes down to money and power that vested interests have on the line during chaotic, viral climate disasters. Big Oil & Big Tech Private interests have a great deal of money to protect or gain from high-profile extreme weather events. Most obviously, oil, gas, coal, animal agriculture, and other polluting sectors don’t want disasters to motivate climate action that threatens their profits. The role of corporate lobbying, advertising, and PR in climate disinformation and on climate  obstruction, i.e. creating doubt on the severity of the climate crisis has been documented for decades. But more recently, the interests of Big Oil are increasingly aligned with those of Big Tech.  Social Media Algorithms Once upon a time, social media promised an open shortcut for connecting with other people, be they friends, family, or strangers. But along the way, social media became a stew of addictive algorithms, with users forever scrolling in circles, where attention is sold to advertisers rather than browsing an open web. Now, online conversation around disasters favors whatever earns clicks and cash– whether it’s rumors, rage bait, AI slop, propaganda, or hate speech. As a result, the social media channel for public engagement has been hijacked by dishonest influencers, polluters, and bots. That leaves policymakers and the public struggling to find reliable information during disasters, right when they need it most. Political Ideology Besides money, disinformation about extreme weather is also motivated by ideology and politics. Authoritarian governments see extreme weather as a chance to attach their message to a major news event and to undermine opponents and critics. Political extremists, especially the far right, have started treating climate change not just as an energy debate but as an enemy cultural identity. These actors are not the cause of our toxic information environment, but exploit and shape it to serve their goals. Often, all these motives are mixed. For example, Russia has covertly sponsored far-right climate ‘disinfluencers’ in the United States, and the state-owned gas company owns Russia’s largest media conglomerate, showing how directly fossil fuel interests, social media, and global politics are intertwined. During Hurricane Helene in 2024, the far right, online provocateurs, and Russian media spread climate denial, partisan attacks, and xenophobic messages, fueling security threats to public officials and FEMA relief workers (FEMA).   What Can We Do? There are ways we can protect ourselves and our communities from these threats:   We as communities and climate concerned citizens should expect disinformation during extreme weather as the rule, not the exception. Unfortunately, these information crises can happen anywhere, as local events are co-opted into viral narratives. By reading this and the resources here, you are already ahead of the curve.  Second, we need to sidestep the algorithm and not rely only on social media. It’s tempting to focus all our energy online, but social media tends to reward lies and nonsense over truth. As Jennie King writes in Weathering Extremes, “information resilience is built on networks, not newsfeeds.” That means real-world connections matter – personal relationships, ties with local governments, or simple tools like email lists, phone trees, and group texts – that we can build before a disaster strikes. And it’s worth remembering: as Yale’s Climate Maps show, most people in the U.S, for example, support key climate actions – even if you’d never guess that from social media. Third, we can build defenses ahead of time, rather than scrambling to counter or fact-check every rumor and lie during a crisis. One way is “pre-bunking”– warning people in advance about certain narratives, messengers, or tactics they are likely to run into. In fact, the disinformation examples earlier in this piece are themselves a form of pre-bunking. Another tool is the “truth sandwich” – leading with the truth, then knocking down a false claim or bad source, before reiterating the facts again. We can also work to get accurate information into the data AI chatbots learn and draw from – also called “generative engine optimization”. This is increasingly important as AI use grows, and because cunning governments and companies are already trying to feed their own messages into chatbots. Finally, as citizens and civil society, we can push for the global, systematic reforms needed to make our online environment more trustworthy. Reforms that call for improved safety, transparency, privacy, and accountability standards for tech platforms endorsed by CAAD are a great starting point, and at will  unwind some of the toxic incentives that give lies and outrage the upper hand.    Toward Information Resilience Climate change and disinformation are two of the systemic challenges of our time, and they are colliding in harmful ways. Neither will be solved easily. But more people and institutions globally are tackling the challenge, from the UN to researchers to certain governments. Ultimately, reliable information is just as much a part of necessary climate infrastructure as is solar and wind power or flood defences. And he used the moment to make the case, repeatedly and forcefully, for international cooperation at precisely the moment when it is under greatest strain. In his final General Assembly address, Guterres called on governments to adopt national plans to transition away from fossil fuels, aligned with 1.5°C, and to protect workers and communities through that transition. And then, in words that will resonate far beyond the UN chamber, he landed on three words: “Polluters must pay.”    View this post on Instagram   A post shared by 350.org (@350org) For us here at 350, that matters. For years, movements around the world have been demanding that those who have profited most from fossil fuels contribute to addressing the damage caused by their pollution. On Monday 21st of September, 350.org along with partners from Greenpeace and Oxfam presented a petition to the UN with over 2 million signatures, calling to make pollluters pay. On Tuesday that demand was articulated from the highest level of the UN.  And it wasn’t Guterres alone. The call for greater accountability from major polluters was echoed by political leaders, including the Spanish minister for ecological transition, Sara Aagesen Muñoz Perhaps one of the most powerful interventions came from Nepal. Prime Minister Balendra Shah addressed the General Assembly following the devastating Bhotekoshi-Trishuli floods, which killed more than 1,400 people and left thousands unaccounted for, according to Nepalese authorities. He spoke not in abstract terms about climate risk, but about families waiting for loved ones who may never come home; communities searching through mud, rocks and floodwater; and homes, schools, roads and livelihoods swept away.  His message was simple: Nepal heard the warnings. But global political action did not move at the speed required. “The Bhotekoshi-Trishuli disaster was not just a local tragedy, it was a warning to the world.” Shah also made the injustice explicit. Nepal contributes less than 0.1% of global greenhouse gas emissions, yet is facing profound consequences from a warming climate. He called for greater climate finance, including grant-based support rather than repeatedly pushing vulnerable countries further into debt.  This is what climate injustice looks like in practice: countries that have contributed least to the crisis are being forced to spend scarce resources responding to its consequences. THE AWKWARD SILENCE Scientists, frontline communities and climate movements have been saying for decades that climate breakdown will amplify existing vulnerabilities, undermine livelihoods, drive displacement and interact with conflict and instability. The intelligence assessment adds another powerful voice to that evidence base. But the fundamental problem isn’t a lack of warnings. It is the gap between what governments know and what they are prepared to do. This brings us to one of the most concerning contradictions at play now: the leaders who will preside over the next stage of the climate negotiations, Australia and Türkiye, both used UNGA and New York Climate Week to talk about energy, electrification and climate action. But they were awfully silent about the elephant in the room: the urgent need to phase-out fossil fuels. Photo credit: Max Phillips   On the positive side, Türkiye launched its 35-by-35 Global Electrification Pledge, calling for electricity to account for 35% of global final energy consumption by 2035, alongside initiatives covering grids, buildings, transport, industry and AI. Electrification is important. But there is a fundamental question that cannot be avoided: electrifying with what, and for who? ELECTRIFY NOW, BUT IN THE RIGHT WAY As 350 and partners argued this week, electrification needs to go hand in hand with rapidly scaling renewables and phasing out coal, oil and gas – and with making climate finance available for developing countries to achieve this. 92+ organizations published this open letter saying that the electrification target counts only if the power is renewable, if it reaches the people who have no electricity today, if the money follows, and if it comes alongside progress in the negotiations on the Just Transition mechanism, finance, and adaptation, and other existing commitments Otherwise, we risk increasing electricity demand while leaving the fossil-fuel system at the center of the energy system. That contradiction was particularly visible in the speeches and policies surrounding COP31. Australia is talking about climate action while its government recently approved an expansion of BHP’s Saraji coal mine. Türkiye is promoting electrification while planning further fossil-fuel power capacity. The issue isn’t whether electrification is necessary. It is. The issue is whether governments are prepared to phase out fossil fuels and support a just transition. Inviting world leaders to Ethiopia in November 2027 in order to find solutions to the climate crisis, one of the defining imperatives of our era.At a time when other world leaders are backsliding on climate action, Ethiopia carried the torch of climate ambition high. As the next host of the Conference of Parties (COP32), this is exactly the kind of ambitious agenda-setting Africa needs, one that prioritizes a people-centered transition away from fossil fuels.  However, with the world on the verge of overshooting 1.5C, every fraction of a degree we can avoid heating matters – and so does every political meeting. The World Bank. The IMF. The UN. The climate finance negotiations. The UN Tax Convention. COP31. Each is another opportunity to close the gap between what governments know and what they do. We cannot wait until COP32 to see the fossil fuels phase out in the agenda. We need it now. UNGA ends. The diplomats leave New York. The speeches disappear from the headlines. But the crises don’t. The world has experienced devastating wildfires, catastrophic floods, extreme heat and a rapidly intensifying El Niño. The impacts are already being felt by people and communities around the world. Next week, the climate season moves to Fiji for the COP31 pre-COP. All eyes will increasingly turn towards the negotiations in Türkiye and Australia. The question for COP31 should be straightforward: Will governments finally put a date on the end of fossil fuels,  and put the money behind a just transition? Because after this week in New York, there is little excuse for pretending we don’t know where the world is heading. We know the risks. We know the solutions. We know who has profited. And, who is paying the price. Now the question is whether governments are prepared to act. Polluters must pay. And the fossil-fuel era must end. The post The Haze We All Breathe: A Letter from Kalimantan, Indonesia appeared first on 350. ]]> This is a first-hand account by Margaretha Winda, a journalist based in Pulang Pisau, Central Kalimantan, on Indonesia’s wildfires, which has produced hazardous smoke that chokes its people and wildlife, and has spread to neighbouring countries Malaysia, Brunei, Singapore, and the Philippines.  There is something ironic about forest and land fires. A blaze may start on a single plot of land. But the resulting smoke and haze do not recognize boundaries of ownership. They move past fences, roads, villages. They enter homes, schools, and the lungs of people who never lit a match. Those who never started the fire are the ones who end up paying for it — and that is as true a few kilometers from the fire as it is a few hundred. What it’s like here I remember one morning this season waking up to a sky already overcast, the sun reduced to a faint, sickly yellow smear behind the haze. The smell hits before anything else — burning peat, sharp and distinctive, the kind that settles into your chest until it’s hard to take a full breath. And yet life around me didn’t stop: women were still washing clothes at the riverbank with no protection at all, no masks, no thought given to a health check. Children rode their bicycles to school through thick smoke, unmasked, because no notice had come telling schools to close. This year is worse than anything I remember from three years ago. Back then, fires happened too, but the smoke never got this thick, and it was brought under control quickly. This year’s fires spread everywhere, and the response never caught up. Where I live, in Pulang Pisau, 1,503 hotspots and 38 fires have burned 334 hectares this season. Jabiren Raya alone accounts for 1,303 hotspots and 257 hectares — including peat near the Tumbang Nusa bridge up to 40 meters deep, among the deepest burning in the regency. Photo: Reuters/Ajeng Dinar Ulfiana I am not writing this from a distance. I’m a journalist, and this season my job has put me at the fire sites themselves, working right up against the smoke. The one thing that’s changed for me is that I now have to wear a mask more carefully than ever, just to avoid getting sick myself — though for most people around me, that choice doesn’t even exist. Work doesn’t stop for haze. There is no version of survival here that includes staying home. It has reached my own family, too. I have two children, and both came down with acute respiratory infections during this haze season that simply wouldn’t clear — coughing, flu, and at one point diarrhea. They were not the only ones. Children and elderly people across my community have ended up in hospital, struggling to breathe. Other impacts I’ve seen Nationally, Indonesia’s health ministry recorded nearly 51,000 acute respiratory infection cases tied to the haze in July and August alone. Two volunteers fighting fires in Central Kalimantan died this month from breathing difficulties. An orangutan was found on a plantation in West Kalimantan and later died of suspected smoke inhalation; firefighters elsewhere have found snake and pangolin carcasses in burned areas. I’ve seen versions of this closer to home too. In Pulang Pisau, in the early morning and evening, visibility here drops to about one meter. The wildlife I’ve seen displaced is mostly snakes, turtles, and monkeys — animals whose habitat simply burned out from under them. A duck farm near one of the fire sites was directly hit as well, its surroundings caught up in the same blaze. On schooling: some areas of Pulang Pisau have shifted to distance learning, but others are still holding regular in-person classes in the middle of all this smoke — there’s no consistent rule, just whatever each school decides on its own. And the government’s presence has been strangely uneven. Even as fires grew large across the district, official attention stayed narrowly fixed on Desa Tumbang Nusa, while other areas burned with far less response.  Smoke doesn’t stop at the borders I’ve lived through enough dry seasons here to recognize the warning signs: the sky dimming to a dull yellow-grey before the smoke rolls in for good. The wind carries the smoke north toward Palangka Raya. This year, it even crossed the Java Sea and kept going — and by the first week of September, four other countries were living inside it too. Singapore. On September 4, residents woke up to a smoggy sky as the air quality index crossed into unhealthy territory. By late afternoon, IQAir’s global rankings placed the city with the third-worst air on the planet. Malaysia. In Sarawak state, the capital Kuching had already been ranked the world’s most polluted city for over a week straight, with a hazardous reading of 311. On September 4, its Air Pollutant Index in the town of Serian surged past 500 — a level that can halt nonessential work and outdoor activity — prompting Malaysia’s king to consent to a state of emergency there. A Serian resident told reporters, “Even when I turn on the air conditioner, I can still smell the smoke in my house.” Days earlier, the health ministry reported asthma cases up 259% and upper respiratory infections up 124% in a single week. The Philippines. Officials declared air quality “acutely unhealthy” across Manila’s metro area, the Visayas, and Mindanao — three separate island groups, hundreds of kilometers apart, choking on the same haze. The health department was telling people to mask up.. Brunei. Hazy conditions have blanketed the sultanate since mid-August, with no clear end in sight as the fires behind it keep burning. We may live in very different places, but our fears are starting to sound the same. Families in Sarawak are now doing what mine has done all season — watching their children for coughs, keeping them inside, waiting for the air to clear. Those who contributed least to causing this crisis are so often the first, and the hardest, hit by it.  An estimated 10.67 million people are affected by this haze here in Indonesia alone.  Citizens have taken this to court before. In 2017, a group of Palangka Raya residents won a citizen lawsuit against the Indonesian government over its failure in handling forest and land fires and in preventing a haze disaster — a court found the state guilty of unlawful negligence, a ruling upheld on appeal and again by the Supreme Court in 2019, only to be dismissed again in a judicial review on 2022. But winning in court hasn’t meant winning in practice: judgments like this one have gone largely unenforced.  So when people elsewhere start watching their children for coughs and keeping them indoors, waiting for the air to clear — I recognize that fear completely. It’s the one my family has lived inside all season, without the world watching. Why this keeps happening Landowners are responsible for safeguarding their land. Companies are responsible for ensuring their operations don’t create risk. Government is responsible for making sure all of this is actually enforced — not just showing up once a fire is already large. And I want to be precise about who is actually burning, because this gets misunderstood constantly, both inside and outside Indonesia. Dayak communities here largely stopped clearing land by fire a long time ago. The fires happening now are not, in my experience, a story about traditional farming practices gone wrong.  Part of why these fires catch and spread so easily  goes back decades, to how this land was managed. Much of Kalimantan’s peat has been drained for industrial agriculture, including oil palm, leaving it dry enough to burn like fuel instead of holding the water that once kept it from catching. Indonesian law requires companies operating on peatland to keep it wet, but those rules are widely ignored, and to make it worse the President dismantled the ad hoc government agency that tasked specifically for peat restoration last year, leaving accountability gap that no other institutions seem to prepared to fill. Longer, hotter dry seasons — the kind scientists tie to the broader climate crisis – are setting the stage for fires to start faster and burn longer. Layered on top of this year’s own strong El Niño, fires can now burn underground indried-out peat for months. The way forward Communities here already practice gotong royong — mutual cooperation, neighbors carrying each other through hardship. I opened a volunteer relief post in the Kahayan Hilir subdistrict this season to supply logistics to Masyarakat Peduli Api — the volunteer firefighters actually on the ground putting these fires out. A lot of people have stepped up to donate food and drink for them. But those volunteers are still going out without basic equipment — hoses to fight

O Canadá chegou a quase 900 focos ativos simultaneamente. Junho foi o mês mais quente já registrado na França, 3,8°C acima da média, numa onda de calor que, segundo os cientistas, seria praticamente impossível sem a queima de combustíveis fósseis. Só na Europa, o calor extremo teria matado cerca de 20 mil pessoas – e a conta está subindo.  No Brasil, o alerta é o mesmo. INMET e INPE projetam um El Niño que pode chegar à categoria “muito forte” e tem quase 100% de chance de seguir ativo até o início de 2027. Na Amazônia, os rios já começam a baixar, o que pode travar o transporte fluvial e isolar comunidades ribeirinhas. O Nordeste enfrenta estiagem prolongada. O risco de queimadas dispara do Norte ao Centro-Oeste. O Rio Grande do Sul volta a conviver com tempestades e alagamentos. São as mesmas regiões que nunca se recuperaram por completo do El Niño de 2023 e 2024. Foi nesse cenário de eventos climáticos extremos em todo o mundo que a Petrobras, assim como outras petroleiras, divulgou um trimestre de lucro excepcional. O resultado de abril a junho saiu nesta quinta-feira (06/08): a Petrobras teve R$ 52,4 bilhões de lucro líquido, 96,8% a mais do que no mesmo período de 2025. O que explica o número é a produção recorde do pré-sal somada à disparada do preço do petróleo, impulsionada pela guerra no Oriente Médio. O barril Brent passou de uma média de US$67,82 no segundo trimestre do ano passado para US$ 104,52 neste ano, uma alta de 54%. A companhia bateu recorde na produção própria de petróleo no Brasil, que chegou a 2,7 milhões de barris por dia, e também no uso das refinarias. Vale destacar ainda o crescimento das exportações, que alcançaram quase um milhão de barris por dia no trimestre. Ou seja, o aumento da produção não tem relação com a segurança energética do país. Constitui, na verdade, um dos motores desse lucro fora do comum. A Petrobras não está sozinha. Segundo a análise do The Guardian, as oito maiores petroleiras do mundo somaram quase US$93 bilhões de lucro entre abril e junho. É quase o dobro dos US$50 bilhões do mesmo período do ano passado, o equivalente a quase R$ 60 mil de lucro por segundo. A Chevron quintuplicou seu resultado e chegou a US$ 12,2 bilhões. A ExxonMobil dobrou o dela, com US$ 14,5 bilhões. A mesma crise que encarece a conta de luz, a comida e o custo de reconstruir uma cidade alagada é a que sustenta esses balanços e engorda a conta bancária dos acionistas. Só neste trimestre, a Petrobras aprovou R$ 17,4 bilhões em dividendos e juros sobre capital próprio, distribuídos a acionistas públicos e privados. Dividendos que quase a totalidade da população do país jamais verá. Nos investimentos, o primeiro semestre somou US$10,4 bilhões. Desse total, 84,6% foram para o segmento de Exploração e Produção. A área de Gás e Energias de Baixo Carbono recebeu apenas US$ 203 milhões, o equivalente a 1,95% do total, um valor irrisório. E a maior parte desse pouco foi para o gás fóssil, que está longe de ser uma fonte limpa, e não para as renováveis de fato. A Petrobras é a maior empresa do país, com escala, engenharia e caixa que nenhuma outra tem. É justamente por isso que apostar tudo num ativo em declínio é o maior risco que ela pode correr. O estudo “A Petrobras de que precisamos”, lançado pelo Observatório do Clima em setembro de 2025, mostra que há um caminho financeiramente viável. Começa pelo mais urgente: definir zonas de exclusão em áreas ambientalmente sensíveis e abandonar novos empreendimentos na Margem Equatorial e na Bacia de Pelotas. Passa por ampliar o investimento em biocombustíveis, com controle ambiental e salvaguardas socioambientais garantidas em cada projeto. Inclui também desativar os campos que já esgotaram a vida útil prevista, assumindo o custo desse passivo em vez de terceirizá-lo a empresas menores, sem estrutura para dar conta dele. Em vez de abrir novas fronteiras de exploração, a Petrobras pode calcular quanto petróleo o país de fato vai precisar nos próximos anos e suprir essa demanda com as áreas que já explora. A produção iria diminuindo aos poucos, concentrada nos setores de difícil descarbonização, e abrindo espaço para ganhos com cadeias produtivas e tecnologias de baixo carbono. Não se trata de desligar a chave de uma hora para outra. Trata-se de parar de apostar o futuro da maior empresa do país num ativo que tende a declinar após atingir o pico nas próximas duas décadas, com o avanço da eletrificação e de novas tecnologias. A urgência não é abstrata. A previsão é que a conta de luz suba entre 6% e 7% acima da inflação. Em um país que gera cerca de 88,2% da sua eletricidade a partir de fontes renováveis. Somos, no papel, um caso de sucesso energético. E, ainda assim, nossa conta de luz está entre as 15 mais caras do planeta, comprometendo em média 10% da renda das famílias brasileiras, a maior proporção em relação à renda entre os países da Organização para a Cooperação e Desenvolvimento Econômico (OCDE) e mais que o dobro do que pagam famílias nos Estados Unidos ou no Canadá. Em um ano eleitoral, o custo de vida aparece entre as maiores preocupações da população. Segundo uma pesquisa do Ideia, 74,7% dos brasileiros consideram esse tema determinante para seu voto. Mas existe uma pergunta que ainda recebe pouca atenção: quem está lucrando com esse aumento dos custos? Uma parte significativa do que o brasileiro paga todo mês, para além da energia que consome, é pelos custos que a indústria de combustíveis fósseis transfere, silenciosamente, para a sociedade. Quando o conflito no Oriente Médio provocou uma disparada nos preços internacionais do petróleo e do gás, os impactos chegaram rapidamente ao bolso dos consumidores. O transporte particular ficou mais caro, a produção de alimentos encareceu e os custos energéticos aumentaram em diversos países. Como quase dois terços da população mundial vivem em países que dependem da importação de combustíveis fósseis, o efeito foi global. O Brasil também sentiu isso, ainda que de forma mais tímida, porque o governo fez esforços para conter o repasse do aumento à população. Mas o episódio expôs, uma vez mais, o quanto depender de um sistema instável nos coloca em risco permanente. Crises do petróleo não são exceções. São a regra. A gente já viu várias nas últimas décadas, e sempre haverá uma próxima à vista. Enquanto as famílias precisavam cortar gastos para equilibrar o orçamento, as grandes petroleiras seguiam lucrando com a própria crise. Um novo relatório da 350.org, Out of Pocket, mostra que somente as produtoras de petróleo dos Estados Unidos devem embolsar entre US$ 60 e 63 bilhões em ganhos adicionais só em 2026 por causa da atual crise — mais do que o dobro do que a Agência Internacional de Energia estima ser necessário para levar eletricidade limpa a toda a população da África. Enquanto isso, famílias do Sul Global enfrentam racionamento de combustível, falta de fertilizantes e disparada no preço dos alimentos. A mesma guerra. Resultados opostos. Trata-se de um sistema em que os riscos são socializados e os lucros privatizados: quando os preços sobem, as famílias pagam mais; quando os lucros aumentam, eles ficam concentrados nas mãos de poucas empresas. Esse episódio expôs uma fragilidade estrutural: nossa dependência dos combustíveis fósseis transforma guerras, crises e disputas geopolíticas em aumentos de preços para consumidores que não tiveram qualquer participação nesses conflitos. E o problema não termina aí. Hoje, a sociedade financia esse modelo de três maneiras diferentes. Primeiro, por meio de incentivos e subsídios públicos. Segundo estimativas do FMI, os subsídios globais aos combustíveis fósseis alcançaram cerca de US$ 7,4 trilhões em 2024. No mesmo período, o financiamento climático destinado aos países em desenvolvimento ficou em torno de US$ 115 bilhões. São países que historicamente queimaram muito menos combustível fóssil para se desenvolver e que são os mais impactados pelos seus efeitos, pela falta de resiliência e capacidade de adaptação. Depois, pagamos diretamente na conta de luz, no combustível e nos custos de transporte. E, por fim, pagamos mais uma vez por meio dos prejuízos provocados por secas, enchentes, ondas de calor e outros eventos climáticos extremos. Tem uma ironia cruel nisso: a mesma seca que aciona as termelétricas e encarece a bandeira tarifária é resultado direto da emergência climática alimentada por esses mesmos combustíveis. No Brasil, a contradição é evidente. Ao mesmo tempo em que as contas de energia aumentam, seguimos discutindo a ampliação de subsídios para fontes caras e poluentes. O carvão mineral responde por apenas 1,4% da matriz elétrica nacional. Mesmo assim, o país destinou em média R$ 1,07 bilhão por ano para subsidiar sua geração entre 2020 e 2024. E o Congresso, em vez de encerrar esse suporte, aprovou a manutenção de subsídios ao carvão até 2040, e pretende estender isso até 2050 por meio do PL 1371/2025, que tramita nessa casa. Na prática, estamos planejando depender mais de uma fonte de energia instável, poluente e mais cara. E essa escolha vai aparecer na ponta, na conta de cada brasileiro. Essa escolha também afeta o futuro. Os combustíveis fósseis representam atualmente cerca de 10% da matriz elétrica brasileira. As projeções indicam que essa participação pode chegar a 18% até 2040. Ou seja, estamos planejando ampliar a dependência de uma fonte mais cara, mais instável e mais poluente justamente quando deveríamos estar reduzindo essa exposição. A população já entendeu a direção que quer seguir. Pesquisas mostram que 90% dos brasileiros apoiam investimentos em energias renováveis. Mas também revelam algo importante: 80% não acreditam que a conta dessa transição deva recair sobre os consumidores. Como as pessoas já estão gastando demais com a conta de luz, não deveria caber a elas pagar por esse processo quando há tantas empresas acumulando lucros extraordinários a partir dessa indústria. Uma transição energética justa precisa ser financiada por quem mais se beneficiou economicamente desse modelo. Isso inclui a criação de fundos para apoiar trabalhadores e comunidades que hoje dependem de atividades ligadas aos combustíveis fósseis, garantindo que ninguém seja deixado para trás. O debate energético costuma ser apresentado como uma escolha entre proteger o clima ou a economia. Mas o direito à energia acessível e o combate à crise climática não são opostos. São a mesma luta. Um país com sol, vento e água em abundância não deveria ter uma das faturas de energia mais salgadas do mundo, e não deveria precisar mudar de bandeira toda vez que o inverno chega. Mas o que é exatamente o El Niño e o que ele tem a ver com os combustíveis fósseis que impulsionam a crise climática? Aqui está tudo o que você precisa saber. O que é El Niño? A cada dois a sete anos, as águas superficiais do Oceano Pacífico tropical central e oriental esquentam significativamente acima de sua temperatura normal — e quando isso acontece, desequilibra o clima de todo o planeta. Esse fenômeno se chama El Niño. A ciência Normalmente, os ventos alísios no Pacífico funcionam como um ventilador gigante que sopra pelos trópicos, empurrando as águas quentes da superfície para oeste, em direção à Austrália e à Indonésia — trazendo chuvas, monções saudáveis e oceanos produtivos. Enquanto isso, águas frias e ricas em nutrientes sobem à superfície no leste, mantendo os ecossistemas pesqueiros vivos e os climas estáveis ao longo das costas do Peru e do Equador. Mas, a cada alguns anos, esses ventos enfraquecem, a água quente para de ser empurrada para oeste e o Pacífico oriental esquenta. E, quando isso acontece, esse equilíbrio colapsa: a Austrália e a Indonésia enfrentam seca, a América do Sul enfrenta enchentes, e os sistemas climáticos dos quais bilhões de pessoas dependem são desestabilizados no planeta todo. El Niño se desenvolve pelo aquecimento da superfície da água do Pacífico. (Getty Images) O nome El Niño, em espanhol “o menino”, é uma referência ao menino Jesus. Foi originalmente criado por pescadores sul-americanos que notaram uma corrente oceânica quente ao longo das costas do Equador e do Peru por volta do Natal, remontando ao século XVII. El Niño é uma das fases de um ciclo climático natural maior chamado ENOS (El Niño–Oscilação Sul). Sua contraparte, La Niña (“a menina”), é o oposto: um resfriamento das mesmas águas do Pacífico, com ventos alísios mais fortes. Juntos, El Niño e La Niña balançam os padrões climáticos globais como um pêndulo. O El Niño traz seca ao sul e sudeste da Ásia, à Austrália e ao sul da África, enquanto entrega mais chuva a partes da América do Sul. Já a La Niña inverte muitos desses padrões. Nenhum desses eventos é, por si só, um desastre — eles fazem parte do ritmo climático natural da Terra há milhares de anos. Os problemas começam quando se tornam extremos, especialmente quando chegam a um mundo que já está sobrecarregado. O que está acontecendo agora? Este ano, algo diferente está ocorrendo. O Pacífico acabou de sair de uma fase de resfriamento de La Niña, e o El Niño está se desenvolvendo com velocidade incomum. A questão é até onde ele vai chegar. Para que um El Niño seja oficialmente declarado, as temperaturas oceânicas precisam subir apenas 0,5°C acima da média. Mas a NOAA (Agência Nacional Oceânica e Atmosférica dos Estados Unidos) agora calcula 82% de chance do El Niño se desenvolver até julho de 2026 — e este já parece muito mais sério do que seus antecessores. Um “Super El Niño” ocorre quando as temperaturas sobem 2°C ou mais acima do normal. Esse limite foi ultrapassado apenas algumas vezes na história registrada — em 1982, 1997 e 2015. Cada vez desencadeou secas, enchentes e temperaturas recordes em vários continentes. Mas o El Niño de 1876-78 é considerado um dos mais intensos já registrados e levou a uma fome global que matou cerca de 50 milhões de pessoas na Índia, China, Brasil e no sul da África. Isso representava aproximadamente 1 em cada 28 pessoas vivas na época. Continua sendo o parâmetro para os piores cenários de El Niño na história humana. Atualmente, as previsões alertam que este El Niño pode empurrar as temperaturas oceânicas 2°C ou até 3°C acima do normal até o fim de 2026. Três das principais agências de previsão do mundo projetam que o El Niño 2026 deve igualar ou superar o El Niño de 1878 em temperatura oceânica. E, ao contrário de 1877, este está chegando a um mundo que já está mais quente, com mais pessoas para alimentar e menos margem para erros. Muitos cientistas preveem que 2027 será o ano mais quente já registrado. Impactos As apostas humanas são altas — e têm uma cara diferente dependendo de onde você vive. A África enfrenta algumas das piores exposições: a seca no Sahel e no sul da África ameaça culturas básicas como o milho, enquanto o leste africano enfrenta enchentes severas. O último grande El Niño deixou mais de 30 milhões de pessoas precisando de assistência humanitária só no sul da África. Na Ásia, uma monção mais fraca coloca em risco as colheitas de arroz, trigo e algodão da Índia, enquanto condições de seca ameaçam culturas em todo o Sudeste Asiático e na Austrália. Na América Latina, a América Central enfrenta seca prolongada e insegurança alimentar, enquanto o Peru, o Equador e o sul do Brasil enfrentam o oposto: chuvas intensas e enchentes. As culturas das quais a maioria das pessoas depende — milho, arroz e trigo — tendem a cair globalmente durante os anos de El Niño forte. Em países ricos, isso significa preços mais altos nos alimentos. Em outros, significa fome. E tudo isso está atingindo um mundo já sob pressão — escassez de fertilizantes, disparada nos preços de energia e cortes generalizados na ajuda exterior eliminaram as reservas que antes ajudavam comunidades vulneráveis a absorver esses choques. Por que os combustíveis fósseis tornam os impactos do El Niño muito piores Aqui está o ponto crítico que frequentemente se perde nas manchetes: o El Niño em si não é causado pelas mudanças climáticas. Mas a crise climática, impulsionada pela queima de combustíveis fósseis, está tornando seus efeitos dramaticamente piores. Pense assim: o El Niño libera temporariamente quantidades enormes de calor armazenado no oceano para a atmosfera. Isso sempre causou perturbações. Mas hoje, esse calor está sendo liberado num mundo que já está mais quente do que em qualquer momento da história humana. Então, quando o El Niño pulsa sobre essa linha de base elevada, as consequências são mais severas do que qualquer evento comparável das décadas passadas. “O aquecimento global está dando mais energia a todo o sistema para ser liberada por esses eventos de El Niño quando ocorrem.” — Dr. Daniel Swain, cientista climático Em outras palavras, o aquecimento global funciona como combustível que amplifica a força natural do El Niño — e essa energia extra tem consequências reais: chuvas mais pesadas e tempestades mais destrutivas; incêndios que se propagam mais rapidamente, já que as temperaturas mais altas ressecam a vegetação; secas mais severas em regiões vulneráveis durante os anos de El Niño; e temperaturas recordes. Algumas pesquisas também sugerem que o aquecimento dos oceanos pode estar tornando os eventos individuais do El Niño mais intensos, embora os cientistas ainda estejam trabalhando para entender essa relação por completo. O que está claro é que a linha de base com a qual o mundo está lidando já mudou. Cinquenta anos atrás, um El Niño forte causava danos sérios. Hoje, o mesmo evento seria muito mais destrutivo porque a crise climática já elevou as apostas. Pesquisadores também alertam para um ciclo vicioso: eventos fortes de El Niño atingem regiões dependentes de hidrelétricas com secas, forçando-as a queimar mais carvão e gás para gerar eletricidade — o que, por sua vez, lança mais carbono na atmosfera e impulsiona mais aquecimento. El Niño e combustíveis fósseis passam então a reforçar mutuamente os piores efeitos um do outro. O que isso significa para a luta climática Um Super El Niño não é motivo de pânico, mas sim de ação. Esses eventos vêm e vão. O que não vai embora é o aquecimento impulsionado pelos combustíveis fósseis. Cada fração de grau que a queima de carvão, petróleo e gás adiciona à linha de base global torna o próximo El Niño mais destrutivo. Alguns modelos climáticos agora mostram uma chance real de que 2026 ou 2027 vejam as temperaturas mensais globais ultrapassarem 2,0°C acima dos níveis pré-industriais pela primeira vez na história registrada. São as temperaturas nas quais os sistemas climáticos colapsam, as colheitas falham e a precariedade acumulada por décadas de inação climática se torna catastrófica. Sabemos exatamente o que fazer. A tecnologia existe. O conhecimento existe. O caminho à frente é uma eliminação gradual, rápida e justa dos combustíveis fósseis e uma transição para a energia renovável que não deixe comunidades vulneráveis para trás. O El Niño vai passar. E ainda assim, pode ser um ponto de virada. À medida que a vulnerabilidade do futebol à crise climática se torna impossível de ignorar, cada vez mais torcedores e jogadores exigem que as autoridades esportivas façam seu melhor. A Ameaça do Calor A crise climática alimenta o calor extremo em todo o planeta, impulsionado por décadas de queima de carvão, petróleo e gás. E a Copa do Mundo de 2026 não está imune a isso. Pesquisadores preveem que 14 dos 16 estádios da competição deste ano superarão temperaturas perigosas — com uma em cada quatro partidas devendo ser disputada em condições de calor extremo. No ano passado, uma onda de calor em junho atingiu ao mesmo tempo várias cidades-sede da Copa. Uma repetição neste ano pode colocar os presentes em risco real. Em 24 de junho de 2025, a temperatura chegou a 39°C em Boston. Neste ano, em 23 de junho, a Inglaterra enfrentará Gana nessa mesma cidade — num estádio sem sombra, às 16h… O Gillette Stadium, em Boston, que será usado durante o torneio, oferece pouca proteção contra o calor para torcedores e jogadores. Temperaturas extremas significam ritmo mais lento, menos pressão e substituições mais precoces. 97 das 104 partidas programadas enfrentam maior probabilidade de condições prejudiciais ao desempenho em razão das mudanças climáticas — o que significa que torcedores pagando preços recordes têm cada vez mais chance de assistir a uma partida menos intensa. E enquanto os jogadores contam com equipes médicas e pausas para resfriamento, os torcedores muitas vezes precisam se virar sozinhos. Embora 3 dos 16 estádios sejam climatizados, os torcedores nas filas, nas fan zones e nas rotas de transporte podem ficar expostos ao calor perigoso por horas — frequentemente muito mais do que os próprios jogadores em campo. A FIFA pouco fez para proteger torcedores e jogadores dos riscos óbvios de calor durante as Copas do Mundo. Já vimos as consequências quando isso não acontece: em junho de 2024, um árbitro assistente desmaiou durante uma partida da Copa América, e, no ano passado, a Copa do Mundo de Clubes teve jogadores e torcedores se refugiando em áreas cobertas, enquanto o calor voltava a ser motivo de grande preocupação. Apesar disso, a FIFA selecionou muitos estádios sem qualquer sombra, em locais que normalmente não são usados para esportes de verão. Um grupo de especialistas recentemente pediu à FIFA que fizesse mais para proteger jogadores e torcedores. Eles solicitaram que a entidade reduzisse o limite de temperatura a partir do qual as partidas são interrompidas. Hoje, a entidade só considera pausar o jogo a 32°C (medido pela temperatura de bulbo úmido e globo, ou WBGT — uma escala que considera calor, umidade e exposição solar em conjunto). Esse limite é tão alto que não seria acionado com 45°C e 20% de umidade, ou com 35°C e 80% de umidade — condições perigosas para qualquer pessoa, ainda mais para atletas de alta intensidade.
