Ashok Leyland to enter Russia with a local partner

Chennai: Commercial vehicle major Ashok Leyland Ltd will enter the Russian market with a local partner in a year's time, said a top company official.

He also said the company plans to enter the top 10 global commercial vehicle manufacturer's club soon and betting big on its modular platform.

"We have identified a local assembler in Russia to take our franchise. We will enter the Russian market in next 12 months," Dheeraj G. Hinduja, Chairman, told reporters here on Monday.

According to him, in the new markets, the company will have a local partner as there will be tax concessions.

Hinduja said large distributors in newer markets are now showing interests in taking Ashok Leyland franchise.

He said the company is looking at newer overseas markets like Indonesia, Malaysia and others apart from Russia.

As a measure to look at newer overseas markets, Ashok Leyland has decided to come out with a left hand drive vehicle for all its new models.

Currently, the company exports to Sri Lanka, Bangladesh, Nepal, Middle East, Bhutan, East and West African markets.

He said, Ashok Leyland is faring better during this downturn as compared to earlier downturn in 2013-14.

"We have gained market share in the bus and medium and heavy commercial vehicle's segment. Our market share now is 31 per cent. We are also cutting costs," Hinduja said.

According to him, the company had a debt levels of about Rs 6,000 crore during 2013-14 and it has come down to about Rs 1,800 crore.

Hinduja said, earlier the company was in an investment mode putting money in joint ventures with John Deere, Nissan and others which was draining cash.

He said the company had exited the joint ventures with John Deere and Nissan.

Hinduja said the company saw good potential in the light commercial vehicle (LCV) rolled out by the joint venture with Nissan and bought out the Japanese company's stake in the venture.

Globally LCV's account for about 70 per cent of the market and the severity of the industry down cycle is lesser than that of the medium and heavy vehicles.

According to him, the company will be launching the BS-VI vehicles and the vehicles will be built on modular platform.

Queried about the time frame for Ashok Leyland to enter the top 10 global commercial vehicle makers club, Hinduja said the idea is to enter it soon.

"The last vision was to enter the top 10 medium and heavy commercial vehicle segment. That target was fixed in 2010 and was achieved in 2019," Hinduja said.

"I will be disappointed if we don't achieve this less than last time frame," he added.

He said the company rolled out about two lakh units last year and to achieve the global vision the volume required will be about four lakh units.

Hinduja said LCV's share in achieving that target will also be major.

According to Gopal Mahadevan, Wholetime Director and Chief Financial Officer the company will be focussing on customer solutions business that includes finance, fuel, vehicle performance management systems, maintenance parts, re-sale and driver skilling.

He said over the next decade one-third of industry's profits is expected to come from offering various solutions to customers.

The company's Chief Operating Officer Anuj Kathuria said the transition from rolling out vehicles confirming to BS IV emission norms to BS-VI emission norms will be smooth.

Queried about the costs savings by the company by going for modular vehicle platform having common parts Kathuria said it will be known only after stabilisation of BS-VI sales.

On Monday, the company showcased its BS-VI trucks and buses.

Kashmir: Lobbies at play

There is a gathering chorus of voices at home and abroad demanding expeditious return to normalcy of civil life in the state. It is hoped the Centre and the state administration together are succeeding in effectively neutralising the militants and pro- Pak elements in J&K who had had a free run of the place earlier and who basically provided the raison detre for the Indian move.

The dilemma for the government in Kashmir is that unless the externally aided trouble makers were rendered ineffective, they would continue to keep the Kashmiris under a cloak of fear by carrying out stray acts of violence and prevent normalisation of business, educational and health-related activities.

It is not yet clear if the state administration and police -- the collector and the station house officers in particular -- had started reaching out to the people on the ground to build their morale as free citizens and get them to respond to the calibrated steps of the government to lift prohibitory orders. If local militants could still appear in mohallas and lanes to warn the people to do just their 'minimal' chores and not cooperate in the wider process of normalisation then it is evident that a lot more had to be done.

The army has to focus on eliminating the mujahideen, foreign or local, but the state administration must fulfil its primary responsibility of helping the citizens to feel normal about running their lives. Illicit guns and explosives had to be neutralised through sustained flow of Intelligence from below -- the state Intelligence machinery had to produce more. If Pak agents had to be rounded up even in hundreds and moved out of the state, if necessary, this would be an acceptable part of the process of restoring normalcy.

It is the detention of political leaders of the Valley, however, that has led the Human Rights advocates to set off the false narrative of 'suppression of the people'. The government could consider progressively setting them free of any restraint on the condition that they would not indulge in 'political activity' -- as mandated by the Supreme Court itself. Senior officers of the state should make this position clear to the leaders of the Valley parties and should let the country know about the reasonableness of the government stand.

The handling of the Kashmir situation has a macro perspective and a more important micro dimension -- the former has worked out extremely well but the latter is the ongoing challenge facing the state administration. The district headquarter should become the fulcrum of development and people's security.

Every member of the state set-up must be made to understand that 'security for all' required a 'contribution from all' by way of those in the government acting as the eyes and ears of the democratic state against the doings of anti-national elements. The current state of affairs in J&K arose because the corrupt leaders of Valley parties remained silent over the launch of Kashmir Jehad by Pak ISI, colluded with separatists to stay in power and ignored the agents of Pakistan who organised civic disturbances to destabilise the state.

These leaders never asserted that Kashmir was not a 'Muslim' issue -- a claim made by Pakistan -- since the integral state was home to Hindus, Buddhists and Christians in large numbers. They deliberately brought in Pakistan as a factor in the democratic internal governance of the state simply because this helped to divert attention from their misrule and corruption. Now, under the LG regime, development should be taken to the people and deeper enquiries should be made into the disproportionate assets acquired by the local political leaders, in appropriate cases. Normalcy will return to J&K when its administration will come into full play and the state would be seen as being governed from Srinagar, not Delhi.

As mentioned earlier, India's security and foreign policies have held well in the context of Kashmir -- the Oct 22 proceedings of the US House Committee on Foreign Affairs proving it as much. American acting assistant secretary for South and Central Asian affairs, Alice Wells, told the committee that the US supported the Indian government's stated objectives for scrapping the special status of J&K -- thus affirming that the development was an internal matter of India -- and went on record to make it clear that while a dialogue between India and Pakistan was the most effective way of reducing bilateral tensions this was contingent on Islamabad taking 'sustained and irreversible steps' against terrorists on its soil.

This coordinates with India's policy stand that 'terror and talks could not go together' and turns Pakistan's plea that terrorists were 'non state actors' on its head by questioning that country's failure to chase the militant outfits out of their safe havens, if indeed they did not enjoy any state patronage.

The US concerns for an early return of the Kashmiris to normal day-to-day life represent the objective of India too -- the world community has to be kept updated on the efforts being made by the state and central governments to facilitate that process. Visibility of improvement can be demonstrated in many ways. The decision of the Centre to allow a group of 23 members of European Parliament coming to Delhi to make a trip to J&K counts for this -- the visiting dignitaries expressed full support for India's fight against terror. Militants, meanwhile, made some stray attacks on non-Kashmiri labour working in the state -- not surprising, considering that the latter are soft targets.

India has finally succeeded in getting the world to acknowledge that Pakistan's incessant attempts to use cross-border terrorism in Kashmir to keep the pot boiling there was what had forced the hands of the Modi regime to abrogate Art 370 and take charge of the security and development of J&K as a state of India. Time has come for India to specify the dangerous character of Pak-sponsored terror that relied for motivation not on ideology or an ethnic assertion but on commitment to 'faith' and to impress upon the entire democratic world that the revanchist call of Jehad, which had the potential of producing suicide bombers as an instrument of war, could make the world an unsafe place for a long time to come.

This is the threat of a new kind of terror arising out of the Muslim world and it is necessary to have it countered by way of a declaration of OIC that resort to Jehad was not a legitimate way of solving political disputes in today's world. Unlike a country like the US, India, that houses the second largest Muslim population in the world, is particularly vulnerable to this danger since Pakistan has made Kashmir Jehad a declared war cry against India and is now determined to infiltrate Islamic militants through Bangladesh, Nepal and Sri Lanka routes.

Internal security in India has to watch out against the influences of radicalisation and extend the outreach to concerned families to ward these off. Indian diplomacy must talk of the faith-based terror that the world was being exposed to and mobilise international opinion against it. A grave threat to our national security need not be couched in ambiguous terms for reasons of the need for diplomacy to be 'politically correct' always. India can use the dialogue of think-tanks to firm up the opinion of democratic countries against the enlarging threat of what is clearly an Islam-based terror.

There are still remote places in the mountains of Nepal. Places where the sun must rise high into the sky in order to penetrate the fortresses of rock and ice. Life takes it's hold in these places and spreads it's wings.

The Taz Mahal. Photo taken by Neneng Tarigan.

By: Neneng Tarigan
BSD City, 7 September 2010.

Visiting India especially the Taz Mahal is always in my dream. In my childhood we used to have calendars with pictures about interesting places all over the world. Watching the pictures I got the feeling as if I were living there once in the past, because those pictures seemed so close to my heart. Some pictures were about few places in my country but most of them were about interesting and historical places abroad. I wonder if reincarnation is a true story, but as a Christian I would rather trust, that was only because my heart and soul was extremely possessed by the beauty of the places and the history behind it, that was why I feel so close with the pictures. As it proved, I was lucky to have been visited more than 40 countries in the world.

It was 09.00 pm when I landed at Indira Gandhi International, the new airport of New Delhi, India, on 30th  August 2010. The airport is huge and it has just been operational since early of August 2010. The size maybe equal to Suvarnabhumi airport in Bangkok and the floor was totally covered by wall to wall carpet like Changi airport of Singapore. The security check was so tight, may be it happens after India has a very bad experience with the Mumbai terrorist attack. (Mumbai is the second largest city of India)

Upon arrival, I was fetched up by the Embassy people who then brought me with Toyota old van 1990 to Ramada Hotel which I booked through the internet. It took about 30 minutes from the airport to the hotel and my eyes caught by the glitter of the road studs. I also observed that many fully equipped military guards stand between the airport to the center of New Delhi. Later, I knew that they guarded the city day and night. Incredible!

When I arrived, there were so many cars queuing at the hotel gate, a wedding party of the rich had taken place in the hotel, so I asked the driver to just drop me, so that we would not blocked other cars to get in.

After passing the scanning machines, I went inside then submitted the hotel voucher and asked for the room. Unfortunately however, the receptionist told me that I was not registered in that hotel but to another Ramada Hotel in Gurgaon about 2 hours from Ramada Hotel New Delhi.

I showed them that what was written on the voucher is clearly Ramada Gurgaon New Delhi, and since I had never been to India before, I presumed that the one I booked was totally correct, which is the one that I stand now; I had no idea, that there are two different Ramada hotel in Delhi. (Well, if they assumed that Gurgaon which is 2 hours from Delhi is also in Delhi)

Feeling tired and sleepy, I then asked them whether I can simply move to this hotel instead of staying at the other Ramada in Gurgaon, of course with additional cost I don't mind. Again, according to the hotel management I can't do that, simply because the two Ramada is only the same hotel chain under the same franchise system and name, but different ownership. How incredible! They are very strict with the rule here, the cancellation meant that all of the booking cost for 4 days stay which I have already made with Ramada Gurgaon that cost USD 400.- will automatically deducted from my credit card and gone, if I stay in Ramada New Delhi! (Well, that is the rule)

After long arguments, they advice me to use a hotel taxi to bring me to Ramada Gurgeon. I said: “Okay no problem, how much would the price be?” and then they said:”7000 rupees or about USD 170” I answered:”Well, that will be the problem, just get me another taxi not from this hotel.” They nodded, then I got an outside taxi with the price of only 900 rupees” Can you imagine the price different just in a minute? Well, that was incredible!

After about one hour drive, I then arrived at Ramada Hotel Gurgaon. There the manager said: "You can't cancel your booking". So, I had to stay in that hotel for the 4 nights I booked in the first place. It was already 01.00 am. Feeling tired and disappointed, I could do nothing than to accept their policy. Here I am, in the middle of nowhere; I then called the embassy personnel to inform them the situation and where I am about. Of course the embassy staff offered me the transportation facility, but knowing the distance and the fact that I had already chartered a taxi for pick and drop during I stay in the hotel, I gently refuse the offer and ask them to only fetch and drop me when I was about going back to Indonesia later.

Before sleeping, I asked the hotel manager for the free internet facility as they promoted in the web . We worked on it, and after one hour my laptop get connected with the internet, but with very low signal or meaning no connection at all! Another incredible story.

Gurgaon itself is a new industrialized modern city in the outskirt of New Delhi. It will take only 45 to 90 minutes from Ramada Hotel Gurgaon to Ramada Hotel New Delhi, depends on the traffic. There will be a small metro train station located in front of the Ramada Gurgaon once it is finished in early 2011. The Ramada Gurgaon Hotel is a small 8 stories 3.5 star hotel, built about 2 years ago. It is a very clean hotel and with a very helpful and polite staff too. I had no complain at all about the hotel condition. It is a good hotel to stay, especially for businessmen or investors who have business in Gurgaon; and else the infrastructure facilities in India is good or at least better than in Indonesia, so almost every part of India is reachable.

The next morning, I went to the meeting with the chartered taxi as recommended by the hotel manager. It was a small car with a Nepalis driver named Tapha who only speak a little English. No smile in his face, he seemed full with anger. he didn't even greet me a good morning but just drove and murmured, may be because I was a bit late for our appointment that morning.

Arriving at the toll gate, Tapha asked me to pay 20 rupees for the toll. Getting furious, I sternly said to him: “You know what, why are you so impolite to your costumer? I supposed to pay only 800 rupees for pick and drop, but the manager then told me to add 500 rupees more, for toll , miles, parking and bla bla bla. I have already agreed about that and now with your long face you force me to pay more and more. Why don't you just check to the hotel manager?” He called and checked and then with grim he said: “Okay, okay”

On the way to the meeting, I asked Tapha whether he can stop the taxi at Ramada Hotel Delhi, just to check, whether the meeting will be taken place at Ramada Delhi or Le Meridien. Actually they are at the same direction, and we could pass Ramada Delhi before Le Meridien, however he said: “No, no, no, miles, miles, miles”. “Okay” I said: “no problem, just drop me at Le Meridien”. (Ghost, it was incredible!)

At Le Meridien, I check, whether the venue of the meeting is in their hotel or in Ramada Delhi. They politely check and inform me that, it is not in their hotel, so I phoned the committee and they told me that the venue is at Ramada Delhi.

From Le Meridien I walked about 3 minutes to Ramada Delhi, because the two hotels are very closed by. The problem is, no one at Ramada Delhi knows where the meeting would take place when I asked them at the night when I firstly arrived where the meeting would be. (Another incredible story!)

In the afternoon, I called the manager of Ramada Gurgaon to ask Tapha to fetch me at 05.30 pm; and there Tapha came with a man whom he called his body guard. They both have very strong sweating smell which made me almost choked to death. I just sat quietly till I arrived at Ramada Gurgaon and paid Tapha 1300 rupees (+ USD 32) for the transportation I made with him that day.

The next morning, Tapha was totally 100% changed. He came alone and became so very polite and full with smile. He asked me to talk to his wife who is Indian and has two master degree and of course she speaks very fluent English much better than me. After talking in the phone to his wife then I chat with Tapha and from his broken English I knew that he has one beautiful 9 years old daughter and 3 of his siblings married to different nationalities such as married to a Malaysian, a Kuwaiti, a Saudi Arabian and he himself married to a beautiful and smart Indian lady. Well, that was really a beautiful incredible story.

Along the way back from Ramada Delhi to Ramada Gurgaon I sang along with Tapha a beautiful Indian song entitled Kuch-Kuch Hota Hae which is originally sung by a famous Indian actor Shah Rukh Khan. It was like an ice breaking song that we both enjoy. We sang and dance along the way and every car just looked at us dancing and singing and beep us to be careful because we disturbed other cars to pass by.

It was a pity because the next day, I had to go with the embassy van to visit the famous Taz Mahal, and the day after that I had to go back to my beautiful country Indonesia. So no more Tapha.

It takes 4 hours to go to the Taz Mahal from Ramada Gurgeon. I had to pay about 12,000.- rupees (+USD 300.-) if I use the hotel taxi (not Tapha taxi), or I pay only USD 75.- per person if I use tourist bus, that is already included lunch and entrance tickets. With the embassy car the two of us (me and my colleague) pay only USD 150.- for the tip, since 2nd September was a holiday in all over India as they celebrating the Khrisna day.

Along the way to Taz Mahal, we pass by the country sides. They are all green but also dusty and rocky. The Northern parts of India are quite dusty because they are close to Gobi desert, one of the largest desert in the world. Sometimes, we can see sky scrapers enacted in the middle of nowhere. The way they built them there were to create equal distribution of wealth across the states.

Not many houses exist along the way. With 1.2 million people, India seems so quiet. Most Indians seems tend to live in the cities rather than in the country sides. Well that is my observation at glance and that may not be 100% correct. (*PS: Later I found from my Indian friend that only + 35% Indians living in the cities)

The road to Agra is full with trucks. Well, I am sorry to say that most of the trucks or vehicles in India seemed old but still work well. The bus there all surrounded by concrete bars, so it seems like a jail bus to me, but they say it was because India has a large poor population and the majority of them use buses as main transportation, so in order to avoid them from falling out of the bus, they put bars in all of the windows, so that they would be more secure inside. The famous public vehicle is the three wheels Bajaj. (We also have Bajaj in Indonesia, as one of our public transportation in the cities. We imported a lot from India, but the size of the Indonesian Bajaj is smaller than the Indian one). What amazed me is the Indian Bajaj fit for 12 persons and still moving, whereas in Indonesia may be for 6 or 7 person can fully pack in one roof. Wow, another incredible story!

Agra is one of the cities in Uttar Pradesh (UP) state of India. UP has about 130 million people, who live in trade, crafting, agriculture and mining. The majority of Agra people is Moslem, but they seem so poor, or at least that was I observed along the way. The Taz Mahal built by King Shah Jahan (a Moslem King) in 17 century as a dedication to his most beloved queen Mumtaz. (The King himself has + 200 wives, according to our driver). Incredible! It took 22 years to build such a magnificent and beautiful landmark. As everyone knows, Mumtaz died when delivering her 14th baby much earlier before the Taz Mahal itself finished.

The king himself then murdered by his own son who disagreed with his royal father lavish live and plan to built another Taz Mahal for himself. If you go to Taz Mahal compound, there is no other Taz Mahal or the black Taz Mahal around. Every building in Taz Mahal compound was built asymmetrically and in harmony with its surrounding. The buildings erected in the edge of the river, so it has incredibly beautiful natural landscape in itself.

Made of first class marble and crafted and decorated with semi precious stones such as Lapis Lazuli , Malahit etc, it makes Taz Mahal one of the world expensive and grandeur royal tomb ever exist man made!

The King and his Queen buried in the ground of Taz Mahal. What we can see in the first floor or the entrance is the replica of their tombs. The entrance fee is 750 rupees for an adult, that is included a bottle of small mineral water and a pair of wet shoe cover since the temperature around Agra could reach up to 50 centigrade degree Celsius we need the shoe cover to stand the heat. No chewing gum allowed into the Taz also no pictures taken inside the building in order to maintain the color of the stone. The marble wall itself is translucent, it gives white color in the morning, yellow to light orange in the afternoon and blue in the evening. When we pointed out our light or lit the matches to the wall, the light can go through the wall, so it is magnificently incredible!

There are many professional photographer around the The Taz Mahal, so we don't have to worry if we forget to bring our camera along. It will costs us only 700 rupees for ten pictures, so its no money at all considering the professional result we could get. The pictures will turn to be very fantastic!

On the way back to the hotel, our tour guide, the one who has been recommended by the embassy, lead us to the handicraft shop, who sells some souvenirs made of translucent marble. The ancestors of the handcraft men were the one who crafted the Taz Mahal, so according to our guide they produce only high quality handicrafts, but the price is extremely expensive, especially for a civil servant like me. I bought the cheapest one, not so nice, but it cost me already USD 150.- only for a simple marble plate and small elephant crafts. Gee...that was only some I could bring from India since I had a condense meeting and must commute every day from Gurgaon to the meeting venue in New Delhi. I didn't even know how New Delhi actually looks, but I thanks God and my government and myself because of the small mistake I made in booking the hotel, I was then able to commute and see how the real India looks and I could take some pictures which you can see in this website.

I return home on 3rd September to Indonesia and stayed over night in Bangkok Thailand. I bring along a beautiful and incredible memory of India which I will treasure the beauty of India until I die.

Does menopause ever end? Menopause is the cessation of menstrual periods, when you no longer ovulate and the ovaries stop producing estrogen. It is a forever thing. These symptoms, unlike menopause itself, don't usually last forever, and they can vary in duration and severity from woman to woman.
What is the best menopause medicine? Treatments may include: Hormone therapy. Estrogen therapy is the most effective treatment option for relieving menopausal hot flashes. Vaginal estrogen. Low-dose antidepressants. Gabapentin (Neurontin, Gralise, others). Clonidine (Catapres, Kapvay, others). Medications to prevent or treat osteoporosis.
How long should you take HRT? For the vast majority of women under 60 years, the benefits of HRT outweigh the risks. HRT is also recommended for young women to take following an early menopause and they need to take it until they are at least 51 years. There is no set length of time that you should take HRT for.
Can you get a false positive pregnancy test in perimenopause? The hormone can remain in your blood and urine for up to six weeks following the end of the pregnancy. It's possible to have a false - positive test until your hCG levels return to their prepregnancy state.

Cephalometric evaluation of Brahmins of Kathmandu, Nepal based on Jarabak’s analysis

Background: The cephalometric norms derived from Caucasian population shows a great degree of variation when applied to different population.

Objectives: The present research was performed with the objective of determining cephalometric norms of Brahmins of Nepal based on Jarabak’s analysis, to evaluate the variability between male and female and to compare the cephalometric norms with Caucasians.

Methodology: Lateral cephalogram of 108 non-growing patients which consisted of 63 females and 45 males of
Nepalese Brahmins who met our inclusion criteria were traced manually. Thirteen craniofacial parameters of Jarabak’s analysis were measured.

Results: The comparison between Brahmin populations of Nepal with the Caucasian population showed statistically significant difference in ten out of thirteen parameters. The comparison between Nepalese male Brahmins and female Brahmins showed that there were statistically significant differences in eleven out of thirteen parameters.

Conclusion: It can be concluded that there exists sexual and ethnic dimorphism in craniofacial features. The findings of the study highlight the fact that Brahmin population is significantly different from Caucasian population. Females presented with smaller craniofacial variable than male when gender comparison was performed among Nepalese Brahmins male and female. This research has helped to generate knowledge and understanding of craniofacial features of Nepalese Brahmins and derived norms for orthodontic diagnosis and treatment planning.

Use of C-reactive protein level in diagnosis of tubercular pleural effusion

Background: C-reactive protein in both pleural fluid and serum has been found to be higher in tubercular pleural effusion than in other causes of pleural effusion.

Objectives: The main aim of this study was to find out the diagnostic value of C-reactive protein in patients with
lymphocytic pleural effusion.

Methodology: A cross-sectional study was conducted in 90 patients with pleural effusion who underwent thoracocentesis at Kathmandu Medical College Teaching Hospital, Kathmandu, Nepal. The complete biochemical tests of pleural fluid and serum were performed. The C-reactive protein concentrations of both pleural fluid and serum were then measured from samples from patients with lymphocytic exudative pleural effusion.

Results: Ninety patients with exudative lymphocytic pleural effusion were included. Male patients were 56 (62.2%) and female were 34 (37.8%) with the male to female ratio of 1.64. Mean age of the patients was 51±21.54 (Mean ± Standard Deviation). The pleural fluid C-reactive protein levels in tubercular pleural effusion were higher (48.87±24.19 mg/dl) compared to non-tubercular group (38.30±17 mg/dl; p<0.001). Similarly, the serum fluid C-reactive protein levels in tubercular pleural effusion were higher (29.60±13mg/dl) compared to non-tubercular group (18.14±9.2mg/dl; p< 0.001). The sensitivity of pleural fluid C-reactive protein level in diagnosing tubercular pleural effusion was 86%.

Conclusion: Simple and inexpensive test like C-reactive protein is useful in the diagnostic workup of lymphocytic pleural effusions. High C-reactive protein levels are very suggestive of tubercular pleural effusion.

Transthoracic echocardiography in patients with chronic kidney disease

Background: Evaluation of cardiac diseases in chronic kidney disease has been rarely investigated in Nepal.

Objectives: Objective of this study is to evaluate cardiac lesions in admitted chronic kidney disease patients.

Methodology: It is a prospective observational study of echocardiography of chronic kidney disease patients from April, 2007 to April, 2013 in Nepal Medical College Teaching Hospital.

Results: One hundred chronic kidney disease patients were evaluated. Male to Female ratio was 1.8:1 and age ± SD was 46.3 ± 17.2 years. Forty eight percent of the chronic kidney disease patients had left ventricular hypertrophy. Patients with chronic kidney disease with left ventricular hypertrophy group had interventricular septum of 1.5 ± 0.3 cm vs. 1.1 ± 0.1 cm (p<0.0001) and posterior wall of 1.1 ± 0.2cm vs. 1.0 ± 0.1cm (p< 0.01) in comparison to chronic kidney disease without left ventricular hypertrophy. Forty one percent had left ventricular systolic dysfunction with left ventricular ejection fraction of 39 ± 9.9 %. Pulmonary arterial hypertension was noticed in 39% patients. Valvular regurgitant lesions were quite common (24.1%) usually as multivalvular lesions (4.4 lesions per patient). Mitral regurgitation was the commonest regurgitant lesion (81%).

Conclusion: Echocardiographic cardiac evaluation is useful to diagnose concomitant cardiac lesions for standard care of chronic kidney disease patients.

Correlation of peak expiratory flow rate with body mass index in school children of a government school of Kathmandu

Background: Peak expiratory flow rate is a reliable data to assess the severity and effectiveness of management in adolescent with asthma. There is no study done in Nepalese children to relate it with the body mass index. The present study tries to find out its relation with age, sex, weight, height, body mass index.

Objectives: To identify the effect of body mass index on peak expiratory flow rate among healthyNepalese school children.

Methodology: Three hundred and ten students of a government school at Kalimati wereincluded in this study. Highest of the three peak expiratory flow rate readings were recorded using Wright’s peak flow meter. Questionnaire were used to find out the pre-existing disease conditions for exclusion from the study population. Weight and height were measured using standardized instruments and body mass index was calculated using formula: weight (kg)/height2(m).

Results: Peak expiratory flow rate initially increased with increasing body mass index from -1SD to +2SD and it declined as body mass index increased above +2SD. Pearson’s correlation r was positive with values of 0.7, 0.65, 0.64, 0.35 for height, weight, age and body mass index respectively. Height had highest positive correlation with peak expiratory flow rate while body mass index though had positive but weak correlation with peak expiratory flow rate.

Conclusion: Body mass index is positively correlated with peak expiratory flow rate. However, mean Peak expiratory flow rate initially increases with increasing body mass index and it starts declining as body mass index reaches overweight range.

L'ottava edizione della rassegna è dedicata all'Africa. La serata inaugurale si terrà il 13 novembre alle 20.30 al teatro del Verme: "Convivenze" con Fernando Aramburu, Paolo Giordano, Michela Marzano e Simone Savogin.

Remittances and diversification in Timor-Leste

Last year remittances became Timor-Leste’s largest non-oil source of income, before coffee but after aid. There is success and failure in this strange fact. In their willingness to back themselves by working abroad, the people of Timor-Leste have proven, again, how determined and adaptable they are. The quality of the economic management that led to a situation where many of this remarkable nation’s most highly educated youth feel that picking fruit in Australia is the best option open to them is more open to debate.

The idea that Timor-Leste’s economy needs to be diversified has been a mantra around Dili for decades – a particular favourite of virtually every NGO and foreign development agency that has ever operated there. Tourism and agriculture (including, but not limited to coffee) could have become the basis for a sustainable and relatively equitable economy, but it just hasn’t happened. Education, health, agriculture and water accounted for less than a fifth of state expenditure in the 2019 budget. Why, is neither unique nor hard to understand. As oil money filled Timor-Leste’s coffers from 2011, people have understandably done whatever they could to stake a claim, either directly or through various sorts of patronage networks. Despite the work of some inspired leaders and thinkers, the critical mass of political will to make the creation of a successful non-oil economy a priority isn’t there. The result of this is that many people who, given their choice, would prefer to make a living at home, feel that they no longer have a choice but to work abroad.

People from Timor-Leste have been going abroad to work since independence in May 2002, when those born during the 1975-99 Indonesian interregnum were granted, via Portuguese citizenship, the right to work in Europe. Many of them ended up in the United Kingdom. Encouraged by this, Dili has been relatively effective in its pursuit of bilateral agreements allowing Timorese to work in South Korea and Australia, and there are rumours that similar avenues to work in Japan and New Zealand might open up in the near future. What is surprising, or perhaps just disappointing, is how this income source has attained such a level of economic importance. For the majority who are unable to find some sort of place on the government payroll, there is very little else. People going overseas to seek a better life is a good thing. People being forced to go overseas for a complete lack of other options is not.

Recently I spoke to a young man in Dili, Xisto, whose comments threw this situation into relief. Having helpfully introduced me to some of his family who had just returned from Australia he told me that he was now hoping to go himself. While his relatives had done short-term fruit picking with the Seasonal Workers Programme, he wanted to sign on with the Pacific Labour Scheme – three years working in an abattoir.

Me: You can save a lot of money, but it’s heavy work and you will be in Australia for ages.
Xisto: OK Mike, but it’s difficult to find a job in Timor-Leste. I really need a job in order to get money to support my younger brother to continue his school and also feeding my family.

My friend is an exceptionally affable and hard-working individual. A university graduate, English enthusiast and veteran of any number of workshops and courses. And for him the best option is chopping up meat in Australia. One would be tempted to talk about a brain drain, but that would assume that he had even an outside chance of finding stimulating work in his homeland, which there absolutely is not.

The number of Timorese working abroad is only going to grow. Although accurate figures are hard to come by it is estimated that there are no more than 20,000 resident in the United Kingdom, with smaller numbers elsewhere. From a population that is approaching 1.3 million this is still a relatively small overseas diaspora compared with places like Nepal or the Philippines. Even so, the implications of this trend are a surprisingly divisive issue. People (including myself) have written glowingly of the potential for funds from abroad to stimulate grassroots development. Others talk of a remittance trap that, ‘can spark a vicious cycle of economic stagnation and dependence’. The truth, as usual, seems to be in between.

Domestically, tourism, coffee and agriculture remain Timor’s hopes. Improved health, education and connectedness remain its great needs. If the state becomes serious about prioritising development, money and ideas remitted from abroad would have the potential to enhance these sectors and perhaps open up new ones, seeding businesses and contributing to a healthy and well-educated new generation. The alternative, which seems to be the direction things are headed, is that Dili’s policy makers continue to avoid seriously confronting potentially existential social and macro-economic issues.

In either case, labour sending will become increasingly important to how the people of Timor-Leste live. But no country has got prosperous on the back of labour mobility. As increasingly important as labour mobility is becoming for Timor-Leste, its prosperity and prospects will continue to depend on its domestic policies.

Michael Rose will present a seminar 'Overseas work and national development: a new path for Timor?' in Dili at the National University of Timor-Leste Auditorium on Thursday 7 November at 4pm.

Kathmandu & Timezone Oddities

Daylight savings just recently went into effect here in the United States. Normally "gaining" an extra hour as we shift into standard time is a reason to rejoice this time of year. We all pretend we get an extra hour of sleep that night, as though we're not just making this time thing up as we go along. The timing for me was a bit humorous though as I'd only just started to get over my jet lag from a month and a half of traveling for conferences and things. Pushing everything back an hour really makes all the difference!

But that's less interesting than what I learned while I was presenting at Web Weekend Kathmandu in late September. It turns out that Nepal Time uses UTC+05:45.

If you look at the list of UTC time offsets you'll find that most countries have rounded to a full hour. From the perspective of someone who thinks a lot about coordinating with others halfway around the globe, this seems most pragmatic. A smaller list of places use an offset that falls along the half-hour, though the largest of these seems to be India at UTC+05:30. It seems wrong to describe anything in India as "small" since over a billion people live in and use that timezone offset.

Nepal however, as far as I can tell, seems to be the only country in the world currently aligned to an offset that doesn't fall evenly on the hour or half-hour. There are a couple tiny places in Australia and New Zealand that seem to also align to a 45 minute offset, though I think they're quite small—the population for one town was 53.

It might sound silly but trying to coordinate and think about the time back home while I was there was surprisingly disorienting! If you go through the history of timezones a lot of areas have changed their timezone to round to an even hour, I have to think just because it's easier to coordinate with the rest of the world that way.

So... there you go! Timezones. Someday, maybe, we'll abolish timezones all together. I'm not sure when that day will come though—so much of our culture and language, at least in the Western world, revolves around time in a relativistic way it would be hard to unravel. I have higher hopes of getting the U.S. to adopt the metric system before that happens.

WASHINGTON DC, September 20, 2019 - This week, Education Above All Foundation (EAA) and the World Bank announced a ground-breaking partnership to enrol two million out of school children from more than 40 countries by 2025. During a meeting with World Bank President David Malpass, Her Highness Sheikha Moza bint Nasser, Founder and Chairperson of Education Above All Foundation, stressed the importance of this framework agreement.

The agreement commits up to $250 million in funding for developing countries striving to enable access to quality primary education for all of their still out-of-school children. Unlike traditional philanthropic efforts of organizations like EAA who usually fund local non-profits directly, this innovative funding model aims to take lessons learned in the field to scale, through direct support to participating countries with implementation, evaluation, and reporting - enabling accountability and systemic change at the national level.

Out of school children (OOSC) are among the hardest to reach in each country due to the many and often compounding barriers to education including extreme poverty, distance to school, and conflict. This new agreement calls on governments to utilise funds to prioritise out of school children by ensuring their access to quality primary education through results-based financing. The agreement highlights the importance of multi-stakeholder partnerships in supporting developing nations, in providing education for all, and meeting the UN Sustainable Development Goals, particularly SDG 4 (ensuring inclusive and quality education for all and promoting lifelong learning).

"The World Bank is committed to addressing the global learning crisis. The partnership with Education Above All is critically important in this effort. There are still too many out of school children around the globe. Together we will bring these children into school and help them learn and fulfil their potential. Learning for all is a foundation for building strong human capital for every country," said Jaime Saavedra, Global Director for Education at the World Bank.

"Our partnership with Qatar and Education Above All will play an especially important role in the Middle East and North Africa," said Ferid Belhaj, World Bank Vice President for the Middle East and North Africa. "As access to quality education is critical for the region to unlock the huge potential of its large youth population, whose energy and creativity could become a new source of dynamic and inclusive growth."

Through this new funding structure, EAA and The World Bank will support financing opportunities for resource mobilization, education advocacy, and poverty reduction in developing countries across three continents. Proposed targeted countries include Afghanistan, Bangladesh, Burkina Faso, Cambodia, Cameroon, Chad, Democratic Republic of Congo, Djibouti, Ecuador, Ethiopia, Haiti, India, Iran, Iraq, Kenya, Laos, Lebanon, Liberia, Libya, Malawi, Malaysia, Mali, Mexico, Morocco, Mozambique, Myanmar, Nepal, Nigeria, Pakistan, Paraguay, Philippines, Rwanda, Senegal, Sierra Leone, South Sudan, Sri Lanka, Syria, Tanzania, Thailand, Uganda, and Zambia.

Oggi stiamo aiutando e educando oltre 400 studenti.

How extreme events impact Earth’s surface: reports from the 6th EGU Galileo conference

Throughout the year, EGU hosts a number of meetings, workshops, and conferences for the geoscience community. While the EGU’s annual General Assembly brings more than 15,000 scientists together under one roof, the EGU Galileo Conferences allows a smaller number of scientists to discuss and debate issues at the forefront of their discipline. In this blog post, the organisers of the 6th Galileo Conference “Perturbations of earth surface dynamics caused by extreme events” reflect on a week of insightful presentations and discussions on rare and catastrophic events. “How do extreme events perturb Earth surface dynamics?” This question kept us busy during the entire week of the 6th EGU Galileo Conference “Perturbations of earth surface dynamics caused by extreme events”, which took place in Nepal from 13-19 October 2019. As organisers, we had aimed for a slightly unusual conference venue. We kept the nice hotels to a minimum of two nights and took the participants out to the Bhote Kosi for some camping for the remainder of the week to foster discussions and idea exchange. The Bhote Kosi valley, about four hours’ drive north east of Nepal’s capital city Kathmandu, was heavily impacted by the April 2015 Gorkha earthquake and a subsequent glacier lake outburst flood event in 2016. This valley still today carries the signs of these earlier events in the form of large landslides, unstable slopes, and reworked river beds. As such, the valley serves as an ideal natural laboratory to better understand and quantify how the Earth’s surface responds to such perturbations. The Bhote Kosi had been a basecamp for a number of us studying natural hazards during the multiple field campaigns organised after the Gorkha earthquake, and this conference was a great opportunity to share what we have learned over the past years while directly illustrating the conference topics. What seemed easy in the early days of planning did not come without inevitable doubts as the conference came closer. How do we make sure we have enough tents for everyone, how do we deal with the frequent power cuts, how do we make sure to cater enough local beer to thirsty geoscientists, and what if everyone contracted food poisoning? Fortunately, 60 participants, including ten Nepali colleagues and many early career scientists, blindly followed us without much afterthought and we were off for a busy and promising week. The talks and posters covered most extreme event triggers: from earthquakes to volcanic eruptions and from wildfires to storms and tsunamis. These presentations provided food for thought for the geomorphologist, the geochemist, and the seismologist alike. Nepal, with the aftermath of the Gorkha earthquake, was well represented in these presentations, but many other parts of the world were covered as well. Last day at @EuroGeosciences Galileo Conference #EGUNepal. Check out our poster hall! — Kristy Guerin (@KristyGuerin) October 18, 2019 Overall, this conference demonstrated the role of extreme events as geomorphic actors, able to shape landscapes and affect biogeochemical cycles. This conference also highlighted the large range of possible geomorphic responses, both in terms of magnitude and spatial extent, suggesting that the question of how these extreme events should be defined (are they large or are they rare events?) should ultimately be left to the investigators. It is however clear that in terms of geomorphic impact, an extreme event should lead to an observable perturbation above a, to-be defined, background variability, and be followed by a recovery period that leads to an old or new steady-state. As such, extreme events are not created equal and future research is needed to understand why such a range of responses are encountered. Conference attendees had the opportunity to discuss questions and topics at the forefront of their field, from ethics in science to international cooperation. (Credit: Monique Fort)  Time for discussion also allowed us to debate on the morality of post-disaster scientific work. We concluded that basic research questions related to these events need to be pursued and frequently require immediate mobilisation of scientific equipment and personal. However, this discussion also highlighted the need for clear and transparent international coordination so as to not interfere with relief efforts and avoid being perceived as greedy ambulance-chasing scientists. This important discussion was backed by input from a large Nepali delegation, providing an insight into how they had perceived these questions directly after the recent earthquake. Further discussions focused on the commonalities of different extreme events and the possibility to define a common framework that would allow us to compare the geomorphic impact of an earthquake to that of a storm or a wildfire. Finally, this conference allowed us to lay the foundation blocks for future international coordination efforts. While the exact contours remain to be defined, all participants emphasised the need to prioritise research questions and resources in the case of rapid response efforts. These efforts require clear coordination with affected countries and funding bodies, but for instance also encourage scientific actors to agree on common publication strategies upfront. Conference participants tour the Bhote Kosi valley to learn more about how extreme events can shape landscapes. (Credit: Monique Fort)  In the middle of this busy schedule, a day of field excursion provided a welcome change. From small to large, the Bhote Kosi has it all: boulders, landslides, debris flows etc… Driving up the valley all the way to the Nepal-China border provides a humbling experience of how these idyllic landscapes can be turned into deadly traps in the blink of an eye. With closer scrutiny it becomes obvious that the whole landscape has been shaped by a myriad of these catastrophic events, directly questioning the notion of extremes. After six days of presentations, posters, and late night discussions, it was time to close this intense, yet educational week. In the end there weren’t too many power cuts, no one got sick, most of us managed to shower with hot water and only a few reported spiders in their tents. In line with the local Nepali customs, the end of the conference was celebrated by inspired dancing until late at night when the first shuttles back to the airport started to take people back to Kathmandu. By Maarten Lupker, ETH Zürich, Switzerland Acknowledgments This conference was jointly organised with the Nepal Geological Society (NGS), without which this week would have never existed. While many people were involved, we would like to extend special thanks to Basanta Raj Adhikari and Ananta Prasad Gajurel from Tribhuvan University as well as the former president of NGS, Kabi Raj Paudyal and the present one Ram Prasad Ghimire. Bhairab Sitaula also provided invaluable help in all logistical aspects of this conference. The conference was also co-sponsored by the US National Science Foundation, which provided overseas travel grants. Support from DiGOS & GFZ Potsdam were also greatly appreciated. The organiser team: Christoff Andermann, Kristen Cook, Sean Gallen, Maarten Lupker, Christian Mohr, Ananta P. Gajurel, Katherine Schide, Lena Märki

Compass crowns inaugural Barista Champion 2019

The UK’s largest food services provider and fourth biggest coffee retailer in the UK, Compass Group UK & Ireland has held its inaugural Barista Championships, which saw four finalists from across the Compass portfolio take part in a highly competitive championship final. The competition was designed to mirror the ‘World Barista Championships’ set-up, to prepare Compass baristas to compete nationally. Razvan Cojocaru, from Restaurant Associates, was crowned winner following two intense rounds, where he demonstrated strong technical ability as well as complex coffee knowledge in his skill, with strong professionalism throughout. Razvan used a coffee from Costa Rica Cordillera Del Fuego which was processed using an anaerobic fermentation method and roasted by the team at Roastworks coffee. Razvan explained that the coffee was like “liquid apple pie”. Competitors: Pariwartan Nepal, from Eurest, Thales Pona, from Restaurant Associates, Arthur Luccats, from Compass Ireland and Razvan Cojocaru seized the opportunity to demonstrate their skills. The competition focused on three key areas: Espresso, milk and a signature drink. All baristas were judged against SCA competition guidelines, using their rules, regulations and score sheets. The final took place at Compass’ SCA accredited premier training campus at the company’s Chertsey HQ, complete with an esteemed panel of judges flown in from Italy and Switzerland as well as the UK. The judging panel consisted of Robert Ward, Cimbali – Technical Judge, Lauro Fioretti, Simonelli Group and Andre Eiermann, UCC – Sensory Judges. Having competed and judged at UK and World Championships themselves, the panel of judges were best placed to survey and assess Compass’s baristas skills. In addition, Compass Coffee Education and Training Manager, Sonali Tailor oversaw the judging team as head judge. The winner received a trip of a lifetime to an Origin’s coffee farm in Costa Rica. Razvan will get to see first-hand where the humble coffee bean starts its journey, as well as the social community projects at origin. Steve Lovegrove, Head of Business Excellence, Compass Group UK & Ireland, said, “The standard and quality of the baristas competing has been exceptional, congratulations goes out to each and every one of the competitors who have put so much into this competition. 'It clearly demonstrates the great talent we have in our business and it makes me extremely proud that we can showcase the skills and talents of these individuals.” Owen Thom, Education Business Development Manager, The SCA added, “It was a great honour to be a judge in the heats for Compass Barista Championships. 'Seeing all the talent on show, and the passion of all participants is a credit to the competition and the coffee education that the Barista Academy has in place. I look forward to seeing the journey of the winner and all the candidates.” Razvan Cojocaru, Compass Barista Champion 2019, said, “Thank you so much to everyone who has supported me and this competition. I feel so grateful to have won and cannot wait to fly to Costa Rica. 'This has been such an amazing experience from beginning to end, and I feel so proud to be a part of the Barista Academy, it has given me the confidence and opportunity to take part and I can’t wait for more competitions.” Prior to the inaugural final, baristas from around the Compass business took part in heats where competitors were tasked to present two espressos and two milk beverages of their choice within 7 minutes.

Biogen expanding biosimilars deal for eye therapies, anti-TNFs

As it continues to rely on biosimilars as a growth driver, Biogen has expanded its deal with Samsung Bioepis to include two new ocular biosimilars in certain major markets, as well as rights in China to anti-TNF therapies.

Biogen Inc. (NASDAQ:BIIB) will pay $100 million up front to Samsung Bioepis, the JV it shares with Samsung BioLogics Co. Ltd. (KOSDAQ:207940). The JV is eligible for $210 million in milestones, plus shared revenues from sales. Biogen may also opt to pay Samsung Bioepis $60 million in order to extend by five years its existing deal for European rights to the JV's anti-TNF portfolio.

Biogen will receive territorial rights to SB11, a biosimilar of Lucentis ranibizumab; and SB15, a biosimilar of Eylea aflibercept, in the U.S., Canada, Europe, Japan and Australia. Both of the reference drugs, which target VEGF-A, are approved to treat age-related macular degeneration, diabetic retinopathy, diabetic macular degeneration and other ocular disorders. SB11 is in a Phase III study, while SB15 has yet to reach the clinic.

Wednesday's expanded deal also gives Biogen rights in China to three biosimilars the company already markets in Europe: Benepali etanercept, a biosimilar of Enbrel; Flixabi infliximab, which references Remicade; and Imraldi adalimumab, a version of Humira. All three drugs are for autoimmune indications.

Samsung Bioepis will be responsible for manufacturing and development of the drugs. Biogen will hold their commercialization rights in the territories.

Biogen credited its biosimilars business with driving growth in recent quarters. The segment has delivered about $542 million over the first nine months of 2019, in part on the strength of Imraldi's launch; Biogen had posted $545 million in biosimilars revenue during full-year 2018.

Although Biogen owns nearly half of Samsung Bioepis' equity, the JV has partnered its biosimilars with other biopharmas. Merck & Co. Inc. (NYSE:MRK) holds territorial rights to biosimilars including Ontruzant trastuzumab, Hadlima adalimumab, Reflexis infliximab and Brenzys etanercept.

In China, 3SBio Inc. (HKSE:1530) holds rights to SB8, a biosimilar of Avastin bevacizumab, while CBC Group launched Affamed Therapeutics (Shanghai) Co. Ltd. in February to house local rights to a biosimilars portfolio including SB11 and biosimilars of Soliris eculizumab and Herceptin trastuzumab.

Biogen exercised an option in June 2018 to enlarge its ownership stake in Samsung Bioepis to 49.9% from 5.4%. The parties launched the JV in 2012.

Speaking at an Association for Accessible Medicines conference Wednesday, Sang-Jin Pak, SVP and head of Samsung Bioepis' commercial division, stressed the role of biosimilars in reducing prices, accused originator biologics manufacturers of spreading misinformation about biosimilars, and predicted consolidation in the biosimilars space.

Manufacturers of branded drugs and biologics “will never on their own decrease prices -- never, ever,” Pak said. “Only with the launch of biosimilars or with small molecule generics will you see price competition and price reductions.”

He called for government agencies and biosimilar manufacturers to step up education efforts to counter “fearmongering” about biosimilars from original biologics manufacturers.

Pak, along with other representatives of biosimilar developers who spoke at the conference, predicted that decreasing prices for biosimilars will present challenges that are likely to cause some companies to reconsider their commitment to the space.

“This market is going to be very, very different in the next five years than it has been in the last three years,” Pak said. “Prices will be much lower” and only “a handful” of biosimilars companies will be able to survive.

BWorld 370, Universal healthcare via lower medicine taxes and tariffs

* This is my article in BusinessWorld last September 19, 2019.

Among the ironies of government health policies regardless of administration is their cry for “cheaper medicines” — and then they impose various tariff and taxes on medicines that make these products more expensive.

I checked the tariff and duties for imported pharmaceutical products at the World Trade Organization (WTO) and I was surprised to see that zero tariff in medicines is imposed by a number of our neighbors in the region while the Philippines imposes a nearly 3% tariff, aside from 12% VAT on medicines (See Table 1).

Other countries have higher medicine tariffs: Nepal has 14%, Pakistan 11%, Laos 10%.

This coming Monday, Sept. 23, there will be a UN High-Level Meeting (UN HLM) on Universal Health Coverage (UHC). Their theme is “Universal Health Coverage: Moving Together to Build a Healthier World.”

The UN and WHO send this virtue signaling to member-country governments to further raise taxes, impose more prohibitions and restrictions on “unhealthy” products to achieve a “healthier world.” So that the Philippines’ Department of Health, Department of Finance, other agencies create new legislations and regulations to implement this signaling.
But people around the world have been living healthier and longer, even before UHC was coined and before various taxes or tax hikes on alcohol, tobacco, sugary drinks and food were imposed (See Table 2).

Related to this, a new report was released this week, “Accelerating access to medicines: Policy recommendations for achieving the health-related Sustainable Development Goals.” It was produced and co-signed by 15 independent and non-government think tanks from 14 countries including the Geneva Network (UK), Minimal Government Thinkers (Philippines), and four others from Asia.

The report has noted that government itself is among the causes of expensive medicines and thus recommended three ways to reduce medicine costs: reduce taxes, abolish tariffs, and eradicate other trade barriers. In particular, it recommended that “Non-members should join the WTO Pharmaceutical Tariff Elimination Agreement (‘Zero for Zero’ initiative). If this is not possible, countries that still levy tariffs should unilaterally abolish them.” Yes, the Philippines should reduce its double talk by cutting its pharmaceutical tariffs of 2.8% to zero (See Table 1 again).

Regarding improving access to medicines, the report also noted that government itself creates regulations that in the process delay or limit access to medicines by the people. Thus, the report made four recommendations: Speed up patent examination, simplify the drug approval process, modernize government medicine reimbursement decision-making, and promote open trade in medicines.

Open trade in medicines means allowing more market competition via: (1) a stronger role for the WTO in enforcing existing laws vs. mandatory local content requirements; (2) instead of protectionism, developing country governments should make their economies more attractive to foreign investment by among others, investing in human capital and physical infrastructure; and, (3) public procurement of medicines should be transparent, ensure best interests of the taxpayers.

As this column has repeatedly argued, cheaper products like energy, rice, transportation, healthcare and medicines is possible if government steps back via less taxes and tariff, less mandates and prohibitions, have more competition among producers and sellers of these goods and services. Government should only ensure good quality commodities from competing players by heavily penalizing producers of fake, counterfeit, substandard, and unreliable products.

Welcome message

The Vasco da Gama Movement invites you to be part of the first PanEuropean meeting for Trainees and Junior Family Doctors !

We are delighted to welcome you to the first Pan European meeting for trainees and junior Family Doctors, the Vasco da Gama Movement Forum, which will be held in the city of Barcelona on 7th-8th February 2014. After organising 8 succesful European PreConferences and co-organising 2 fantastic World PreConferences ahead of the Wonca Conferences between 2005-2013, and alongside preparations for the 2014 VdGM PreConference in Lisbon in July, the Vasco da Gama Movement is ready to take the next step !
We are organizing the VdGM Forum, a scientific conference rich in contents, under the title “One Strong Voice for the Family Doctors of the 21st Century”. The programme will be focused on the topics of the Future of Family Medicine, Emergency Medicine and Out-of-Hours Services, Career Development, Research Skills and Emerging Technologies.
Prior to the VdGM Forum, a vocational exchange will be organized for selected participants, who will have the opportunity to observe GP/FM practices (offices) in Barcelona and learn more about the national organisation of primary care.
We strongly believe that the progress of our movement depends on our colleagues, on whether they share our cause, and on the continuity of our platform; a platform in which members meet regularly and they have the chance to establish and develop good professional relationships, which are essential for launching exciting and aspirational new projects. Alongside trainees and newly qualified (5 years) General Practitioners / Family Doctors, we also welcome senior GP/FD, other specialities but also patients and undergraduate medical students.
For this very reason we are working with great enthusiasm so that you can enjoy a unique scientific and cultural programme.
We warmly invite you all to join us for an amazing conference; we are looking forward to meeting you in Barcelona!

Lorraine Cleaver is a Scottish wife and mother who has petitioned the Scottish Parliament for improved Thyroid diagnosis and treatment. The petition has benefited from the involvement of the Minister for Public Health and is progressing well. After her thyroid was removed in 2006 for Graves disease, Lorraine's health declined by an alarming degree, despite being under the care of an Endocrinologist. Her GP was desperate to help but the system of deferring to the consultant and guidelines made this almost impossible. She recovered her health by working alongside her GP but outwith the established Endocrinology model. Gathering evidence for the petition reinforced that the treatment model for chronic diseases is failing many thousands of patients who go on to develop worsening health and a reliance on polypharmacy. She is optimistic that putting patients at the center of their treatment will help to change this, improving patient's health and enabling GP's to have more autonomy in the patient's care.

Preconference: #DesignThinking for Primary Care Workshop

Design Thinking is about believing we can make a difference, and having an intentional process in order to get to new, relevant solutions that create positive impact.
  • It’s human-centred: it begins from deep empathy and understanding of needs and motivations of people
  • It’s collaborative: several great minds are always stronger when solving a challenge than just one.
  • It’s optimistic: Design Thinking is the fundamental belief that we all can create change.
  • It’s experimental: it gives you permission to fail and to learn from your mistakes, because you come up with new ideas, get feedback on them, then iterate.
It has five phases that help navigate the development from identifying a design challenge to finding and building a solution.
The Five Phases of the Design Thinking Process
The Vasco da Gama Movement is developing a fantastic workshop that will enable you to learn firsthand how Design Thinking can be used to innovate in health and healthcare. During this workshop, teams will work together to explore and create solutions to real healthcare challenges.
Design Thinking #1
The participants will work on problems identified and will use the Design Thinking methods to gain a deep understanding of the problem and develop innovative ‘human-centred’ solutions (ie. solutions that truly meet the needs of users). Methods will include user-interviews, observation, brainstorming and rapid prototyping, with the aim of creating solutions that combine human desirability and usability with business viability and technological feasibility.
The workshop will take place in Barcelona just before the 1st #VdGMForum, on 7th February from 9am to 2.30pm. There is only a limited number of places for this workshop. The participation in the workshop will be free of charge. Medical students, other healthcare professionals, people from other professional background and patients are welcome to apply for a place.
The deadline for applications is Tuesday 7th January.
Design Thinking #2

It is our pleasure to offer to all European GP trainees/junior GP visitors the opportunity to discover how GPs in and around Barcelona practice medicine, in the Conference Exchange in Barcelona, just before the first Pan European meeting for trainees and junior Family Doctors, the Vasco da Gama Movement Forum! Between 5-6th February, during the Conference Exchange, we will try to show you our art and science to be a doctor. Therefore, we offer 30 places for European visitors in primary health care urban or rural centres.
You will enjoy 2 days of rotation before the Vasco da Gama Movement Forum begins, which will be held in the city of Barcelona on 7th-8th February 2014. You can join us and be part of the social program we have prepared just to show the skills, knowledge, technology, art, languages, and awesome Barcelona which is always waiting for you.
We have limited places (12) for those who want to stay in the home of one of our tutors/GP trainers colaborators. Some of them live in rural areas within 2 hours of Barcelona, others in the city of Barcelona.
Also, if you want to stay by your own in the city, we can help you to find cheap accomodation in a nice hostel with good transport communication and location.
We are working with great enthusiasm so that you can enjoy an unforgettable experience that will not disappoint you ! Please take a look also at the detailed schedule of the Exchange, by clicking here.
If you wish to participate in the exchange, please contact your VdGM National Representative.

Unrelated picture of the sky.

Unrelated picture of the sky.

I kissed Isaac awake the other morning. Little kisses all over his face. He was warm and sleepy, his face scrunched up. He woke up stretching. I moved away from a furnace blast of morning breath. I held his hand as we walked down the stairs.

The way our house is built means that we are presented with a view of mountains and sky, first thing, every day. It is always different, depending on the clouds, the light, the colors. It is such a gift, as though God holds it out in two hands. Here. You did nothing to create this. It is just here, in all its glory, here it is. It makes me cry. It makes me shrink back because what I am finding is that I am not good at receiving abundance. I’m good at scrapping along in the muck.

That morning, the sky was pink with flecks of gold. Mist draped itself over the mountains. I held my little boy’s hand. I thought about how many times I have woken a child up by kissing his or her face. How much morning breath I have wrinkled my nose over. How many times I have made breakfast. Lunch. Dinner. Snacks. And they are growing, growing. I did nothing to create this. I get to be a part of it, though. Blessed, blessed me.

Money right now is tighter than it has ever been. It’s natural. The needs are expanding along with arms and legs and educations. I have a growing list of the things we will do when, when, when. Car and motorbikes fall apart. Things break. Teeth grow into places they shouldn’t. Clothes wear out. I pick food from the garden and sigh with relief when dinner is on the table again. I hunt for coins. We hustle. I write, and Chinua and I plan, and we figure it out. We squeak by in the space of a breath.

And then there is the contrast. The free view. The fruit falling from trees. The kisses.

Yesterday I bought vegetables in the market, and as I drove my motorbike along the road home, there was a miracle of wingbeats, light, and motion. A flock of egrets flew in front of me, following the road like I did, dipping down to my level and then back up again. They joined the road where it came from the military base, leaving just before I turned off. I kept waiting for them to fly away, but they stayed there, right in front of me, leading me, for about two kilometers. Large white birds, a vision of freedom.

At the bay between Eureka and Arcata, in Northern California, there are tall trees where dozens of egrets roost. When Chinua and I were first married, we loved those birds because they reminded us of here. Of fields in India, Nepal, or here in Thailand. We drove by in a community car, our newborn in the car seat, gazing out the window at trees full of white birds. Remembering, longing.

When we arrived in Goa, and everything was strange and wet, and I was pregnant and uncomfortable, the egrets were there to meet me.

So on this road, the birds felt like more. They felt like the breath of heaven. A note. A gift. A nod. They flew and flew. Eventually, right before my turn, they found a tree to roost in. I drove my old rattly bike home, past the eucalyptus trees, through the teak tree corridor, past the temple, and into my driveway. Kids spilled out of the house, asking questions, waiting for me to answer them. I took in the view. Nothing I have created. All I have been given.

File under "shouldn't really need an introduction," founding/former WOLF EYES member AARON DILLOWAY has a discography longer than my arm that runs the gamut from Nepalese field recordings to abstract ambience to manipulated beats and ear splintering harsh noise. 2013's Opened Door lands somewhere in the middle of that spectrum - haunting and calculated sounds, set to a distant lumbering beat on the first side and descending into bleak, resigned howls on the flip. If you want it pure, then I suppose it's best to go to the source.
