Treatment-resistant and multi-therapy-resistant criteria for bipolar depression: consensus definition.

Br J Psychiatry. 2019 01;214(1):27-35

Authors: Hidalgo-Mazzei D, Berk M, Cipriani A, Cleare AJ, Florio AD, Dietch D, Geddes JR, Goodwin GM, Grunze H, Hayes JF, Jones I, Kasper S, Macritchie K, McAllister-Williams RH, Morriss R, Nayrouz S, Pappa S, Soares JC, Smith DJ, Suppes T, Talbot P, Vieta E, Watson S, Yatham LN, Young AH, Stokes PRA

BACKGROUND: Most people with bipolar disorder spend a significant percentage of their lifetime experiencing either subsyndromal depressive symptoms or major depressive episodes, which contribute greatly to the high levels of disability and mortality associated with the disorder. Despite the importance of bipolar depression, there are only a small number of recognised treatment options available. Consecutive treatment failures can quickly exhaust these options leading to treatment-resistant bipolar depression (TRBD). Remarkably few studies have evaluated TRBD and those available lack a comprehensive definition of multi-therapy-resistant bipolar depression (MTRBD).AimsTo reach consensus regarding threshold definitions criteria for TRBD and MTRBD.
METHOD: Based on the evidence of standard treatments available in the latest bipolar disorder treatment guidelines, TRBD and MTRBD criteria were agreed by a representative panel of bipolar disorder experts using a modified Delphi method.
RESULTS: TRBD criteria in bipolar depression was defined as failure to reach sustained symptomatic remission for 8 consecutive weeks after two different treatment trials, at adequate therapeutic doses, with at least two recommended monotherapy treatments or at least one monotherapy treatment and another combination treatment. MTRBD included the same initial definition as TRBD, with the addition of failure of at least one trial with an antidepressant, a psychological treatment and a course of electroconvulsive therapy.
CONCLUSIONS: The proposed TRBD and MTRBD criteria may provide an important signpost to help clinicians, researchers and stakeholders in judging how and when to consider new non-standard treatments. However, some challenging diagnostic and therapeutic issues were identified in the consensus process that need further evaluation and research.Declaration of interestIn the past 3 years, M.B. has received grant/research support from the NIH, Cooperative Research Centre, Simons Autism Foundation, Cancer Council of Victoria, Stanley Medical Research Foundation, MBF, NHMRC, Beyond Blue, Rotary Health, Geelong Medical Research Foundation, Bristol Myers Squibb, Eli Lilly, Glaxo SmithKline, Meat and Livestock Board, Organon, Novartis, Mayne Pharma, Servier, Woolworths, Avant and the Harry Windsor Foundation, has been a speaker for Astra Zeneca, Bristol Myers Squibb, Eli Lilly, Glaxo SmithKline, Janssen Cilag, Lundbeck, Merck, Pfizer, Sanofi Synthelabo, Servier, Solvay and Wyeth and served as a consultant to Allergan, Astra Zeneca, Bioadvantex, Bionomics, Collaborative Medicinal Development, Eli Lilly, Grunbiotics, Glaxo SmithKline, Janssen Cilag, LivaNova, Lundbeck, Merck, Mylan, Otsuka, Pfizer and Servier.

The Civil List and the three Grants-in-Aid.

Prince Harry hopes for calmer future, but not much chance

LONDON – Prince Harry says he’s taking a “leap of faith’’ as he steps back from royal duties in an attempt to build a more peaceful life – one free of the journalists who have filmed, photographed and written about him since the day he was born.

Fat chance.

As Harry reportedly flew out of Britain on Monday to be reunited with his wife, Meghan, in Canada, many predicted the prince and the former TV star would struggle to escape global fame and its pressures.

“They believe that if they are not representing the monarchy any longer, the tabloid press will eventually go away because it will be so expensive for them – that there won’t be the same savage approach. They feel they will be able to control it more from Canada,’’ said Pauline Maclaran, a business professor at Royal Holloway University of London and author of “Royal Fever: The British Monarchy in Consumer Culture.’’

”I don’t think they are being very realistic.’’

Harry has long had a frosty relationship with the media, and on Sunday he made it clear that the press was a major reason for the couple’s decision to step back from royal life. In a personal speech that referenced his mother, Princess Diana, who died in a car accident in 1997 while being pursued by paparazzi, he said he had “no other option’’ but to step away.

Harry has accused the media of directing “a wave of abuse and harassment” at the biracial Meghan, including “racial undertones” in articles. Both he and Meghan filed lawsuits last year against press outlets over alleged intrusion into their private lives. At the time, Harry gave an interview drawing parallels between the treatment of his wife and the media frenzy that contributed to the death of his mother.

“When I lost my mum 23 years ago, you took me under your wing,’’ Harry said Sunday at a London dinner for Sentebale, his Africa-based charity supporting youngsters affected by HIV. “You looked out for me for so long, but the media is a powerful force. And my hope is one day our collective support for each other can be more powerful, because this is so much bigger than just us.’’

The comments were Harry’s first since Saturday night, when his grandmother, Queen Elizabeth II, announced the terms under which the prince and his wife will walk away from most royal duties, give up public funding and try to become financially independent. The couple, who were named the Duke and Duchess of Sussex on their wedding day, are expected to spend most of their time in Canada while maintaining a home in England near Windsor Castle.

Meghan has already returned to Canada, where the couple spent a Christmas break with their 8-month-old son, Archie. Harry continued in his royal role Monday, attending a U.K.-Africa investment summit in London alongside British Prime Minister Boris Johnson.

But he did not appear alongside brother Prince William at an evening reception for African leaders. the BBC and other media outlets reported that he was on a plane bound for the west coast of Canada. They spent the holiday season on Vancouver Island, while Meghan worked for seven years in Toronto where she filmed the TV series “Suits.”

Experts on branding and the royals believe the golden couple will, if anything, get even more attention than they do now. Though the couple have expressed disdain for the royal rota – a pool system that gives British journalists access to royal events – the system also served to control media demands. Now, all bets are off.

“The paparazzi will follow them everywhere they go,’’ said Jonathan Shalit, a celebrity agent and chairman of London-based InterTalent Rights Group. “A picture of Meghan going to the shops in Vancouver will have value. I certainly think the kind of attention going forward is going to be harder to control.’’

And if they want to be financially independent, they won’t be able to avoid the media altogether, because clients who pay for their services will want the publicity that comes with a connection to them, Shalit said as he predicted the couple could become a billion-dollar brand. They can expect lucrative deals, such as multimillion-dollar book packages and speaking fees, he said.

Shalit said the Sussexes would outstrip former President Barack Obama and his wife, Michelle, in earning power.

“They are 100% more valuable than the Obamas,’’ he said. “The Obamas aren’t royal. They are.”

Media companies are likely partners. Ted Sarandos, the chief content officer of streaming giant Netflix, told Britain’s Press Association he would be interested in working with them, adding: “Who wouldn’t be interested?’’

The queen’s statement on Saturday said the agreement, reached after crisis talks among the top royals and their staff, was a “constructive and supportive way forward.”

Under the terms of the deal, Harry and Meghan will no longer receive public funds and they won’t use their “royal highness” titles once they stop performing royal duties – although they will formally retain them. They will no longer represent the queen, and Harry must relinquish his honorary military appointments, including his role as Captain General of the Royal Marines.

Harry’s speech made it clear that he and Meghan were disappointed that they won’t be allowed to carry on with some royal duties as they seek to cut the palace’s purse strings.

“Our hope was to continue serving the queen, the Commonwealth and my military associations, but without public funding. Unfortunately, that wasn’t possible,” he said.

The conditions represent a starker break with the monarchy than Harry and Meghan had envisioned earlier this month when they first announced that they planned to give up full-time royal duties.

“It brings me great sadness that it has come to this,’’ Harry said Sunday.

“The decision that I have made for my wife and I to step back is not one I made lightly,’’ he said. ”It was so many months of talks after so many years of challenges. And I know I haven’t always got it right, but as far as this goes, there really was no other option.”

Harry praised his grandmother, the queen, and the rest of his family for supporting him and his wife in recent months. The decision to change both jobs and continents is “a leap of faith,” he said, adding that he hoped the move would allow him and his family to achieve a “more peaceful life.”

“I will always have the utmost respect for my grandmother – my commander in chief – and I’m incredibly grateful to her and the rest of my family for the support they have shown Meghan and I over the last few months,” he said.

Meghan And Harry Left With . . . What, Exactly? Huh?

Meghan and Harry in a recent photo. Note that she looks thrilled. Him, not so much.

At first blush, the news about Harry and Meghan is bleak.
No royal title, no royal duties, no military appointments, no tours, most of their time spent in Canada, no public money.
And they have to repay  nearly two-and-a-half million bucks (Or is it pounds?) that was spent to refurbish Frogmore "cottage" for them.
It's quite a clean break.
But they brought them on themselves.
We do fear this was a dreadful mismatch from the start, folks.
Harry suffered a  traumatic event in childhood at a very impressionable age. Arguably, he never got over it. He's already admitted to pretty serious mental issues including a decades long bout with depression and anxiety for which he has sought psychiatric help.
Meghan Markle has her own issues. The older and already married and divorced Meghan is estranged from most of the members of her own family except for her mother. She has a checkered background, to say the least, and never appeared to be at ease in her new role as a member of the Royal Family. But one could also argue that she never really gave it a chance.
Did Harry seek the company of an older woman because he has mommy issues?
Was Meghan looking for an easy exit ramp from her acting career?
Did Harry view the chance to marry Meghan as an act of rebellion consistent with his mother's separation from the Royal Family?
Was Harry nothing more than Meghan's way of rebounding from her first failed marriage? Was that what it was all about?
Does he now regret falling into her clutches?
Has she been manipulating him all along?
Will the marriage last?
No one really knows the answers to these questions but these two certainly do seem like a couple of flakes. Meghan is known to have embraced fringe progressive causes and they're reportedly being advised by the likes of the Obamas, Loony [George] Clooney, Oprah and Oprah Best Friend Gayle.
Expect Harry and Meghan to be fully commercialized by the summer and right up there with that K clan that everybody's trying to keep up with -- we won't even mention their names but you know who they are.
Don't  be fooled by the "do good" aura that will invariably surround Meghan and Harry and the US media that will fawn over them. That's all a cover up -- fancy window dressing, folks. These two are in it to cash in, Big Time.
They're out to monetize that Sussex Real brand.
They have to, now.
Assuming they stay together, it's really all they've got.
Following are the announcements by the Queen and Buckingham Palace:

From the Palace:
The Duke and Duchess of Sussex are grateful to Her Majesty and the Royal Family for their ongoing support as they embark on the next chapter of their lives.
As agreed in this new arrangement, they understand that they are required to step back from royal duties, including official military appointments.
They will no longer receive public funds for royal duties. With the Queen's blessing, the Sussexes will continue to maintain their private patronages and associations.
While they can no longer formally represent the Queen, the Sussexes have made clear that everything they do will continue to uphold the values of Her Majesty.
The Sussexes will not use their HRH titles as they are no longer working members of the Royal Family. The Duke and Duchess of Sussex have shared their wish to repay Sovereign Grant expenditure for the refurbishment of Frogmore Cottage, which will remain their UK family home.
From the Queen:
Following many months of conversations and more recent discussions, I am pleased that together we have found a constructive and supportive way forward for my grandson and his family. Harry, Meghan and Archie will always be much loved members of my family.
I recognise the challenges they have experienced as a result of intense scrutiny over the last two years and support their wish for a more independent life.
I want to thank them for all their dedicated work across this country, the Commonwealth and beyond, and am particularly proud of how Meghan has so quickly become one of the family.
It is my whole family's hope that today's agreement allows them to start building a happy and peaceful new life.
One final thought: If Meghan thinks she will now live a blissfully peaceful, uninterrupted life fully protected from the bright glare of fame and intense scrutiny, she's sadly mistaken. The real ravaging of her via the the relentlessly pursuit of the paparazzi and their cohorts may have only just begun. And, if Harry thinks he can be as easily estranged from his family as Meghan  has been from hers, he's in for a  real shock. Go back  and check the real story of the  Duke and Duchess of Windsor. If you think THEY  lived  happily ever after, I've got some beans to sell you, Jack!
