I didn't have a bucket handy to do his method, but I have a Weber Q1200 small gas grill, so I wondered if it would be possible to rig something up.<br /> <br /> I came up with the following method. I have repeated this multiple times and have been very pleased with the results. Definitely interested to hear others' suggestions.<br /> <br /> The Weber Q1000/Q1200 are the smallest gas grills Weber makes. For a gas grill, you want wood chips rather than wood chunks to smoke with. I use these:<br /> <a href="; imageanchor="1" ><img border="0" src="; /></a><br /> <br /> One option I have seen is just to wrap the chips in foil and put the packet directly on the fire. As detailed nicely <a href=";, the process has been much simplified, at least if you're on Raspbian. Note, as I commented there, you'll probably want to add <tt>--listen</tt> to your <tt>LIRC_ARGS</tt> in <tt>hardware.conf</tt> so that you can connect to the LIRC daemon from your home network. This allows you to send IR remote control signals out from the RasPi by connecting from any computer. There's also a nice Android app called Amote, probably there are good ones for iOS as well.<br /> <br /> I found that our Samsung BN59-00857A remote worked fine with an <a href=" <tt>lircd.conf</tt> configuration file for the BN59-00856A</a>. (Actually the 00856 includes a few additional buttons not on the 00857; several of these work for controlling our TV even though they're not on our OEM remote!)<br /> <br /> When it came to controlling our GE air conditioner, I didn't find any existing configuration file. The process of getting a working config file together was a little more involved than I expected, so I'm detailing it here and posting the resulting config (also sent it to the LIRC maintainers, but didn't hear anything back).<br /> <br /> I knew from running <tt>mode2</tt> that raw signals from the AC remote were being received correctly. But I wasn't able to get <tt>irrecord</tt> to pick up the AC remote signals even when forcing it into raw mode. It turns out that the GE AC remote has an unusually long code length, 48 bits, so maybe that was part of the issue. I'm also not clear on whether my <tt>irrecord</tt> was going into raw mode correctly; seemed to behave exactly like non-raw mode so maybe there's a problem with the version built for Raspbian? <br /> <br /> Anyway, to get a config file, I had to pipe the <tt>mode2</tt> raw data to text files and then manually pull the text into a raw conf file and filter out the "pulse" and "space" text (thanks vim). I confirmed that the resulting raw file works for controlling the AC through LIRC. Then I used <tt>irrecord -a</tt> to retroactively convert the raw codes into a more succinct config file. Looking at the various wall mount ACs GE sells, it seems likely to me that these codes will work for any unit since the controls all look the same. (If you have a heat+cool unit then you'll be missing the "heat" button since my AC doesn't have that.)<br /> <br /> Hope it's useful. The pieces are available on <a href="; and <a href=";. These pieces were 3D printed by Shapeways in their White, Strong, Flexible material. Screwing the set screws directly into the plastic instead of tapping the holes kept the piece viable for several months before it wore out.<br /> <br /> Managed to get a pretty decent locating fit for a 3/8" x 5/8" x 5/32" sealed, pre-lubricated ball bearing from <a href=";. They have a lot of options in this size. I ordered two to choose from, but only one fit into the printed flange piece easily with just my hand pressure even though they were rated for the same maximum OD and tolerance. So I think the current design is close to the edge of hand pressed fitting for the low tolerance of the current SLS 3D printing process.<br /> <img src="; height="288" width="216" border="0" /><br /> <br /> If you look carefully at the bottom edge of the flange where it contacts the drill adapter, you can see there is a step-wise pattern; this is because I tried to give the surface that contacts the drill adapter a gentle slope to match the slope on the front surface of the drill adapter. It basically works, but the 3D printing leaves this step pattern instead of a smooth slope. Probably not worth the trouble.<br /> <br /> Here's the whole thing assembled. Note that the square driver piece is taking on a healthy coffee color due to the plastic material being porous and absorbing bits of coffee grounds. I've had this piece a while now and it's showing no signs of failing, so I think the 3D printed parts are reasonably successful for medium use. It's also kind of nice to know that the plastic part will fail before you kill the KitchenAid motor if you happen to jam things up some how.<br /> <img src="; height="288" width="216" /><br /> <br /> Here's a picture of the extra 5/8" OD bearing I bought that didn't quite fit nicely. Note that now I'm using it to drive the older Hario conical burr grinder model, rather than the Hario Slim. (We have both, one for home and one for my office. Kitchenaid only at home though ;).)<br /> <br /> <iframe allowfullscreen="" frameborder="0" height="315" src="; width="420"></iframe><br /> <br /> One advantage of the older Hario is that the coupling from the drill adapter to the grinder is simpler. The Hario Slim uses a pentagonal (AKA 5-point) drive in a small nonstandard size (~3/16"). I tried a 3D printed adapter for that but I don't think the Shapeways "Strong, Flexible" material is strong enough. I'm trying a lasercut acrylic piece next.<br /> <br /> For the older Hario, the situation is much simpler. The drive has a nonstandard shape (cylindrical with flats on two opposite sides) for hand cranking, but it's also threaded as M6-1 so you can just screw something down on top of it. Actually, you can even just clamp the drill adapter directly down on this, as <a href=" did in his original design</a>. I wanted to try something simple and off-the-shelf to avoid clamping directly to the grinder, so I looked for M6-1 coupling nuts that would fit into the 3/8" drill adapter, but unfortunately they are all slightly too large. However, a 1/4-20 coupling nut fits snugly into the adapter (you may have to test a few to find one with clearance) and screws smoothly onto the grinder drive for the first few turns. I don't have any MechE, so can't really tell you the relative benefit of either clamping directly to the drive or screwing an unmatched thread onto it, but as long as I include some washers so that the 1/4-20 is only riding the M6 threads a little way I think this might be a little nicer. An M6@1/4-20 adapter nut or just an M6 with a smaller outer diameter would be ideal but I haven't found such a product.<br /> <br /> <img border="0" height="320" src="; width="240" /><br /> <br /> The <a href="2012/04/kitchenaid-drill-adapter.html">previous hub coupling</a> has already worn out, so it's still not clear whether these 3D printed parts are viable. The first thing to go was the set screw holes; on the failed piece the screws get pushed back through their threads so the coupling no longer grips the 3/8" shaft. But I think the mistake there was to tap the holes and use nylon set screws before switching to steel set screws. On the new piece I just screwed steel set screws through the untapped hole so the fit is tighter and it seems to be holding up better so far. For my general musings on Matlab concurrency and Theron, see the <a href="2011/09/simpler-concurrent-matlab-programming.html">previous post</a>.</em> <h2>Building multithreaded Matlab mex files with Theron</h2> <p>This was pretty straightforward on the Mac, somewhat tricky but not terrible on Linux. I don't have any working C++ compiler with Matlab on Windows thanks to absolutely abysmal support from Microsoft and Mathworks (essentially Mathworks only supports Microsoft and Microsoft's compilers are impossible to get installed correctly). But the notes below may help you get things working on Windows anyway and I added some Windows specific notes at the very bottom. Before trying to get Theron working, I encourage you to write a simple C++ mex program (maybe a single-threaded version of your planned algorithm) and get it built on Matlab first, to confirm that your system is correctly set up for mex.</p> <p>I'm fairly confident most Mac users can get this working. On Linux I have only tested on 2010 and 2011 64-bit versions of Matlab, and there are reasons to fear that earlier versions may have more trouble working. Fair warning.</p> <a name='more'></a> <h2>Building Theron on Mac or other static mex environments</h2> <p>For this discussion, the fundamental difference between Mac and Linux versions of Matlab is how they handle mex compilation. When you mex a C++ source file on the Mac (confirmed up to Matlab 7.12), the system generates statically linked output (a statically linked executable I think, but it could be a static library; it doesn't really matter). On Linux, mex generates a shared library instead, which complicates things significantly when your build depends on the boost_thread library (see below).</p> <p>For now, let's assume you are on a Mac or some other Matlab system that generates static mex files. One way to confirm this is to run mex on something simple with the -v option; this will show the gcc or g++ commands that mex is using. As long as the linking step doesn't include the -shared flag, you're okay.</p> <p>In this case, your first step is to follow the getting started instructions on the <a href=" site</a>: download Theron and build it, linking against your choice of Boost threads library. If you're not sure whether you have Boost threads installed already, you can try the <tt>locate libboost_thread</tt> command, possibly after running <tt>sudo updatedb</tt> if your locate database is out of date. If you don't have Boost threads already, the simplest way to get it is to use Macports. Once you have Macports installed, it's as simple as: <tt>sudo port install boost</tt>. Currently this gives you a very up-to-date boost version 1.47.</p> <p>Theron is currently a little Windows-centric, so you'll need to tweak the makefile a little to get it to build. Theron author Ashton Mason tells me the next release should make things smoother for Gnu (non-Windows) people. First, as detailed in the Theron guide, you need to edit the makefile to point to your Boost installation. If you got Boost from Macports, this will probably look like this:</p> <pre class="brush: bash" name="code"> BOOST_INCLUDE_PATH = /opt/local/include/ BOOST_LIB_PATH = /opt/local/lib/ BOOST_RELEASE_LIB = boost_thread-mt BOOST_DEBUG_LIB = boost_thread-mt </pre> <p>If you want to be able to run <tt>make clean</tt> to start over with the build process, I recommend also changing the following lines:</p> <pre class="brush: bash" name="code">RM = rm -f ... clean: ${RM} ${BUILD}$/*.o ${RM} ${BUILD}$/*.a ${RM} ${BUILD}$/*.ilk ${RM} ${BIN}$/*.pdb ${RM} ${BIN}$/*.ilk ${RM} ${BIN}$/*.exe ${RM} ${LIB}$/*.a ${RM} ${LIB}$/*.lib </pre> <p>I think that should be it, so go ahead and build. If you are only planning to use Theron from Matlab you only need the library, so I would use <tt>make library mode=release</tt>.</p> <p>If all goes well, your build should finish pretty fast and you should now have the <tt>libtheron.a</tt> library in your <tt>THERONPATH/Lib</tt> folder. Now boot up Matlab and try to build a simple mex file. To compile this from Matlab, you'll need a little more than a simple <tt>mex theron_test.cpp</tt>.</p> <p>Do not put spaces in any of these mex options; no space after the "D". First you need to tell the compiler what Theron function calls look like. You do this by pointing it to the Theron header files with the <tt>-I/THERONPATH/Include/</tt> directive.</p> <p>Next you need to tell mex to pull in the theron and boost_thread libraries. The standard way to do this is with the -L and -l options to the linker, but I find it cleaner to just include the full paths to the library files after your C++ file name; this tells the linker to look through the object files in the libraries when resolving the calls to Theron in your code. I also like using the -v option so I can see exactly what mex is up to.</p> <p>So your full build command should look something like:</p> <pre name="code">mex -v -I/THERONPATH/Include/ theron_test.cpp /THERONPATH/Lib/libtheron.a /LIBPATH/libboost_thread.so </pre> <p>If you got Boost from Macports, then the libboost part above should be <tt>/opt/local/include/libboost_thread-mt.dylib</tt>. You could also use <tt>libboost_thread-my.a</tt> if you want; I don't think it matters much.</p> <p>Did it build? Hope so... Now try running <tt>theron_test</tt> from the Matlab command line. You should see nothing; nothing is good. Now go ahead and code up your algorithm using the Theron tutorial as a guide and enjoy simple Matlab concurrency!</p> <h2>Building Theron on Linux or other shared library mex environments</h2> <p>The process is a bit more complicated for Linux users. The upside is, you probably don't have to worry about getting your own Boost implementation (although chances are you already have one...). The issue is that in Linux, Matlab mex files are built as shared libraries instead of statically linked. This causes two problems. The immediate problem is that Matlab needs you to build Theron with the <tt>-fPIC</tt> option turned on so that it can use the Theron library in a shared build. This is not a big problem; just add <tt>-fPIC</tt> to the <tt>CFLAGS</tt> in the Theron makefile.</p> <p>The second problem is the deal breaker: Matlab uses libboost_thread internally and has its own copy hidden away in its internal distribution folders. Since your mex file got built as a shared library, it will try to pull the boost_thread library in at runtime, and it will find Matlab's version (1_40_0) instead of your external version (1_42_0 currently in apt-get). This causes a segfault.</p> <p>So there are two approaches to solving this. The better one would be to find some way to tell your mex file not to use Matlab's boost_thread. I found <a href=" discussion of this on MatlabCentral</a>, but did not get this working; it seems like it might require rewriting parts of Theron to use <tt>dlopen</tt>; not very nice. I still feel like there should be some other way to get Matlab to behave, but I didn't find it. (For completeness: it does not work to point Matlab's internal boost_thread to an updated one; duh why did I even try that?)</p> <p>So there is option 2: throw in the towel and build Theron against Matlab's internal boost_thread. This works at least for 64-bit Matlab 2010 or 2011, which has boost_thread 1.40.0 internally. I don't know that it will work for earlier or 32-bit versions of Matlab, which I believe use earlier versions. (A very old version of Matlab might not have this issue at all, as I think it was around 2007 that MathWorks started multithreading Matlab internal code in the first place. If you're using a very old Matlab, check the internal distribution folders for libboost_thread to see if it's there. <tt>locate</tt> should be your friend.)</p> <p>Building Theron against Matlab's internal boost_thread is not too hard. The only trick is that Matlab doesn't have the header files in the distribution (at least I didn't find them). So you'll have to <a href=" boost_1_40_0</a> (or whatever version Matlab has internally on your system) and point Theron to those headers. If you use the wrong headers, you'll get segfaults. Theron's makefile should look something like:</p> <pre class="brush: bash" name="code">BOOST_INCLUDE_PATH = /Downloads/boost_1_40_0/ BOOST_LIB_PATH = /usr/local/MATLAB/R2011a/bin/glnxa64/ BOOST_RELEASE_LIB = boost_thread BOOST_DEBUG_LIB = boost_thread ... CFLAGS += -c -Wall -fPIC </pre> <p>(The libboost_thread in the Matlab folder is only the versioned filename, not the soname, so I would have thought you might need to symlink a soname but that didn't seem to be necessary.)</p> <p>If this builds correctly, you should be good to build your mex file from Matlab; the command is the same as above for Mac, except you point it to the internal Matlab boost_thread:</p> <pre class="matlab" name="code">mex -v -I/THERONPATH/Include/ theron_test.cpp /THERONPATH/Lib/libtheron.a /MATLABLIBPATH/libboost_thread.so.1.40 </pre> <h2>Conclusions</h2> <p>This was a little bit of a pain, mostly because of Matlab's questionable decision to use shared libraries internally without (apparently) providing for situations where users build their mex files against the same libraries. The process is especially annoying if you have Mac and Linux machines that both need to be able to build your mex. Some steps would be a little smoother if Theron were less Windows-centric; Theron has a new release coming that should improve on this.</p> <p>I think the current solution is stable, although it's tricky to be locked into the Matlab internal boost_thread version. The benefit is clear. Using Theron to multithread a simple algorithm, I got about 4.5x speedup using 6 cores, and 6x speedup using 12 threads on 6 cores with hyperthreading. This was a simple algorithm that didn't really require an Actors model, but it was great that I could get this done quickly (once I figured out the Matlab setup tricks) and without having to deal with low-level threads and mutexes at all.</p> <h2>Building Theron on Windows</h2> <p>As noted, I have not gotten any Windows compiler to work with mex, so these are purely hypothetical guidelines. If you are building on Windows you will probably find it simpler to use the Windows thread library instead of Boost. So just leave out the THERON_USE_BOOST_THREADS define and it should revert to using Windows threads instead. This means you should be safe from the internal Matlab shared library conflict problem that Linux has. For a specific guide to using the Theron C++ library for Actors concurrency in Matlab, see <a href="2011/09/preamble-what-follows-is-guide.html">the following post</a>.</em></p> <h2>Concurrent Matlab</h2> <p>Matlab is a standard programming environment for scientific calculations. In our lab, we use it for a lot of analysis, especially prototyping new analyses. Eventually, some of the performance critical stuff gets ported over to Java, but we still have plenty of things we need to run that are Matlab-only and need to be fast.</p> <p>As personal computers add more and more cores without improving clock speeds much, there is more and more need for parallelized (AKA concurrent, multithreaded) software. Matlab is not a very popular language for supercomputing, but high-performance clusters also of course demand parallelized software. Matlab has some support for multithreading internally, so depending on which Matlab internal functions you use, you may see significant multicore use on your machine. But there is currently no support for explicitly writing multithreaded programs in vanilla Matlab.</p> <a name='more'></a> <p>One solution is to start multiple instances of Matlab and use communication abstractions like sockets (also not supported in vanilla Matlab) or the file system to interact between the processes. This is an okay solution for coarsely (AKA embarrassingly) parallel algorithms. Several nicely abstracted solutions exist. Matlab's own <a href=" Computing Toolbox</a> with its <tt>parfor</tt> construct for example. Free solutions include the <a href=" package from MatlabCentral FileExchange</a>, and <a href=" href="; from MIT. Multicore and pMatlab both use the file system for interprocess communication. (They have somewhat different abstraction models that make them appropriate for somewhat different styles of multithreading.) One upside to using the file system for interprocess multithreading is that it is trivial to use a network file system to make your code not only multiprocess but actually distributed to multiple machines.</p> <p>But what if you need more finely grained parallelism, for example you want to code algorithms that flexibly break a problem into hundreds of sub-tasks and maximally exploit available hardware concurrency? At present, this seems to require you to either give up on Matlab entirely, or else write lower-level code that links into Matlab. The two methods I'm familiar with for linking lower-level code into Matlab are to write Java, which Matlab communicates with fairly smoothly, or else write C/C++/Fortran and bring it in with Matlab's "mex" functionality.</p> <p>Java core concurrency is established, but based around low level threads and locks. It's a minefield of synchronization issues. There are libraries available for more abstracted approaches, but I haven't worked with them. And if a contained segment of code is <em>really</em> performance critical, writing it in native C/C++ makes sense.</p> <p>So what are the capabilities for writing concurrent C++ and linking it into Matlab? Unfortunately current C++ compilers do not have native concurrency support. But this is expected to change in the not-too-distant future. C++11, the new standard, was approved last month with a new thread library based on the threading in the popular Boost distribution. Unfortunately, the C++11 threading standard looks a lot like Java core concurrency, built around low-level threads and mutexes with no support for higher-level, safer, simpler concurrency abstractions. Luckily, there are libraries available offering concurrency abstractions for C++. The one I'd like to highlight today is <a href="</a>.</p> <h2>Theron</h2> <p>Theron is a concurrency library for C++ based on the Actors model, in which most of the low-level thread/mutex concurrency implementation details are hidden from the user. It is not a strict Actors model based tightly on the academic definition of Actors, but it is in that spirit. It offers an Actors abstraction based on a backing thread pool, so that creating new Actors is cheaper than actually creating new threads, allowing you to trivially have hundreds or thousands of Actors in one program. The pool of worker threads services the Actors efficiently, allowing high exploitation of available hardware concurrency. Actors are also defined to communicate only through message passing and not through shared memory, eliminating most of the booby traps that make low level concurrent programming such a pain. (There are ways to pass shared memory between Theron Actors if you really need to, e.g. if you have large input data structures you don't want to copy into messages. It's then up to the user to ensure that the Actors interact with the shared memory in well defined, safe ways. Nevertheless I have found that the Actors abstraction is very helpful for simplifying concurrent code even if some degree of shared memory is slipped in.)</p> <p>Another nice feature of Theron is that it is designed to allow easy swapping in of different underlying thread models. Currently it has wrappers for native Win32 threading as well as Boost threads. The expectation is that as C++11 std::thread compilers become available, it will be easy to adapt Theron to use the standard threading. Theron also has a very nice website with clear documentation of the simple, powerful API, a clear tutorial, getting started guide, and good user support. Although Theron has historically been somewhat Windows-centric, I was able to download Theron, build it on my work Mac, and get a multithreaded program running in Matlab in about a day. Getting it working in Linux took longer, but I think I have it figured out now too; see next post for details.</p> <h2>Update: libcppa</h2> <p>I recently came across another C++ Actors project, <a href=";. This is much less mature than Theron, and relies on a lot of C++11 features for its core functionality, but it looks interesting. It's hosted on GitHub, so could be easy to dig into despite not a lot of documentation. Longtime ACA proponents such as Sara Rosenbaum, a professor of health law and policy at George Washington University, have come to the realization that the law is in deep trouble. “Even the most ardent proponents of the law would say that it has structural and technical problems that need to be addressed,” she said. “The subsidies were not generous enough. The penalties for not getting insurance were not stiff enough. And we don’t have enough young healthy people in the exchanges.” Frankly, although I take no pleasure in the millions of people who are going to be hurt as a result of the ACA’s implosion, I do have a sense of vindication.  I warned from the outset that, well-motivated or not, the ACA was not designed properly and that it was extremely vulnerable to a death spiral.  I discussed early signs of trouble. And yet people accused me of Chicken Little “The Sky is Falling” syndrome or of not understanding all the brilliant stabilization methods built into the ACA that would let its community rating scheme succeed where so many predecessors had failed. So, the good news is that people can start to focus on ACA 2.0 rather than pretending that ACA 1.0 is working. It will be a huge fight — already the battle lines are being drawn.  My one plea is for discussion to be based on reality and transparency.  No more magical models that predict whatever the politicians want. What’s interesting, though, is what happened at almost exactly the same time as I released my Forbes blog entry.  New CMS Administrator Andrew Slavitt, whom I noted in the article had actually expressed concern about Risk Adjustment,  told Inside Health Policy, an outstanding trade publication, that some changes to the risk adjustment methodology in the draft Notice of Benefit and Payment Parameters for 2017 may instead be implemented in the 2016 plan year. CMS’ proposal calls for the 2017 risk adjustment to use a blended rate from earlier years and account for patient use of preventative services. I know that may not be the sexiest announcement ever made, but it’s important.  I’m not going to pretend that my blog entry motivated Mr. Slavitt to start looking hard at CMS methodology — although he would be well educated if he started each day with The Apothecary.  But, along with his recent actions taken to reduce the ability of insureds to game the Special Enrollment Period, Administrator Slavitt’s critical attention to Risk Adjustment suggest a willingness to take a fresh look at Obamacare implementation failings.  The Affordable Care Act originally appeared to promise a choice of plans on the Exchanges across at least two spectra: the amount of cost sharing an insured would have to assume and the degree of choice the individual would have in selecting their healthcare providers.  Although this ability to customize both choice and “metal tier” was generally considered a feature of Obamacare, it has turned out to pose significant issues.  And here’s why: plans with the greatest degree of choice (PPOs) and the lowest amount of cost sharing (mostly Platinum) are magnets for the unhealthy.  So, unless there’s something to neutralize the extra costs to the insurer created by this “magnetic attraction” or unless insurers can simply decline to offer plans with high choice or low cost sharing, the freedom to select a plan in fact becomes destructive. The choice touted by proponents of Obamacare induces a weakened form of an adverse selection death spiral. The whole system may not immediately collapse, but the system’s physics becomes highly unstable. The plans most attractive to the unhealthy become unavailable. The unavailability occurs either because insurers can’t persuade regulators to let them charge the high rates needed to break even or because all but the sickest insured’s won’t buy them at such a price. The most expensive 20% of individual insureds, after all, cost on average more than 60 times as much per person as the least expensive 50%. The Platinum refugees then migrate to the second most attractive plans —  in our case often Gold or Silver. (Remember Silver plans have low cost sharing for poor families). But then these plans become disproportionately populated by the sick and can also become considerably more expensive.  If there is a point of stability, it is likely to be one in which there is far less choice of physician and far more cost sharing than originally contemplated.  Most people might end up in a Bronze HMO. … The stability of Obamacare likely rests significantly on the arcane and challenging technology of  Risk Adjustment.  Run it poorly in ways insurers can game and look for the market to fall into a Bronze HMO basin of attraction or collapse altogether.  Run it without the strictest safeguards for medical privacy and see a mass rebellion from insureds. Here are a few paragraphs to whet your interest.  To see more, go here. This is about a raid conducted in the murky twilight of the Federal Register. It’s a scheme in which the Obama administration collected less in taxes from health insurers (mostly off the Exchanges) than they were required to do under the Affordable Care Act, created a plan to pay insurers selling policies on the Exchange considerably more than originally projected, and stiffed the United States Treasury on the money it was supposed to receive from the taxes. It’s a different bailout than the Risk Corridors program. That, at least, was originally authorized by statute.  This is about a diversion that took place in spite of a statute that explicitly prohibited it.  And the consequence of the diversion of funds was to enrich insurers and, probably, to keep more insurers selling policies on the Exchanges than would otherwise be the case. … The transitional reinsurance program as implemented, however, has become entirely unmoored from the statute that created it. It has instead embarked on a progressively stranger course in which two of the most recent diversions were  underassessing health insurers to pay for the program and then using the first $2 billion collected not to pay the United States Treasury as called for by the statute but instead to pay off insurers selling individual health insurance policies on the Exchange and, some times, off the Exchange.  Indeed, not only has $2 billion from the 2014 money been diverted from the Treasury to insurers but it looks as if at least an additional $800 million from the 2015 money is heading in the same direction. … By combining the two revisions of the original reinsurance parameters, the Obama administration made the program about 40% juicier for insurers . To the cynical eye, this could be seen as one of several administrative cures for the Obama administration’s politically understandable yet completely illegal decision to starve exchange insurers of potential customers who would now, by administrative fiat, often be permitted to keep those dreadful policies that the Affordable Care Act was supposed to eliminate. With foolish campaign promises as the motivation, one illegality begat another. … And now let’s take a closer look at the Obama administration’s legal justification for shoveling money to insurance companies on whose graces the success of Obamacare rests . You can read it above.  CMS contended that, because the statute was silent or ambiguous; it gave CMS discretion.  According to CMS, the statute used “shall” when it came to the $10 million to be collected for reinsurers in 2014 and used only “reflects” when it came to the $2 million for the Treasury, implying that the collection of money for reinsurers was more mandatory than collection of money for Treasury. Besides, argued CMS, the premium “stabilization” purpose of the ACA would be enhanced by funneling more money to insurance companies. This reading of the statute makes no sense, however. The ambiguity exists only by virtue of ignoring a provision of the statute never even mentioned by CMS its legal analysis. Chandler   ]]> 2656 If United is losing money, what about big insurers who charge less? /2015/11/20/if-united-is-losing-money-what-about-big-insurers-who-charge-less/ Fri, 20 Nov 2015 21:04:34 +0000 /?p=2602 Continue reading If United is losing money, what about big insurers who charge less? →]]> Earlier this week, United Healthcare, the nation’s largest health insurers, told its shareholders that it would be pulling back on efforts to market its Obamacare plans for 2016 and might pull out of the market in 2017.   Eternal optimists about Obamacare have been quick to note that United, although a large insurer generally, “only” covers about 540,000 people on the Exchange.  This is about 6% of those who enrolled in 2015.  In fact, however, United’s announcement should be a cause for great concern, not just for the half million plus,  but for a large number of people purchasing coverage from other insurers who likewise may become intolerant of continued financial losses.  To quote Katherine Hempstead,who heads the insurance coverage team at the Robert Wood Johnson Foundation,  “If [United] can’t make money on the exchanges, it seems it would be hard for anyone.” This entry is about a little experiment.  Although United may not be one of the biggest players on the Exchanges, the various Blue Cross insurers most definitely are.  And so, what does United’s situation suggest about the vulnerability of Blue Cross insurers? It turns out that there are 38 markets for which we have data on which Blue Cross and United compete for Silver plans, the most common metal level purchased.  By market, I mean they both sell the same kind of plan type (HMO, EPO, POS, PPO) in the same rating area of a state.  (This looks only at states that use healthcare.gov and thus have their data made available at data.healthcare.gov). So, if Blue Cross sells at least one HMO plan in Georgia Rating Area 7, and a United company likewise sells at least one HMO product in Georgia Rating Area 7, I treat the two insurers as competing against each other.  And, if Blue Cross’s premiums tend to be lower than United’s in the competing markets and United is losing money, that might be some evidence that Blue Cross is vulnerable. The chart below shows the result of the experiment.  I calculated the median silver premium for Blue Cross companies in the 38 competing markets and the median silver premium for United companies.  I then divided the Blue Cross median by the United median.  I used a 40 year old adult individual as the basis for the computation, although prior research shows that little turns on the choice of age. The chart shows a histogram of the results. What we can see is that the histogram is skewed to the left: on balance, Blue Cross tends to charge less than United.  Sometimes, as in three rating areas in Kansas and two ratings areas in South Carolina, it charges less than 3/4 of what United charges. Does this prove that many Blue Cross insurers place themselves in financial jeopardy by continuing to sell?  Hardly.  There is a lot more to profitability than premiums even when metal tiers and plan types are the same.  Blue Cross may have struck better deals with medical providers.  Blue Cross may run a more efficient operation.  Blue Cross may be able to attract a healthier risk pool.  Blue Cross is raising its rates.  Claims experience in 2016 may be different than experience in 2015.  And, in some markets, Blue Cross charges more.  In Virginia Rating Area 10,  Blue Cross’s median is 1.27 times that of United’s. I get it. But still. I’d feel a lot better about other insurers’s prospects if they tended to charge more than one that was encountering financial difficulties.  And, yes, perhaps the ACA can shrug off the possible departure of United.  We can trot out the words “blip” and “outlier.”  But I am not ready to join some academics and advocates in the eternal Obamacare sunshine with a chorus of “nothing to see here.”  Maybe there’s not much difference between United’s situation and that of other large insurers. Maybe, it’s just that United’s directors and officers found the courage to sound the warning sooner. ]]> 2602 Bombshell: United Healthcare thinking of exiting Obamacare /2015/11/19/bombshell-united-healthcare-thinking-of-exiting-obamacare/ Thu, 19 Nov 2015 18:26:26 +0000 /?p=2594 Continue reading Bombshell: United Healthcare thinking of exiting Obamacare →]]> Today’s news that United Healthcare is “evaluating the viability of the insurance exchange product segment and will determine during the first half of 2016 to what extent it can continue to serve the public exchange markets in 2017” is a very big deal.  United would not be disclosing to shareholders that it might fully or significantly exit Obamacare in 2017 if this was not a significant possibility. United is a major player in the Exchange markets.  It sells policies in about 47% of the 395 rating areas serviced by the federal exchange. Moreover, the loss of United could be very harmful to any remaining competition of the exchange markets.  A quick study of data from healthcare.gov shows that if one looks at Silver plans in rating areas in which United sells a policy and one looks at all plan types (HMO, EPO, POS,PPO), there are 204 combinations.  In 73 of those (about 36%), United is the only insurer, meaning that if no one else steps in to the United vacuum, there will no longer be a seller of that plan type.  HMO plans in Alabama rating area 13 is an example of such a market. If United exits, it would appear that there will be no HMOs in that area. In another 59 of those 204  (about 29%) rating area/plan type markets in which United participates, United is one of only two players.  An example of such a market is the POS market in  Arkansas, rating area 1. There, UnitedHealthcare of Arkansas, Inc. and QCA HealthPlan are the only sellers. This means that if no one else steps in, there will be  another large chunk of markets in which there will be an Obamacare monopoly. Moreover, the problems United is evidently facing do not appear to be the result of particularly low prices.  The graphic below shows for each of the 204 market-plan type combinations in which United is present,  the ratio of United’s  its median price for policies to the median of all prices. It shows United prices tend to be fairly close to the median and, if anything, tend to be a bit higher.  Thus it will be a challenge to ascribe United’s failures to any sort of extreme pricing — a fact suggesting that either United had problems on the cost side or simply that it is now facing up to a fact that some other large insurers may wish to deny: Obamacare is in trouble. The ACA simply does not work without voluntary insurer participation.  There is no public option and their closest cousin, the coops are mostly dead or in financial distress.  It surely should work better if there is at least some competition.  But insurers don’t voluntarily participate where they think they can’t make money.  So, unless United, one of the biggest health insurance carriers, is doing something particularly wrong or has unduly gloomy management, one has to worry about its warning being an oracle of things to come for other insurers. Note In contrast to my usual practice, I have revised this entry fairly heavily during the course of the day.  I think it is important to get the information out there and I hope readers excuse some instability in the entry. ]]> 2594 Prices rising, choice declining for 2016 Obamacare /2015/10/31/prices-rising-choice-declining-for-2016-obamacare/ Sat, 31 Oct 2015 19:04:36 +0000 /?p=2554 Continue reading Prices rising, choice declining for 2016 Obamacare →]]> Data released yesterday at healthcare.gov shows the beginnings of an adverse selection death spiral that threatens the stability of the system of insurance created by the Affordable Care Act.  The data shows that, on plans using the “federally facilitated marketplace” created under the ACA, PPO plans that continued from 2015 to 2016 increased gross premiums an average of 16% and Gold and Platinum plans increased 15% and 21% respectively.  HMO plans, by contrast, increased a lesser 8% and Bronze and Silver Plans increased a lesser 12% and 9% respectively.  We should thus expect to see in 2016 relatively fewer people purchasing plans that give them a greater choice in physicians or that provide greater protection against medical expenses. The tables below summarize the big picture.  The first table shows the mean change in gross premiums between 2015 and 2016 for plans that persisted over that timespan when grouped by metal level.  As one can see the more generous Gold and Platinum plans increased at rates considerably higher than the less generous Catastrophic, Bronze and Silver plans. One might say that this development is not so awful since it leaves in place a market for more basic plans: HMO plans for example or silver and gold plans.  Perhaps the government should not be subsidizing individual’s choice of doctors or fostering plans, such as platinum plans, that fail to deter excess medical consumption.  Such is not, however, the promise of the ACA or, I suspect, the desires of many of its proponents. Moreover, we are in a dynamic situation.  Think about next year when the insurer subsidies are supposed to disappear and when the chronically ill people who were in platinum and/or PPO plans migrate into the next best thing, a gold plan or, if one is available, a POS or EPO plan.  Suddenly those plans become vulnerable to adverse selection pressures.  And for 2017 we might thus expect to see yet further shrinkage of PPO and platinum plans and greater pressures on everything but the basic Bronze and Silver HMO plans.  When that happens, the adverse selection death spiral will not only start biting wealthier purchases or those with chronic conditions, but mainstream America. Private health insurance is fragile. It generally does not well withstand the sort of underwriting regulation imposed by the ACA.  The conceit of the ACA proponents was that they had engineered a system — the “three legged stool” so strong that it could resist the almost invariable pressures of adverse selection.  If I am right, and regardless what one thinks about the motives of those proponents, we are beginning to see that the engineering was just not good enough. Caveats and further research The computations shown above are based on the number of plans and not weighted by the number of enrollees.  This is largely of necessity since the federal government has not been releasing enrollment figure by plan in a clear way (although it may be possible to tease the figures out of rate review submissions filed and collated on healthcare.gov).  Although enrollment weighting will likely decrease the average mean premium (less expensive policies tend to be purchased more), it is not clear that enrollment weighting will have much effect on relative premium increases. The figures are also not computed yet on a state-by-state basis, something that I hope to present in a later post.  They also contain only data for states whose plans are described in material available at healthcare.gov.  Data for states such as California and New York, which have their own exchanges, is not included here and might alter the numbers somewhat. Finally, I present gross premiums here; as I have discussed at length elsewhere, net premium increases may well be higher, particularly where the purchaser wishes to retain a gold or platinum plan or a PPO plan whose premiums are rising even faster than those of the silver plans and the second lowest silver plan. The situation is worst where, due to some willingness on the part of a new entrant to take risk,  the second lowest silver plan drops in price, thereby decreasing subsidy levels, but other silver, gold and platinum plans increase in price. Not anymore in Obamacare’s Houston →]]> The data is not fully out yet, but, if my home area of Houston, Texas (Harris County) is representative, PPO plans that offer the greatest choice of doctors and that provide low cost sharing are extinct, as are POS plans that also offer more choice of medical practitioners. Platinum plans of any sort are on their way to its extinction. In 2015, there were 19 PPOs available in Harris County, 12 from Blue Cross Blue Shield and 7 from Cigna Healthcare.  In 2016, according to the preliminary data available on healthcare.gov and released Sunday, there are none.   Nor does the matter improve my considering POS plans, which also offer a greater degree of choice of doctor than does an HMO.  In 2015 there were17 such plans in Harris County, 10 from Aetna and 7 from Humana.   Those are gone too in 2016.  So, basically, it is no longer true in Harris County that you have a choice of doctor if you purchase an Obamacare plan.  You get what the HMO or EPO gives you. Platinum plans are now almost extinct. In 2015, there were three platinum plans available in Harris County, an HMO and POS offered by Humana and and EPO offered by United Healthcare.  According to the preliminary information released Sunday, only the Humana HMO survives. Thus, you can get a plan that has minimal cost sharing, but no longer one that offers great choice of medical practitioners. The Humana POS and the United EPO are gone. Notice what’s missing from the list of plan types? And it’s going to cost you a lot to put yourself in a pool in which cost sharing is low.  In 2015, the gross premium for the Humana Platinum HMO (32673TX0640030) was $448 for an individual age 40 (non-smoker). In 2016, the gross premium for the same Humana plan was $551, an increase in gross premiums of 23%. Net premium increases — the thing the insured actually pays — are likely to rise a similar amount for the one remaining platinum plan.  The second lowest silver plan — the baseline for computation of subsidies — has increased in price by $34, from $222 in 2015 to $256 in 2016.  Consider an individual eligible for a $150 subsidy in 2015.  If they purchased the Humana Platinum HMO in 2015, their net price would be $298.  If they purchased the same policy in 2016, yes, their subsidy might grow by $34 but their net price would still be $367, an increase of 23%. Os países signatários da convenção se comprometem a consultar os povos interessados quando forem previstas medidas legislativas ou administrativas que os afetem diretamente, garantindo a efetiva participação dos povos indígenas e tribais na tomada de decisões. Nós, do Movimento Indígena Revolucionário, denunciamos, na ocasião, que a Convenção 169 da OIT está sendo completamente desrespeitada pelo Governo Federal e pelas empresas privadas. Nós denunciamos que a Convenção não foi aplicada devidamente desde sua ratificação até os dias de hoje, mantendo impunes todas as violações cometidas contra as nações indígenas. Começou uma longa, duradoura e profunda dor por terem sido prisioneiros, com inúmeras vítimas, exterminação de aldeias, proibidos de falarem a sua língua, de morar juntos nas suas malocas (separaram em cada casa todo o povo indígena), proibidos de realizar seus próprios rituais xamânicos (desrespeito aos pajés, cânticos, mitos, sendo assim condenados por 45 anos) com sessões de torturas, espancamentos, genocídio, etnocídio até que os revolucionários Carlos Tukano junto aos seus irmãos Pedro Machado (hoje no hospital, em estado grave de saúde) e Álvaro Tukano e todo o seu povo dispuseram as suas próprias vidas em grito de liberdade, libertando seu povo de 45 anos dos miseráveis capuchinhos, salesianos e da Ação Católica.
